CVS Group has announced that European internal medicine specialist Caroline Kisielewicz MVB CertSAM DipECVIM-CA is joining ChesterGates Veterinary Specialists with effect from 1 October 2014.
Caroline Kisielewicz graduated from University College, Dublin in 2006 and, after working in first opinion small animal practice for two years, completed an ECVIM-residency training programme at the Small Animal Hospital at the University of Glasgow in February 2013.
Most recently she has been working at the Queen Mother Hospital for Animals at the Royal Veterinary College. She is interested in all aspects of small animal internal medicine, particularly emergency medicine, transfusion medicine, immune-mediated diseases and endocrine diseases.

Vets that took part in the Vet Charity Challenge 2013 have raised a total of £50,000 which was presented to three animal charities at the London Vet Show.
The Vet Charity Challenge took place in September, when 53 teams of four spent the day walking, running, cycling, kayaking and orienteering, along with a variety of mental tasks. The money it raised has been distributed between Hearing Dogs for Deaf People, SPANA and Animals Asia
Jason Rogers, BCF Technology Marketing Manager and one of the organisers said: "The response we got for the Challenge this year has been remarkable with double the number of teams we had in 2012. It was quite a sight to see over 200 people at the briefing at the start of the day. Everyone seemed to really enjoy themselves and get into the spirit of the event. The amount raised this year is truly amazing."
One of the participants, Kerry Lambert from Ashworth Veterinary Group, Hampshire said: "The day was both exhilarating and exhausting. It was very well organised and friendly."
Caitrina Harrison from Vétoquinol added: "As one of the sponsors for this year's Vet Charity Challenge, Vétoquinol had three of the 53 teams entered. Genitrix has repackaged its Dentagen plaque prevention chews to provide added value and convenience to pet owners.
Packs of the chews, launched in 2008 as part of Genitrix' Dentagen dental range, will now be available in 'one month' packs in response to customer feedback. The new, bigger value packs will also be priced more cost-effectively and will offer environmental benefits as the amount of packaging is reduced.
Dentagen dental chews are impregnated with a plant extract called RF2 which enables them to act as an 'anti-biofilm' treatment, modifying the membrane of oral bacteria to prevent them from producing plaque. The chews, which come in two sizes for large and small dogs, are designed to be given on alternate days and form part of the unique Dentagen Dental Care Programme. Earlier this year, Genitrix also launched Dentagen Aqua, an innovative water additive treatment to control plaque and prevent bad breath in cats and dogs.
The new size packs of Dentagen chews, which will be available exclusively through veterinary channels, cost £6.35 for a pack of 14 small chews (one month's supply) and £8.09 for 14 large chews.
The RCVS Disciplinary Committee has dismissed a charge against Nicholas Robert William Horniman MRCVS, a veterinary surgeon from Cinderford, Gloucestershire, that alleged he was guilty of pet insurance fraud, along with one of his clients, and of dishonestly altering clinical records.
The charge was that, between December 1 2008 and June 30 2014, in relation to Cassy, a Labrador retriever belonging to Jayne Bowkett, Mr Horniman:
The Committee heard that Cassy, who had previously been diagnosed with hip dysplasia, had been registered with Pets Barn Veterinary Group in Gloucestershire in May 2008 when Mr Horniman was the Principal and owner of the three-practice group. Cassy was treated at the Cinderford branch of the practice where she received regular prescriptions for her condition.
At some point in 2008/2009, Mrs Bowkett had relayed concerns to Mr Horniman that her current pet insurance policy needed to be renewed, that the renewed policy would not cover Cassy's existing hip problems and that she would be unable to pay for any future operations herself. It was alleged that, in response to Mrs Bowkett's concerns, Mr Horniman told her that they could take Cassy off the practice computer and put her back on under a different insurance policy in a different name.
Mrs Bowkett took out a policy with Petplan, beginning on 13 August 2009, using her father's details instead of her own. It was alleged that Mr Horniman then arranged for new practice records to be set up in the name 'Cassy Griffiths'. These new records were first used substantively in May 2010 when Cassy returned to the practice with hip problems, following which two separate records were maintained for the dog under 'Cassy Bowkett' and 'Cassy Griffiths'.
