The original editorial team of Daniel Brockman and David Holt has been joined by Gert ter Haar to produce a new edition which considers the substantial progress that has been made in this important field, including the challenges of treating airway abnormalities linked to brachycephalism, the constraints of complex bony anatomy and rich vascularity of the head when performing radical surgery, and the treatment of trauma.
The book includes a number of new techniques, including dorsal rhinotomy and total ear canal ablation.
The BSAVA says operative techniques, instructive photographs and full-colour illustrations remain a key feature of the manual, which is aimed at the general veterinary practitioner.

Working in collaboration with researchers at University of Texas, Southwestern (UTSW) Medical Center, the team has succeeded in using gene editing techniques on dogs to restore the expression of the dystrophin protein that is absent in DMD patients’ muscles to levels that could be life changing.
DMD is the most common, fatal, genetic disease that affects children. The same disorder also occurs in many dog breeds. It is caused by mutations in the gene that encodes dystrophin, a crucial protein that helps protect muscle fibres and the heart when they contract. Patients with the disease produce effectively no dystrophin protein, so their muscles and heart become damaged during contraction, leading to inability to walk, skeletal deformities, breathing problems and heart failure. Patients with DMD have a life expectancy that does not go far beyond their twenties.
Researchers say that this preliminary research, published in Science, demonstrates an approach that could lead to an effective, in principle one-time treatment for DMD for the most common genetic mutations affecting human patients. Through a single intravenous injection, researchers have been able to modify the dystrophin gene in many muscles of the body, including the heart, with the result that some muscles now had almost the normal amounts of dystrophin protein.
The team, led by Eric Olson, Ph.D., professor and chair of the Department of Molecular Biology at UTSW Medical Center and founder and chief science advisor of Exonics Therapeutics, and Richard Piercy, Ph.D., MRCVS, Professor of Comparative Neuromuscular Disease at the RVC, identified a naturally-occurring mutation in the dystrophin gene in a dog brought to the RVC’s Small Animal Referral Hospital.
Working now with relatives of this first dog, the combined group has used a technique called ‘SingleCut CRISPR gene editing’, where the researchers target a specific region of the subject’s own DNA. Harmless viruses encoding the gene-editing protein CRISPR/Cas9 were used to express the enzyme in muscle to cut the dogs’ own DNA at a precise location in the dystrophin gene through use of a molecular guide. The body’s own processes then repair the cut, which results in removing the negative effects of the existing mutation.
The researchers have demonstrated that dogs treated in this way had restored ability to produce large amounts of the dystrophin protein in their muscles and heart, and in the dog with the highest dose, cardiac muscles were found to have restored dystrophin protein to up to 92% of normal amounts after the single treatment.
The team believes this is the first time that this form of gene editing therapy has been conducted after birth in an animal larger than a rodent. With further longer-term research, efficacy and safety testing, it is hoped that this breakthrough will have human application. This treatment may be a significant step toward developing a gene editing therapy to repair genetic mutations for DMD, as well as a range of diseases caused by genetic mutations.
Richard Piercy, Professor of Comparative Neuromuscular Disease at the RVC, said: "Researchers have been looking for an effective and durable treatment for Duchenne muscular dystrophy for many years. This SingleCut gene editing approach, pioneered by Dr. Eric Olson’s group, has the potential to be a one-time treatment that could restore dystrophin protein. It is hugely exciting that through a single intravenous injection we’ve been able successfully to modify the dogs’ own DNA so they start generating dystrophin protein again. There is more work to be done, but with this exciting breakthrough, we feel we are a significant step closer to finding an effective treatment.
"A great many genetic diseases affect humans, dogs and other species - many, like DMD - with tragic consequences and this form of gene editing treatment might well be applicable in many of them. We’re hopeful that we’re paving the way for future research into treating some of the most serious genetic conditions that affect us today."

