CVS says the use of ear cytology in OE is beneficial, both as a diagnostic and to evaluate the response to therapy.
However, in 2019 the company looked at 26,269 of its patients and found that ear cytology was only being used in 19.2% of cases, in line with the rest of the profession.
Only 11% of cases were being provided with in-house point-of-care cytology, whilst 8.2% had it performed externally.
80.8% of patients with OE were being prescribed antibiotics without cytological support, with 5% of these the highest priority critically important antibiotics (HPCIAs).
This low level of cytology use in primary care was suspected to be a major contributory factor to the significant use of unnecessary antibiotics.
To support its practices, CVS first looked at the barriers to performing ear cytology interventions, which were identified as a lack of confidence in using the microscope (particularly focusing on x100 and identification of pathological events), a perceived lack of time, insufficient or poorly working equipment, a lack of access to equipment, and the perception that cytology does not change the treatment choice or outcome.
As a second step, a minimal list of cytology equipment was developed to ensure that all sites had the right kit.
New equipment was then supplied to a practice if it did not meet minimum standards.
The company then produced education and resource materials, including new guidelines, imagery and video guides, to upskill both its vets and nurses.
The training focused on: equipment maintenance, cleaning and troubleshooting, how to use the microscope – including x100 magnification and the slide oil immersion technique, how to identify pathological changes, and time management to get a consultation done – including swabbing, microscope analysis, diagnosis and client consultation.
Each site was then provided with its own data, so that it could monitor progress.

A survey carried out by Intervet/Schering-Plough Animal Health to launch Plerion, a chewable, flavoured wormer for dogs, has shown that most UK dog owners turn to their vet rather than a pet shop for advice on worming.
Over 83% of owners who took part in the survey said they follow their vet's advice on worming whilst fewer than 8% would follow the advice of their local pet shop. 63% choose to buy wormers from their vet against 35% buying wormers from a pet shop or online pharmacy.
Survey participants understood the importance of carrying out a regular worming programme with almost 80% stating that a dog should be wormed every three to four months. However, some owners cited forgetfulness as a reason for not worming their pet regularly and expressed a need for a reminder from their local vet practice.
Almost 85% of those who took part in the survey believed that not all wormers provide the same level of protection and almost 75% said that vet recommendation is the reason they purchase a specific brand of wormer. 94% of owners are aware that worms can affect people as well as dogs.
The survey revealed that 1 in 5 owners experienced difficulty in administering a complete dose due to their dog spitting out part or the entire tablet.
Intervet/Schering Plough says that Plerion should help reduce compliance issues; its innovative and tasty soft chew formulation meaning that most dogs will take it right out of the hand.

National charity Canine Partners, which provide specially trained assistance dogs to people with disabilities, will benefit from an Eastbourne vet's efforts to run the Brighton Marathon on Sunday 10th April in less than 3.5 hours.
Simon DeVile is undertaking a gruelling training scheme, covering up to 50 miles a week, to ensure peak performance when he takes to the streets to raise money for Canine Partners - a charity close to his heart.Simon, 44, is one of the partners at St. Anne's Veterinary Group in Eastbourne. He started running to keep fit in 2006 and has since run the London Marathon twice and the Beachy Head Marathon three times. Simon regularly comes into contact with canine partner puppies because a local fundraiser and puppy parent is one of his clients. His own dog is also part of a locally bred litter, five of which went on to train as canine partners.Simon said: "I wanted to enjoy a new marathon experience this Spring and Brighton proved very popular last year. My aim is to enjoy the race but hopefully get below 3hrs 30 mins and raise money for Canine Partners. I am very aware what an important role these dogs play in the lives of a person with disabilities. I am also continually impressed with the way Canine Partners look after the dogs in their care so well."

In launching The Mobile Vet, their vision was to create a mobile clinic which could provide all of the services normally provided in a traditional clinic rather than just some. The veterinary team uses the latest technology and veterinary software to communicate with clients and works from vehicles specifically designed by Dan and Kirsty.