In August 2010 Dariusz Drozdz MRCVS, a vet who had treated Cassy several times, had been told by Mrs Bowkett that the dog had two separate records. He told the Disciplinary Committee that he queried this with Mr Horniman who suggested that it was a mistake and that they 'RIP' the name Cassy Bowkett. Mr Drozdz disagreed on this course of action but was told by Mr Horniman to 'leave it to me' and the records were then changed to say that Cassy Bowkett had died.
Mrs Bowkett alleged that, at some point in 2011, Mr Horniman explained to her that it would no longer be possible to claim for treatment for Cassy's hip dysplasia under the Petplan policy in her father's name. However, the policy remained active, and was transferred into Mrs Bowkett's name upon her father's death.
In July 2011 Cassy Griffiths was seen by Jennifer Jones MRCVS, a part-time locum vet at the Cinderford practice, who told the Committee that she was puzzled about why there were very few clinical records relating to the animal. A receptionist explained to her that there were two sets of records for the same dog and that Mr Horniman had instructed her to mark the original pet as having been euthanased and close its records and create a second new record for the dog. Ms Jones tendered her resignation shortly afterwards citing concerns that a pet had been 'reincarnated to ensure continued income from insurance claims'. In November 2011 Ms Jones submitted a formal complaint to the RCVS.
During the course of the hearing the Committee heard evidence from Mrs Bowkett, Mr Drozdz and Ms Jones, amongst others.
In giving evidence Mrs Bowkett admitted that she had knowingly engaged in fraud but maintained that at all times she had acted with the knowledge and on the advice of Mr Horniman. However, the Committee found that she was unable to remember any relevant material dates and was unclear about the chronology of events. It was suggested on several occasions that she was lying to the Committee, which she denied. However, the Committee believed that she took no responsibility for her own fraudulent actions but merely blamed Mr Horniman and that she had a strong motive to engage in the insurance fraud.
In regards to the evidence given by Mr Drozdz the Committee noted that he did not record his suspicions of possible fraud, that he continued to treat Cassy Griffiths after he raised his concerns, and that he appeared to have accepted Mr Horniman's assurance that he had dealt with the issue.
The Committee found that Ms Jones was correct in her suspicions that there was only one dog. However, it felt she offered no evidence to support her suspicion that this was part of Mr Horniman's 'scheme to maximise income for the practice' as she alleged in her witness statement.
The Committee also heard oral evidence from and on behalf of Mr Horniman who categorically denied suggesting to Mrs Bowkett that she commit insurance fraud and attempting to cover this up through the maintenance of two separate records. He maintained that it was not until Ms Jones voiced her concerns that he realised that Cassy Bowkett and Cassy Griffiths were the same dog.
The Committee voiced a number of concerns about Mr Horniman's actions and behaviour and, at times, found his evidence to be unsatisfactory. For example, the Committee found it difficult to understand why 'alarm bells' did not ring that he was dealing with only one dog when he prescribed medication for two almost identical dogs in May 2010 when only one was presented for examination. It also queried why, when he became aware that the insurance policy for Cassy Griffiths was fraudulent, practice records were not updated and no attempt was made to contact Petplan to inform them of this, as is clearly advised by the RCVS Code of Professional Conduct.
Furthermore, the Committee was concerned that, in his correspondence with the RCVS, he was less than transparent, candid and honest. The Committee considered this to be unacceptable behaviour from a professional in dealings with investigations undertaken by the regulatory body.
In making its judgment, the Committee had to make a decision on whose account it felt to be more reliable in regards to the first element of the charge.
Noreen Burrows, chairing the Committee and speaking on its behalf, said: "In coming to its conclusion ... the Committee is faced with conflicting evidence from Mrs Bowkett, who is an admitted fraudster, admits to acting dishonestly and to lying to Petplan. This is in contrast to the evidence from the Respondent, who is of good character, had an unblemished personal record over 23 years and is supported by impressive character references."
The Committee found the evidence of Mrs Bowkett to be "vague, lacking in clarity and inconsistent with the facts" and therefore rejected her evidence concerning the first element of the charge. In regards to the second element of the charge, the Committee was not satisfied that Mr Horniman had arranged for or allowed the records to be changed as was alleged. In regards to the third element it found that, since it had already rejected the charge that Mr Horniman suggested to Mrs Bowkett that she embark upon insurance fraud, it was unlikely that Mr Horniman would have acted dishonestly in the manner alleged.