The app allows trainers to store and share medical records and treatment plans, identify horses and, with a Bio Thermo chip, monitor temperature non-invasively.
In Ireland, the Irish Horse Racing Board (IHRB) requires every yard keep a Blue Book which has to be submitted monthly. In the Blue Book, every medication given to every horse is recorded, with the amounts, the name of the medication and its batch number all written in by hand. EquiTrace enables a trainer to email the blue book information direct from their mobile phone.
Vincent O’Connor, senior vet at Sycamore Lodge Equine Hospital on the Curragh, Ireland, says he has found the app very easy to use and particularly appreciates the way it suggests stand down periods required for intraarticular medication, where injections into different joints require different numbers of days withdrawal before competition. The experience of John Oxx, whose winner I Know I Can was disqualified because of a positive test for TCA (triamcinolone acetonide) on 2 July 2020, is a case in point. Dr Lynn Hillyer, IHRB Chief Veterinary Officer and Head of Anti-Doping, stated that the minimum recommended stand down time for TCA would be 14 days but added that the stifle is a complex joint and can be associated with longer excretion times than might be expected.
Kevin Corley said: "Keeping a horse’s identity straight might seem a simple thing, but when you have a yard where staff and horses can change frequently and records are kept in the office, far away from the stables and fields where the horses are, it is not as easy as you think. I remember a time when I was called out to genetically test four two year olds, each valued at over €50,000 because their registration documents had been lost before they had been filed and no one knew which one was which."

Bayer Animal Health has taken over the marketing and sales of the antibiotic Nisamox (tablets containing Amoxicillin and Clavulanic Acid), from Fort Dodge Animal Health.
Under the new arrangement, the design of Nisamox tablet packaging will change to reflect the change of ownership, however, Bayer says the quality of the tablets and the competitive pricing remain unchanged.
Nisamox tablets are licensed for use in cats and dogs to treat infections of the skin, urinary tract, and respiratory tract caused by susceptible Staphylococci, and also to treat enteritis and urinary tract infections caused by susceptible E.coli.

Bayer Animal Health has announced that it will launch Seresto, a new POM-V collar for cats and dogs that kills fleas and repels and kills ticks for up to eight months, in March.
According to Bayer, Seresto is the first treatment to provide repellency against ticks in both cats and dogs, and represents the fruit of a multidisciplinary effort incorporating the company's research into insecticides, polymers and veterinary parasiticides. The end result is a polymer-based collar that transfers the active ingredients to the lipid layer, from where they are spread by the animal's neutral oils.
Bayer Animal Health group product manager, Caroline Braidwood, said: "Seresto is simple to use on both cats and dogs. And because it is long lasting, it offers pet owners an alternative to those struggling to maintain a monthly routine. Bayer believes in innovation and Seresto is an enhancement to our product portfolio that meets the needs and requirements of pet owners and vets alike in the treatment of ticks and fleas."
Seresto contains two active ingredients - imidacloprid for fast action against fleas and flumethrin, which repels and kills ticks.

Bayer says that because of its repellent (anti-feeding) effect, Seresto reduces the risk of vector-borne disease transmission - a first for cats. The company also says that because the active ingredients are stored on the inside of the polymer, it doesn't matter if the pet is shampooed and the lipid layer stripped off - the active ingredients are quickly replenished by the collar.
Seresto is odour-free and has a safety release mechanism to prevent strangulation. It is available in three sizes, Seresto Cat for cats less than 8kgs, Seresto Small Dog for dogs up to 8kgs and Seresto Large Dogs, for dogs over 8kg.

Fourteen veterinary surgeons stood for election this year and 8,542 voted, a turnout of 24.5% of eligible voters. That compares with a turnout of 26.2% in 2020, 25.5% in 2019 and 22.7% in 2018.
Danny scored a total of 4,759 votes, leaving the other successful candidates Tshidi Gardiner, Colin Whiting and Louise Allum nevertheless trailing in his wake, with 3,228 votes, 2,957 votes and 2,368 votes respectively.
The VN Council election also had fourteen candidates standing for one elected place, the other having been taken by Susan Howarth RVN who was automatically re-elected as the only candidate standing at the time of the original deadline in January.
Donna Lewis was elected with 404 votes from those cast by 2,341 veterinary nurses, which amounted to a turnout of 12.4% of eligible voters. That compares with a turnout of 17.1% in 2020, 14.5% in 2017 and 10.9% in 2016.
All of those elected to either RCVS or VN Councils will formally take up their seats at the RCVS Annual General Meeting on Friday 9 July 2021.