The trial is the result of a six-week consultation held by the College in June 2016, asking for the profession’s views on a proposed new system of CPD - one that concentrates less on hours logged and more on interactive, reflective learning and measuring the impact that CPD has on the individual’s practice and patient health outcomes.
The College says that while an overall majority of the 3,357 people who responded to the College’s consultation agreed with the proposed changes to the CPD requirement, certain elements received less support than others. The lowest amount of support was received for the ‘reflection’ component with 35% of respondents disagreeing with it.
The RCVS Education Committee and VN Council therefore agreed that a pilot of a new outcomes-based approach should be held during 2017 before making a recommendation to RCVS Council. The RCVS Council approved this proposal at its meeting on 10 November 2016, after which 117 volunteers were recruited, 60 of whom were able to attend the training days.
A veterinary surgeon who signed up for the pilot and works in veterinary industry, Gina Dungworth, said: "I really appreciate the acknowledgement of non-clinical CPD practices, and while I was originally sceptical of the proposed system the pilot day has so far been clear and helpful."
A veterinary nurse volunteer, Lindsey Raven Emrich, said: "CPD is such a varied part of a veterinary professional’s career, and it very much depends on the person how easy it is to do. I find myself doing a lot of reflection as a natural part of reading articles, and I’m hoping this new system will expand that way of thinking into other areas of CPD."
The volunteers will now pilot the new proposed system and report to Education Committee, VN Council and RCVS Council.
The initiative comes after BEVA's new nurse committee ran a survey which found a poor understanding within the profession and amongst clients of what equine nurses can do.
Nurses said they believed they could be given more responsibility and do more within their roles and that there was a lack of opportunity for progression.
BEVA President David Rendle, who is championing this initiative, said: “RVNs are a significant an asset to any equine practice.
“They have invested a lot of time and effort to become highly skilled but in some instances their significant attributes are not being utilised to the full.
“Allowing RVNs to perform the clinical tasks that they are qualified to carry out under Schedule 3 of the Veterinary Surgeons Act 1966, will provide better job fulfilment and career development as well as free up veterinary surgeons to carry out other tasks, all increasing the efficiency of the veterinary team.
“I feel, and the RVN survey results would seem to confirm, that veterinary surgeons and practice managers do not appreciate how much veterinary nurses are allowed to do in equine practice.
"We all know veterinary nurses have the ability; look at what they do in small animal and human nursing, but we have felt unnecessarily constrained by Schedule 3 of the Veterinary Surgeons Act 1966 and the associated RCVS guidelines.
"If you actually look at Schedule 3 it is clear that we can and should be letting our qualified equine nurses do far more.
“There are so many things in equine practice that nurses do better than vets.
"In addition to performing clinical tasks they can add wider value; they are generally (or are perceived by clients to be!) more patient, empathetic and caring so we should utilise these skills and get nurses to take on some of the roles that take more time and require longer client interactions – discussing parasite control programmes, running weight loss clinics, reviewing healthcare plans, making follow-up phone calls and visits to name just a few.
"Nurses can take a lot of pressure off vets and can add a significant revenue stream for the practice.”

There are three full equine streams running for the duration of the two days, programmed by the Royal Veterinary College and the British Veterinary Association in partnership with the British Equine Veterinary Association. Organisers say the programme has been designed to ensure it caters for both mixed and equine only veterinary surgeons.
The lecture streams are complemented by the show’s ‘Meet the Professor’ equine sessions, where a number of the speakers are able to have more intimate discussions with a smaller group of delegates.

Located in Wolverhampton, the new practice uses different lighting in different parts of the building: 'cool' for public areas such as the waiting room, 'comfortable warm' for the dog, cat and exotic wards and daylight for clinical areas. The LED system uses 60% less energy than conventional lighting and requires half the number of fittings. In addition, following research in the USA at Banfield, consulting rooms are fitted with dimmable lights to ease stress for patients and clients during euthanasia.
Other highlights include a specialist cat ward that White Cross says exceeds the iCatCare Cat Friendly Clinic requirements. The ward includes raised cat kennels that are carefully positioned so that cats can’t see each other, to further minimise stress-levels. There are also separate, sound-proofed wards for dogs and exotic pets.
To reduce anxiety in dogs and cats pre- and post-surgery, the practice uses pheromone diffusers and specially composed music played in the wards.