Noreen Burrows added: "In the light of the above findings, all charges against the Respondent are dismissed."
In addition to enhancing current standards of veterinary dentistry, the WSAVA says it hopes that the guidelines will help to bridge what it perceives as a significant gap in veterinary education globally and to encourage a greater emphasis on dentistry in the veterinary curriculum.
The WSAVA’s Global Dental Guidelines include information and images of oral anatomy and common pathology, as well as best practice recommendations for oral examinations and an easily implementable dental health scoring system.
Evidence-based guidance on periodontal therapy, radiology and dental extractions is also included, together with details of minimum equipment recommendations.
A key theme is the WSAVA’s rejection of anesthesia-free dentistry (AFD), which it describes as ineffective and a cause of unnecessary stress and suffering to patients.
The guidelines have been developed by the WSAVA’s Global Dental Guidelines Committee (DGC) whose members include veterinary dentists from five continents, together with representatives from the WSAVA’s Global Pain Council, Global Nutrition and Animal Wellness and Welfare Committees. They are based on the WSAVA’s three-tier continuing education system to ensure that they are relevant for all veterinary surgeons, whether they are working in an advanced hospital setting or in a clinic in a country in which companion animal practice is still developing.
Dr Brook Niemiec, a Board-Certified Specialist in Veterinary Dentistry of the American and European Veterinary Dental Colleges and a Fellow of the Academy of Veterinary Dentistry, is Co-chair of the WSAVA DGC.
He said: "Dental, oral and maxillofacial diseases are, by far, the most common medical conditions in small animal veterinary medicine. They cause significant pain, as well as localized and potentially systemic infection but, because pets rarely show outward signs of disease, treatment is often delayed or not performed with a corresponding impact on the welfare of the patient. In developing the Global Dental Guidelines, we felt that the lack of perception of patient pain was a key issue.
"Our Committee members were also unanimous in their opposition to AFD. We believe that anesthesia is essential for the execution of any useful dental procedure and this is a central recommendation of the Guidelines. To support it, we have provided a detailed analysis of anesthesia and pain management approaches."
He added: "Our patients are not well served by the current variation in standards of care, level of equipment and procedural knowledge of dentistry. Misinformation which clouds the sector is a further hindrance. As clinicians, we cannot allow a fear of the unknown, the discomfort of client pushback or ignorance of current techniques to keep us from doing our best to relieve pain and suffering in our patients.

Veterinary Insights gets its data from over 400 practices using Vet Viewer, its benchmarking software which practices use to monitor business metrics.
Apparently, the figures show that while overall turnover has bounced back relatively robustly, sales in certain areas are still lagging, reflecting changed routines in the ‘new normal’. For example, with many practices requesting clients wait in the car park, there has been a sustained drop in the sale of products typically sold in the waiting room, with sales of pet food falling by 44% during early lockdown, and still down by 25% compared with February’s figures.
The company says there are also significant business stresses that may not be captured in current revenue data.
Alexander Arpino, Managing Director of Veterinary Insights, said: “Anecdotally, we’re hearing that vets are increasingly hard-pressed for time, with logistical difficulties arising from social distancing. Consults are taking longer, and there’s an additional headache for receptionists as payments have to be collected afterwards over the phone. While there are some quick fixes, for example using a telemedicine app such as LinkyVet that sets payment expectations up front, there is no doubt that it’s difficult to make all this extra time investment sustainable in the long run.”
Paradoxically, while consults may be taking up more of vets’ time, some clients feel that they are getting less value for money as there is little face-to-face interaction with the vet. Alexander added: "One of the biggest challenges in the current environment is preserving and fostering those positive relationships with clients that are the cornerstone of companion animal practice.
"Ultimately, we want to bring clients back into the consult room, but only as and when it’s safe to do so. Otherwise, the question is: How can we make adjustments to maintain a high level of client satisfaction, without putting undue stress on vets?"