Squamous cell carcinoma affects horses’ genitalia, eyes, or skin around the eyes. The tumours typically have a guarded to poor prognosis, and the therapeutic approaches available have variable success rates. Euthanasia on welfare grounds is necessary in a significant number of cases.
Before this new research was published, pathologists and researchers Dr Alejandro Suárez-Bonnet and Professor Simon Priestnall, both from the RVC, had already undertaken research which showed that equine squamous cell carcinoma affecting the penis frequently acquires the ability to degrade the extracellular matrix and become much more biologically aggressive2. This is known as ‘epithelial to mesenchymal transition’.
This new study, recently published in Scientific Reports, is the product of a multi-institutional effort, which used both “classical” anatomic histopathology assessment and artificial intelligence to demonstrate a correlation between chronic inflammation, equine papillomavirus infection and progression of equine penile squamous cell carcinoma.
Several additional cancer-related signalling molecules, which are important in researchers’ understanding of human penile cancer, were also studied and the results obtained will lead to a much better understanding and treatment of the cancer.
Simon Priestnall, Professor of Veterinary Anatomic Pathology at the Royal Veterinary College, said: “Equine squamous cell carcinoma is the second most common type of cancer in horses, with a variable prognosis, so we are delighted to have made this breakthrough that provides a greater understanding and can lead to more effective treatments for horses and their owners.
"The RVC has always recognised the value of collaboration, and Alejandro and I enjoyed working alongside the teams at KCL, the University of Edinburgh and UCL. This project shows what can be achieved when different organisations work together with a common aim and we hope the similarities between the tumour in horses and people can offer a true One Health benefit."
Dr Alejandro Suárez-Bonnet, Lecturer in Comparative Pathology at the Royal Veterinary College, said: "I am so pleased with the results of this study, which will hopefully pave the way for improved prognosis of horses diagnosed with equine squamous cell carcinoma.
"Our work is not over, however. Thanks to the help of a grant from the Horse Race Betting Levy, Simon and I are leading a Master’s degree project investigating equine penile, ocular and vulvar squamous cell carcinomas from a broader perspective.
"Our hope is to identify if variability exists between equine papillomavirus infectious status, morphological and microscopic difference, and cancer progression, with the aim of identifying potential therapeutic targets as well as diagnostic markers with prognostic implications for maximum benefit to the British equine population.”

In the survey of 354 owners, 65% thought their cats get on well with each other. However, 60% reported signs of disharmony, including: chasing (65%), swiping paws (53%), hissing (48%), keeping a distance from one another (31%), fighting and acting aggressively (27%), staring (30%) and sitting in front of resources to block access (25%).
In addition, some of the cats were so stressed that they would often scratch furniture or household items (46%), hide from their housemates (26%), meow excessively (20%) or spray (17%).

Inigo’s abstract entitled “Predictive value of cardiac remodelling on survival in dogs with persistent atrial standstill – the UK perspective” sought to determine the survival of dogs in the United Kingdom with persistent atrial standstill (PAS) after pacemaker implantation and the prognostic value of cardiac remodelling. The Congress Programme Committee were impressed by Inigo’s innovative presentation, praising the planning and execution of the multi-centre project.
Ditte’s presentation was entitled “Evaluation of a point-of-care dot Enzyme-Linked Immunosorbent Assay to detect antibodies against canine parvovirus, distemper virus and adenovirus before revaccination”.
Her study described the findings of a prospective non-inferiority study of a point-of-care semi-quantitative titer test performed on canine serum samples and compared to gold standard analysis. Sensitivity, specificity, positive and negative predictive values of the point-of-care titer test were determined for the gold standard titer cut-offs for canine adenovirus, canine distemper virus and canine parvovirus. Ditte was commended by the Congress Programme Committee for the quality of presentation as well as the practical and clinically applicable findings of the study.