Each charge related to fraudulent pet insurance claims that Mr Johnston had made for the treatment of animals when he was in practice in Banbridge, County Down, two of which were fictitious, and where he had arranged for the insurance claims to be diverted and paid into a personal bank account, rather than the practice’s bank account.
At an initial hearing, which concluded on April 2022, Mr Johnston had admitted all the charges against him as well as admitting that his conduct was dishonest and amounted to serious professional misconduct.
The Committee was satisfied that his conduct amounted to serious professional misconduct, with Committee Chair Paul Morris saying: “The Committee has no hesitation in concluding that the respondent’s dishonest conduct will have severely undermined the confidence of the public in the veterinary profession and, further, that his conduct fell far short of the standards and conduct properly to be expected of a member of the veterinary profession.
"The Committee is satisfied that this conduct by the respondent brought the profession into disrepute.”
The proceedings were then adjourned to allow a psychiatric report and other mitigation to be prepared.
At its resumed hearing in November 2022 the Committee considered what sanction to impose in relation to Dr Johnston’s actions.
At this point, the Committee decided to postpone its decision on sanction for a period of two years on the condition that Dr Johnston enter into undertakings to the Committee including refraining from any form of gambling, subjecting himself to a close regime of support and supervision, and repaying some of the sums he had defrauded.
The hearing reconvened in October 2024 to decide on an appropriate sanction.
The Committee noted that Mr Johnston had complied with the undertakings and provided the Committee with the interim reports required of him.
He also continued with the therapeutic interventions and programmes specified, as well as implemented measures designed to minimise the risk of a relapse into gambling.
The Committee also noted that reports from the gambling support services, to which Mr Johnston had signed up, all spoke positively about his involvements and confirmed the progress he had achieved in managing his addiction.
The Committee also took into account a psychiatric report as well as evidence under oath from Mr Johnston’s wife who confirmed his compliance with the undertakings.
As a result, the Committee was satisfied that the prospects of a repeat of the conduct which led to the charges laid against Dr Johnston were now greatly reduced.
The Committee said it was less impressed with the evidence provided by Mr Johnston.
While he had largely complied with the letter of the undertakings he gave in 2022, it remained troubled by his apparent unwillingness or inability, on account of lack of effort, to fulfil the assurances previously given that he was in the process of changing his name to Johnston, from Fegan, on official documentation in order to be consistent with his legal name, when he had not done so.
The only formal name change in place was on the RCVS Register, and he had failed to alter his name on his driver’s licence, on his registration with the Veterinary Council of Ireland, his passport, bank accounts, and one of his email addresses.
As a result, the Committee did not find Dr Johnston to be an entirely satisfactory witness.
Paul Morris, chairing the Committee and speaking on its behalf, said: “Having made the progress that he has over the period since the commission of the charges found proved in 2019, the Committee considers that the imposition of a sanction of suspension on top of the period of postponement would be excessive, in the particular circumstances of this case.
“For the same reasons, as stated above, it is considered that it would be excessive now to impose a sanction of removal from the register.
“That process of reasoning has driven the Committee to the conclusion that the sanction of a reprimand and warning as to future conduct is what the facts and circumstances of this case call for.
"That is because the respondent can be under no illusion about the outcome were he to appear again before this Committee.
"A failure to take advantage of the exceptional course adopted by this Committee on this occasion would be regarded as a serious aggravating factor were he to appear before the Committee at any time in the future.”