Veterinary Insights says its software can help practices navigate these challenges by presenting up to 24 months of historical data extracted from practice management systems, enabling businesses to draw comparisons with past performance.

Her talk ‘Evidence-based care for people who have self-harmed: risk prediction, psychosocial assessments, and aftercare’, will outline the importance of improving mental health services for patients who have harmed themselves, via discussion of evidence, policy, and practice for risk prediction, psychosocial assessment, and aftercare.
Leah's talk will be followed by presentations delivered by veterinary mental health researchers from across the UK and Europe.
Topics will include post-Covid wellbeing amongst veterinary professionals, the impact of companion animal euthanasia, workplace stressors and how they change with career stage, and the quality of mental health support received by veterinary nurses.

Companion Care Vets has announced that it will open its 100th surgery in Llantrisant, South Wales on 7th September 2012.
The surgery will reside within the Pets at Home store in Llantrisant and will be owned and managed by Joint Venture Partner Rhian Bullock who has returned home to Llantristant after working and studying for many years in the USA.

The College points out that there is a new voting platform this year, designed to make it easier to vote online.
The new system includes regular personalised email reminders from Electoral Reform Services which include a link to the voting platform and the ability to log in securely straight away, without having to refer to the ballot paper in order to find the security codes. Using the email address provided the College, veterinary surgeons can also ask ERS to reissue the security codes if needs be.
Voting for both of this year’s elections opened on Friday 24 March with an email sent to all those eligible to vote for whom the College holds an email address. Ballot papers have also been despatched to all eligible voters along with booklets containing the candidates’ biographies, contact details and manifestos. Instructions on how to vote online and by post are included with the ballot papers.
There are 16 candidates contesting six places on RCVS Council, including three existing Council members eligible for re-election and 13 candidates not currently on Council. They are:

Once again this year the College invited members of both professions to ‘Quiz the candidates’ by putting their questions directly to all those standing for election. Some of the key themes this year included Brexit, complementary/ alternative therapies, the structure of the veterinary industry, out-of-hours work and veterinary nurse remuneration among others.
Each candidate was invited to choose two questions to answer from all those received and produce a video recording of their answers.

The first part of the charge was that, between 3 November 2014 and 10 December 2016 he did not provide CPD records to the RCVS requested in four separate letters dated November 2014, September 2015, November 2015 and November 2016. The second part of the charge was that, between 11 December 2016 and 26 July 2017, he did not provide the RCVS with his CPD records despite requests.
Dr Zukauskas admitted to the charge against him at the outset of the hearing.
The Committee considered whether Mr Zukauskas’ failure to respond to requests for his CPD records constituted serious professional misconduct.
The Committee found that he had breached the RCVS Code of Professional Conduct for Veterinary Surgeons by not responding to the repeated requests for information from the College, although the Committee noted that there had not been total silence. Mr Zukauskas had made email contact on at least two occasions in response to RCVS letters and gave evidence that he had attempted to phone the authors of the letters. The Committee also noted that in early February 2017 Mr Zukauskas had made repeated attempts to give the RCVS access to his online CPD account, which he had been updating to reflect CPD work that he had undertaken.
Mr Zukauskas’ explanation for his failure was that he had not fully appreciated the importance of the letters, that his English was poor, and that he failed to obtain appropriate advice about the content of the letters until recently. It was only in a witness statement dated 27 July 2017 that he finally disclosed his full CPD records.
The Committee noted Mr Zukauskas' admission in his evidence that his English was not good, particularly in light of the obligation, brought into the Code in February 2016, for veterinary surgeons to be able to communicate effectively in written and spoken English.
Chitra Karve, chairing the Committee and speaking on its behalf, said: "From that date, if not earlier, the respondent should have been concerned to understand English sufficiently well to address the correspondence from the College. Whilst the Committee did not consider that his conduct in this respect amounted to disrespect, it did consider that he had shown a disregard of his obligations.
"At all times he could and should have made appropriate efforts to respond to the correspondence from the College and obtain appropriate advice. In effect he put off dealing with these matters and put his head in the sand."
Regarding his failure to respond to requests between December 2016 and July 2017, Ms Karve added: "This caused the College a considerable amount of concern and extra work. Had he done so much earlier, much of this matter would have been avoided. The respondent was once again in breach of his obligations."