The consultation, which was held by the College early in 2017, asked for the views of veterinary surgeons and nurses, animal owners, and stakeholders on the use of telemedicine in veterinary clinical practice.
The consultation was designed to help identify potential risks associated with telemedicine, identify areas where it may help address the needs of both clinicians and the public, and support the potential development of new professional standards and guidance.
The online survey of veterinary professionals received 1,230 responses, while the public consultation received 229 responses and the survey of organisations/stakeholders received eight responses.
The headline question asked of veterinary professionals was whether RCVS 'supporting guidance to the Code of Professional Conduct' should be amended to allow remote examination to take the place of physical examination in certain circumstances. 41% said 'Yes', 40% said 'No' and 18% were unsure.
Veterinary professionals and organisations were then asked a series of questions in order to establish how they rated the risk associated with telemedicine according to activity type, practice type, clinical sign or syndrome, mode of technology, and familiarity with client, animal or environment.
Unsurprisingly perhaps, the majority felt that providing just general advice presented a low risk. At the other end of the scale, most felt that the use of telemedicine to diagnose disease or injury would be either 'high risk', or 'not appropriate at all'.
Likewise, the majority said the risks would be low or medium where the client and environment were known and the animal seen before, for the same problem. By contrast, the majority said telemedicine would be either 'high risk' or 'not appropriate at all' when the client, animal and environment were all unknown.
When asked whether the current definition of 'under care' should be extended to allow veterinary surgeons to prescribe veterinary medicines where there has been no physical examination of the animal, 69% said 'No', 16% said 'Yes' and 15% were unsure.
However, when asked whether certain types of veterinary medicines should be able to be prescribed without a physical examination of the animal, the majority of respondents to the professional survey (52%) were in favour.
The results of the consultation were first considered at a special meeting of the Standards Committee in August 2017, where it was noted how the consultation had revealed significant confusion around current supporting guidance to the RCVS Code of Professional Conduct and that, at a minimum, clarification as to what was currently permissible was needed.
The Committee determined a key issue going forward was whether to change the Supporting Guidance to the RCVS Code of Professional Conduct regarding 'under care' to allow veterinary surgeons to prescribe POM-V medicines based on telemedicine alone.
Given the complex nature of the issues and the wide-ranging implications, the Standards Committee presented a range of options for amending RCVS Guidance to RCVS Council at its meeting in November 2017. After discussion, Council asked the Standards Committee to continue their review and to present more detailed proposals to Council regarding the future of telemedicine in clinical veterinary practice.
Anthony Roberts, RCVS Director of Leadership and Innovation, said: "We would like to thank all those who took the time to respond to the consultation – although Council has not yet made any firm decisions, we felt it would be useful to share our research so far.
"The use of telemedicine is growing rapidly in human healthcare and it is only right the RCVS assesses the opportunities it could bring to improve access to veterinary services. It is critical, however, that we understand the issues it presents 'at the coal face' and consider all the available evidence before making any changes to our Guidance. The RCVS should ensure its regulatory framework fosters innovation and maximises the opportunities to improve the quality, efficacy and accessibility of veterinary services, whilst at the same time protecting animal health and welfare."
The Standards Committee will meet again in April 2018 to take further evidence and develop proposals to take the issue forward.

However, many of the causes of above-inflation increases in the cost of veterinary care go well beyond the remit of the CMA and it therefore has limited scope to propose or enforce changes that will likely have any significant impact on costs.
The CMA concluded that the main barriers to healthy competition are: owners not being given enough information, barriers to owners being able to make appropriate choices and an out-of-date regulatory environment.
To correct these issues, the CMA has proposed that veterinary practices should be compelled to:
Given that treatment inflation, rather than a lack of competition, is likely the biggest contributory factor to rising veterinary costs, it seems unlikely that providing price lists, itemised bills or written estimates will do much more than increase the administrative burden on veterinary practices.
What's more, it is very hard to see how price lists and estimates can ever be truly representative in such a complex area as medicine.
So these proposed measures are no more than rearranging deckchairs on the Titanic.
To switch metaphors, the problem is less that pet owners don't know what they're buying and more that the veterinary profession is trying to sell caviar to people who only want or can afford a Big Mac and large fries.
Real change can only be driven by the veterinary profession - not just veterinary surgeons, but the regulator, universities, manufacturers and others, after reflecting on the real, rather than assumed needs of its clients and patients, together with the ethical issues in play.

Bayer Animal Health (makers of Advocate, Advantix, Advantage, Seresto, Profender, Drontal and Droncit) has launched 'It's a Jungle Out There', an educational initiative aimed at simplifying parasite control for veterinary practices and pet owners alike.
The impressive-looking initiative comes on the back of research carried out by the company which found:

With ongoing support and practical development tools, Bayer's 'It's a Jungle Out There' Programme is an educational initiative which will arm vet practices with the tools they need to make parasite control clearer and more accessible to owners, helping to ensure the UK is a nation of healthy parasite-free pets and happy owners."
Bayer says the first stage of the 'It's a Jungle Out There' initiative aims to help the veterinary community identify opportunities where they can help to improve pet owner compliance. Vet practices will then be supported in developing a parasite protocol which will then be launched to consumers via a heavyweight advertising and PR campaign.
The initiative comprises a new educational website: www.jungleforvets.co.uk, which includes practice development workbooks, market research and trends, extensive technical information on a wide variety of parasites and CPD assessments. Additional downloadable content includes a range of materials such as images, videos, downloads and web buttons.
From the end of August, practice support materials will be available including a waiting room display pack, parasite guide and dispenser, vet nurse reference guide, waiting room posters, lifestyle questionnaire, badges and static window stickers.
The pet owner focus will begin in September and will include an interactive website, advertising and an extensive print and broadcast PR campaign with celebrity ambassadors David Bellamy and TV vet Steve Leonard. This activity is aimed at driving owners towards their vet practice for advice on parasite risks.
Professor Steve Dean MRCVS has been elected as the 14th Chairman of the Kennel Club. He is the first veterinary surgeon to hold the post.
Professor Dean has been a Member of the Kennel Club since 1992 and has been a member of the General Committee since 2005. He has served on the Crufts and Finance & General Purposes Sub-Committees for the past two years and previously spent four years on the Disciplinary Sub Committee.
Professor Dean said: "I am honoured to be elected as Chairman of the Kennel Club and to be given the opportunity to help improve the lives of dogs by ensuring that they live healthily and happily with responsible owners.
"I am aware of the scale of some of the challenges that we face but am excited about the opportunities that exist to help us make a real difference for dogs. I look forward to supporting and working with the millions of responsible dog breeders and owners that there are in this country, and to standing shoulder to shoulder with vets and other individuals and organisations, who are dedicated to improving the health and welfare of dogs."
Professor Dean, who turns 60 in August, is currently the Senior Veterinary Surgeon at Crufts each year. He is a breeder, exhibitor and judge of Border Terriers and has been involved in the breed for 30 years. He is currently Chairman of the Southern Border Terrier Club as well as the overall breed health co-ordinator for all seven breed clubs. He is also President of Windsor Dog Show Society.
Away from the world of dogs, Professor Dean is a keen reader and gardener and is an avid supporter of rowing, treasuring his Henley Regatta medal equally to his achievements in the show ring.

The 'Room For Rabbits' campaign follows research in which 38% of vets said they think rabbits’ environment is the most neglected welfare need1, with over a quarter of the UK’s rabbits being kept in inadequate housing conditions, and almost half of them living alone2.
The RAW survey also found that 90% of vets believe that rabbits’ welfare in the UK is improving3.
Room for Rabbits will throw a spotlight on rabbits’ environment, with a variety of initiatives during RAW including vet blogs and videos on how to adapt bunnies’ housing for all seasons, boredom buster ideas, a competition to encourage owners to create a RAW ‘gold standard’ environment, and downloadable packs containing resources for practices to create their own RAW campaigns.
Holly Castle, Senior Brand Manager at Burgess Pet Care, RAAG member and organisers of RAW, said: “We are delighted that RAW continues to have a positive impact, with our survey showing that 90% of vets believe that the welfare of the UK’s rabbits is improving - but there is always more than we can do.

The study, titled “The utility of combined urine dipstick analysis and specific gravity measurement to determine feline proteinuria”, assessed the utility of the urine dipstick alone and combined with the urine-specific gravity (USG) for detecting proteinuria in cats.
For the study, the clinical records of cats presenting to a referral hospital between January 2011 and January 2017 were reviewed retrospectively.
To be eligible for inclusion, feline urine samples had to have undergone a complete urinalysis including dipstick evaluation, USG and urine protein-to-creatinine (UPC) measurement.
A total of 121 urine samples were included and diagnostic agreement and test accuracy were calculated for the dipstick test alone and in combination with the USG, using different cut-off values for proteinuria. Receiver-operating characteristic (ROC) curves were also calculated.
Jorge Pérez-Accino, corresponding author for the paper, said: “The diagnostic agreement between the urine dipstick and UPC ratio was poor and did not improve if the USG was considered together with the dipstick.
"A dipstick result of equal or greater than “Trace” (0.1-0.3g/L) had a sensitivity of 81% and a specificity of 31% to detect proteinuria.
"With regards the ROC curves, the area under the curve (AUC) of the urine dipstick alone was poor (0.57). When combined with the USG results, this improved to fair (0.78) but the specificity and negative predictive value (NPV) were still low.”
Nicola Di Girolamo, Editor of JSAP said: “These findings indicate that clinicians should not rely on the results of the urine dipstick test in combination with USG in cats for detecting proteinuria. Instead other quantitative methods, such as UPC ratio, should always be performed to detect proteinuria in cats.”