The RCVS Disciplinary Committee has dismissed an application to be restored to the Register of Veterinary Surgeons from a former Kent-based veterinary surgeon, Warwick Seymour-Hamilton.
Mr Seymour-Hamilton was originally removed from the Register in June 1994 for failing to maintain his equipment and facilities such that it evidenced a total disregard of basic hygiene and care for animals, thereby bringing the profession into disrepute.
The restoration hearing on Friday 18 March was Mr Seymour-Hamilton’s fourth application for restoration, with previous applications being submitted but refused in July 1995, June 2010 and January 2015. However, as the Committee made its decision on the merits of the case before it, those previous applications were not admitted as relevant to its decision.
Mr Seymour-Hamilton told the Committee that he currently works as a herbalist and naturopath for humans and wished to be restored to the Register so he could include animals in his research. He had completed a course in herbal and naturopathic medicine at the College of Naturopathic Medicine in Dublin in 2010, and told the Committee that he believed that being restored to the Register would lend credence to his endeavours to secure funding and other support. He stated that he did not intend to work again in a veterinary practice.
However, the Committee rejected his application on a number of grounds, including the impact on animal welfare should Mr Seymour-Hamilton be restored to the Register; the length of time he had been off the Register and the fact that he was therefore not up-to-date with contemporary veterinary practice and professional conduct; and that his efforts to keep up-to-date in terms of knowledge, skills and developments in practice were insufficient.
Judith Webb, chairing the Committee and speaking on its behalf, said: "The Committee is concerned about the length of time that has passed since he last practised and the paucity of the evidence he has provided to establish that, if permitted to return to practice, he would be able to attain the professional standards required of a modern veterinary practitioner, either as regards surgical capabilities/competence or as regards his knowledge of currently available veterinary medicines."
She added: "This Committee’s obligations and duties are to see that the interests of animal welfare are properly protected by ensuring that only those who are properly trained, knowledgeable and experienced are permitted to treat animals and that public confidence in the standards of the profession are maintained. The risks attendant on a restoration of this applicant to the Register are, in the judgement of this Committee, plain and obvious. Accordingly, this application is refused."
Sioned Timothy, Ruminant Technical Manager at Boehringer said: "The financial and production impact of liver fluke should not be underestimated. Recent abattoir research in Ireland, published this year, has shown that a fattening steer with fluke infection will put on 36kg less live weight gain over its lifetime versus a healthy steer, costing the producer around £63, per animal.
"Additionally, analysis of data from cattle at a Scottish abattoir showed that animals with 1 to 10 liver fluke present in their liver took an average 31 days longer to reach slaughter weight, while animals with more than 10 liver fluke present took an additional 77 days longer to reach slaughter weight versus animals without any liver fluke infection2.
"Liver fluke infections can also lead to hormonal and metabolic imbalances, affecting reproduction in female animals and extending breeding targets3. Affected animals may also be more susceptible to secondary bacterial infections, such as Salmonella and Clostridial infections3."
Boehringer says that with cattle being brought in for winter housing, now is an ideal opportunity to remove productivity-limiting parasites that were acquired during the grazing season. The main targets for parasite control at this time of year are gutworms, lungworm and liver fluke, however the specific risk will vary from farm to farm.
The company adds that the choice of treatment for liver fluke at housing is complicated by emerging resistance to triclabendazole. This is the only flukicide that treats the early immature stage of liver fluke, responsible for acute disease in sheep but not of clinical relevance in cattle.
Sioned said: "Prescribing an alternative flukicide for cattle at housing will reduce selection for resistance and, by targeting the later stages of the parasite, control the impact of liver fluke on productivity.
"Flukicides containing clorsulon (contained in Ivomec Super) will remove the adult stages of fluke in cattle, while those containing closantel, or nitroxynil (the active ingredient of Trodax), are effective against both late immature and adult stages.
"To ensure that cattle have no residual fluke burden, they may need to be re-treated at an appropriate interval after the initial housing dose. Diagnostic testing can be used to determine whether a second treatment is necessary. This approach will ensure that cattle are turned back out onto pasture fluke free."

Typically advice is provided as a written report within 24 hours, with options for text chat or telephone conversations when vets may want to discuss the case with a specialist.

Until now, follow-up advice had been capped at two weeks.

MI:RNA says that current testing practices for Johne’s disease mean that identification of the disease is difficult, with current sensitivities of around 10-40% and little to no ability to diagnose early stages of infection.
The loss of productivity due to Johne’s to the UK agricultural economy is estimated to be in excess of £10 million annually.
MI:RNA says it is the first diagnostic testing company to use microRNA assay technology.
MicroRNAs are newly discovered biomarkers that manage the immune system and immune responses and act as regulators for disease progression or resolution.
This, says the company, makes them excellent biomarkers of disease, and when combined AI, can significantly improve identification of Johne’s and other complex conditions, and predict disease outcomes.
MI:RNA says this development will allow veterinary surgeons, farmers and pet owners to test for a variety of conditions, not just Johne’s.
Target areas include heart and kidney disease, osteoarthritis and bovine tuberculosis, along with effective general wellness and preoperative screening.