Having found Mr Zukauskas guilty of serious professional misconduct in relation to both parts of the charge the Committee then considered its sanction against Mr Zukauskas, taking into account the fact that the Veterinary Nurse Disciplinary Committee had recently suspended a veterinary nurse from the Register for a period of two months having found her guilty of similar charges.
In mitigation the Committee considered a number of testimonials from colleagues and clients, his hitherto long and unblemished career in the United Kingdom, and his open and frank admissions and subsequent efforts to avoid repetition of his behaviour. Language problems were also considered as an explanation for why the situation had occurred. However, it also considered the aggravating factor that the misconduct was sustained over a period of time and that there was unacceptable disregard for the obligations he had to the College as a veterinary surgeon.
In summing up Ms Karve said: "The Committee has determined to impose a reprimand. In doing so it acknowledged that the respondent has shown considerable insight into his behaviour. He had acknowledged that he has needed help in communicating with the College. It noted that he is a good and proficient veterinary surgeon in the work which he undertakes. He expressed remorse for his behaviour. He has carried out sufficient CPD and since December 2016 has been communicating with the College. The Committee considers it unlikely that he will transgress again.
"The Committee has decided that it is appropriate in this case to add a warning to the decision to impose a reprimand. It is mindful of the fact that other veterinary surgeons registered with the College have a duty to discharge their CPD obligations and they honour those obligations. Moreover, the conduct of the respondent has involved a considerable amount of work and expense for the College."
The warning was that in future Mr Zukauskas must respond in a timely and appropriate manner to any communications from the RCVS.
First they looked at the names of all the cats and dogs they saw in 2018, finding that the most popular names for dogs were Bella, Poppy and Alfie, whilst the most popular for cats were Charlie, Bella and Molly.
By comparison, the top ten names registered at the practice in the thirties were: Susie, Sally, Judy, Penny, Candy, Sam, Betty, Simon, Wendy and Bridget.
Honestly, who calls their dog Simon? Even in the 1930s.
Tom Ward, from White Cross Vets, said: "The most common names in our research reveal a leaning towards giving pets human names. It’s fascinating that this has gone full circle and was also a trend in the late 1930s, before names such as Fido, Patch, Sooty, Duke and Fluffy became popular with cats and dogs."
White Cross says that more recently it has also seen a surge of pets named after TV and film characters including Elsa from Frozen, Baloo from The Jungle Book, Peppa the cartoon pig, and Khaleesi, Sansa, Brienne and Tyrion, the Game of Thrones characters.
The survey was held in mid-June and gathered 196 responses from UK practices. That compared with the 532 responses to the initial survey conducted early in April and 251 responses to the second survey conducted at the start of May.
One of the main findings was a marked increase in the number of practices running a near-normal caseload, from 3% in May to 32% in June. Practice turnover data similarly reflected a shift back towards normality, with 46% now reporting a reduction of less than 25%, compared with 19% last month.

Lizzie Lockett, RCVS CEO, said: "This latest survey has demonstrated a continuation of the previous survey’s positive trends including an increase in practice turnover with more practices approaching a ‘near normal caseload’ and with a reduction in the number of practices impacted by staff self-isolating or with confirmed cases of Covid-19.
“In this survey we also asked about what difficulties practices may be experiencing with EMS placements for vet students and VN training placements as a result of Covid-19, and this will help us to understand how we can better support students and practices in these areas.
“We will continue to monitor the situation via these regular surveys, with the next one planned for later this summer. I would urge as many practices as possible to continue to complete them, so that we can build up a stronger evidence-base on how veterinary businesses have been affected and how they are recovering.
"This information is not only vital for our own policy decisions but also allows us to present a stronger case to the Government and other public bodies where we wish to influence the decisions they make that will impact the veterinary professions and businesses.”

According to the charity, one in ten homeless people have a pet, which is around 32,000 people in the UK. However, the majority of hostels do not accept animals, so they are unable to access accommodation.
The new StreetVet Accredited Hostel Scheme will make sure that hostels are well-equipped to support pet-owning residents. This includes having a variety of pet-friendly health, hygiene and safety policies; access to vet care including telemedicine, free provision of pet essentials, transport to vet practices when needed, emergency kennelling should the pet owner be taken ill and trained hostel team members able to offer support.
The scheme - which will roll out nationwide – has launched today with its first accredited site, The Elms in Hemel Hempstead.
The charity was able to launch the programme after winning £41,000 in funding from Purina’s BetterwithPets Prize which rewards social entrepreneurs and innovators seeking to harness the pet-human bond to tackle societal issues.

Access to pet-friendly hostels is their first step towards independent housing. With homelessness on the rise due to the economic impact of COVID-19, it was important to us to launch this scheme before Christmas, as winter sets in."

Cranial cruciate ligament insufficiency (CCLI) affects 3% to 5% of dogs1.
Past research has demonstrated satisfactory short (6-12 weeks)2 and medium (mean 16 months)3 term outcomes for lame dogs treated with TTA.
However, Steve says there is limited research on longer (>16 months) follow-up periods.
It has also been speculated that dogs with tibial plateau angles (TPA) of greater than 30⁰ are not well suited for a TTA4.
For the study5, the researchers reviewed patient records for a total of 149 TTA-Rapid procedures carried out in 120 dogs over an 80 month period.
They also reviewed owner questionnaires that evaluated the outcome of at least six months after surgery, using a validated clinical metrology instrument, Liverpool Osteoarthritis in Dogs (LOAD).
Follow-up questionnaires were received from owners of 64 of the 120 dogs (53.3%), with a median follow-up time of 37.5 months.
This cohort included a wide range of breeds, aged between 2&12 years and with bodyweights ranging from 6.3 to 41.8kg.
The dogs included 37 females and 27 males, and of the 79 limbs treated in these dogs, 41 were left and 38 were right.
The researchers found 26 of the dogs (40.6%) had a TTA performed with a TPA less than 30◦ and 38 (59.4%) with a TPA of 30◦ or more.
Of the 79 procedures, meniscal injury was noted in 27 (34.2%), surgery to stabilise the patella was required in 11 (13.9%), there was one surgical site infection (treated successfully with a short course of antibiotics), one dog sustained a fracture of the tibial seven days after surgery, and one sustained a late meniscal tear 12 months after initial surgery (treated successfully by partial meniscectomy).
The outcome was judged to be satisfactory, based on frequency of lameness, in 63 (98.4%), while satisfactory outcomes were achieved in 61 (95.3%) based on severity of lameness and in 50 (78.1%) based on mobility score.
Statistical analysis showed that age, bodyweight, tibial plateau angle (TPA), meniscal injury, and concurrent patellar surgery did not influence the outcome, but time to follow-up positively correlated with mobility score (suggesting mobility declined with time from surgery).
The researchers concluded that the long-term (median of 37.5 months) outcome for dogs undergoing TTA-Rapid for cranial cruciate ligament disease is satisfactory, with no risk factors associated with poorer outcomes identified.
The study also revealed no evidence to support the suggestion that dogs with TPAs of greater than 30◦ may be inappropriate candidates for TTA - after testing several parameters to judge outcome including frequency of lameness, severity of lameness, distance walked daily and mobility score.
Steve said: “This research is important in trying to ensure that decisions on whether to use this treatment modality, are based on sound clinical evidence.
"The only factor identified that influenced the outcome measure was that of time since surgery.
"All other factors, including TPA, had no effect on the outcome.”

All veterinary surgeons, veterinary nurses and VN students are to be invited to take part in an RCVS survey of their profession and, for the first time, these surveys will measure mental wellbeing at a population level.RCVS Registrar, Jane Hern said: "Both surveys are being undertaken on our behalf by the Institute of Employment Studies - an independent research organisation. As in previous surveys, all the data will be anonymised by the IES before being shared with us. This will ensure individuals cannot be identified."The information will provide a snapshot of the veterinary profession and help the RCVS to understand and analyse changing trends. Some of the anonymised data will also be shared with researchers at Defra and at the School of Medicine, University of Southampton, who will analyse selected topics in more depth.Jane added: "These surveys produce very useful information about the veterinary and veterinary nursing profession, not least because they usually get a good response rate.