Molar Ltd is launching a cross infection control product called VetroxxTM at this years' BSAVA Congress
VetroxxTM is a range of 100% natural disinfectant products approved by the Department of Health (HPA) for use as a hospital disinfectant and has been approved by the British Government's MAFF (now DEFRA) to control against outbreaks of significant agricultural diseases (e.g. Foot and Mouth Disease, Tuberculosis, Newcastle's Disease found in chickens & Swine Fever).
The range includes a convenient disinfectant hand gel, surface wipes and a multi-surface spray. According to Molar, VetroxxTM has been scientifically tested, proven to reduce microorganism numbers by up to 99.9998%, and kills most problem bugs, including: MRSA, Bird 'flu (H5N1), SARS, HIV, C diff and many others.
The range contains Citroxx®, a naturally-derived disinfectant made from orange pith. Molar says the product is both safe and highly biodegradable, and very kind on skin and surgery surfaces (such as examination tables, flooring and bench tops). The company adds that since Citroxx contains several antimicrobial elements, it is unlikely that resistance will develop.
During Congress, Molar will be challenging vets hand-washing skills using a special UVA (long wave UV) light box to highlight failures.

BSAVA Congress was invaded by Orangemen this year to promote the Vet Charity Challenge (VCC) taking place on 15th September 2012.
Veterinary surgeons and industry colleagues are being encouraged to take part in the one day event in which teams of four will complete a series of physical tasks and puzzle-solving challenges. Teams can choose their own routes which include walking / running, cycling and kayaking stages and can make it as easy or hard as they like.
The Vet Charity Challenge is hoping to raise in excess of £20,000 for Hounds for Heroes; SPANA, and Pet Blood Bank. VCC says it will be a great PR opportunity for practices and an opportunity for team-building.

The new.graduatevet programme (or 'Dot' programme) offers hands-on surgical training and seminars from Advanced Practitioners at Pets'n'Vets' Roundhouse Veterinary Hospital in Glasgow, together with a dedicated website for all new and recent graduates: www.new.graduatevet.co.uk.
Pets'n'Vets says the programme has now been further enhanced by the addition of new CPD elements and the provision of additional monthly benchmarking and seminars.

The first course will be overseen by Jodie Hughes, an RCVS and European specialist in anaesthesia and analgesia. The hospital’s inaugural resident will be Luisa Oliveira (pictured right), who earlier completed a rotating internship at North West Veterinary Specialists in Cheshire, and, more recently, an anaesthesia-specific one at NDSR.
Jodie said: “Luisa’s residency will comprise mainly the provision of anaesthesia to multiple animal species, ranging from the ‘simpler’ procedures to the most critical and complex, which we frequently encounter in our interventional cardiac procedures and emergency work.
“A significant component of the training will involve the management of pain, both in the acute setting (in the peri-operative period) and in the chronic setting, whilst dealing with our pain clinic patients.
“Luisa will also be trained in the management of emergency and critical care patients and will undergo rotations in other services to better grasp their requirements.
“As we work exclusively with dogs and cats, we will work in close collaboration with several other hospitals in the UK and continental Europe, to complement Luisa’s training with other species.
“As part of this, Luisa will be exposed to multiple advanced techniques, including ultrasound-guided regional nerve blocks and interventional procedures in our pain clinic.
Luisa said: “This is a fantastic opportunity for me to work alongside an incredibly experienced and skilled team at NDSR.
The training programme has been developed by Rui Pinelas, RCVS and European specialist in anaesthesia and analgesia at NDSR, who is optimistic there will be further training opportunities in the near future.
Rui said: “This was something NDSR has been keen to implement for some time and we had been having ongoing discussions to choose the perfect time.
“In terms of logistics, it took around three months for us to set up this anaesthesia residency, which was largely spent organising our partnerships with the other training facilities and undergoing an evaluation as to whether our facilities and case load would meet the specialist board’s strict requirements, which we are delighted they did.”
The British Veterinary Association has appointed David Calpin as its new Chief Executive. David joins the BVA from Defra, where he was a Deputy Director and has held a number of senior management roles. David's policy responsibilities have included climate change, GM crops, sustainable land management, and, most recently, bovine TB. For a period last year David was Acting Director for Animal Health and Welfare in Defra.
David has been in the senior civil service since 2005, and has substantial experience of both staff and financial management. He spent two years working in Brussels representing the UK Government in EU negotiations, and has lived and worked in France, South Africa and Brazil. Prior to joining the civil service, David worked for Barclays Bank.
BVA Chairman Derek Williams said: "David's appointment comes at a very exciting time for the BVA as we respond to the challenges set out by our members in our membership research 'Your BVA, Your Say'. David has a proven track record as a senior manager, extensive experience of managing stakeholder relationships, and an excellent understanding of the political climate in the UK and Europe; all of which will ensure BVA continues to thrive and to represent members effectively."