"We will use the information, for example, in our discussions about new veterinary legislation, 24/7 and the Professional Development Phase for newly-qualified vets. The Royal Veterinary College has carried out the first ever open-heart surgery to treat a stenotic tricuspid valve in a dog.
The three-year-old Labrador, Mabel, suffered from congenital tricuspid dysplasia. She was referred to cardiology specialists at RVC Small Animal Referrals where her tricuspid valve was found to be completely fused in the middle, with just two very small openings for her blood to flow through.
In a procedure that took a little over an hour, the RVC team examined and worked on the valve. The College reports that Mabel’s surgery was a success and her stenosis was relieved.
Mabel was subsequently found to have a moderate amount of regurgitation into the right atrial chamber, but after a few more days of additional monitoring at the RVC she was able to return home.
The surgery was carried out by Professor of Small Animal Surgery Dan Brockman at the Queen Mother Hospital for Animals. He was assisted by a team of RVC specialists to provide pre and post-operative care, including a perfusionist, a team of three anaesthetists, two surgery nurses, three surgeons and a cardiology veterinary specialist-in-training. In addition, Professor of Veterinary Cardiology Virginia Luis Fuentes and veterinary specialist-in-training Dr Rosie Payne were members of the Cardiology Service’s investigation into Mabel’s condition.
RVC students were also involved in the Mabel’s initial examinations at the hospital, providing them with an insight into the procedure. The examinations included cardiac ultrasound using the newest generation of cardiac ultrasound equipment, which is able to capture 3D and 4D images. The new ultrasound scanner was funded in part by the RVC’s charity, the Animal Care Trust.
After the surgery, the emergency and critical care (ECC) team and the Cardiology Service took over Mabel’s care, with Professor Brockman remaining involved. This saw up to four senior ECC clinicians, a number of veterinary specialists-in-training and half a dozen highly trained ECC technicians working in shifts around the clock to care for Mabel and ensure a good recovery.
According to the RVC, there have been previous attempts by veterinary surgeons to open stenotic valves with balloon valvuloplasty. However, because it is very difficult to tear the stenotic valve, this approach has not proved very successful. There is also a high risk when putting canines onto bypass. Canines have fragile blood vessels, and cannulae need to be inserted into several of these vessels, creating a danger that a major vessel tears.
Commenting on why the RVC team decided to perform this procedure at this point, and on Mabel, Dan said: "Lots of things fed into this decision. First and foremost, we had a patient whose pathology made surgery the best solution. I explained to the owner and was very honest that this procedure would be really breaking new ground. Based on what the human paediatric cardiac surgeons will do, and our limited experience of balloon-valvuloplasty suggests this is the best approach for these dogs
"The operation itself is risky, much worse than most other operations. In our hands, for this type of disease, we have about an 80% chance of getting them through the procedure. The owner has to gamble what life the dog has left against the promise of a more normal quality of life and life-span following the operation."
In relation to Mabel’s prognosis if the surgery had not been undertaken, Professor Luis Fuentes said: "It is likely that the signs of right-sided heart failure would have been more difficult to control. Her ability to exercise would have become less and less, and she would have also been at risk of having abnormal heart rhythms associated with the enlargement of the right atrium. Generally we expect them to develop more and more signs as the disease progresses. These are difficult cases to manage on a long-term basis with drugs. Her life expectancy would have been substantially reduced.
"I thought Mabel was almost a perfect candidate to have the procedure here as Dan has a lot of experience replacing tricuspid valves in dogs with tricuspid dysplasia. In those cases there is usually leaking, but Mabel’s case was unusual in that she has minimal leaking and she was most affected by the stenosis. That means you may not have to implant any additional valve structure."
Speaking two weeks after the operation, owner Annabelle Meek, from Melton Mowbray, said: "Mabel has just come back in from wandering around the perimeter of my garden for the first time since before the operation. It’s more than half an acre and she has been ambling around happily following a scent.
"I would like to thank absolutely everybody. It’s been an enormous team effort on all sides. Every single person I met at the RVC, from the man on the gate to all the hospital staff, was so friendly. After she came home Dan Brockman phoned me every day to see how she was doing. The RVC should be very proud of what they have done for Mabel."
