Fort Dodge has announced that a challenge study has demonstrated the ability of Duvaxyn^® IE-T Plus equine influenza vaccine to cross protect against A/equi-2/Sydney/07 (H3N8) two weeks after a two dose primary course.  This strain was responsible for the financially devastating equine influenza outbreak in Australia in August 2007.

Duvaxyn IE-T Plus is an inactivated whole virus equine influenza vaccine, adjuvanted with an aqueous-based adjuvant.  According to Fort Dodge, experts at a recent WHO meeting on human vaccines expressed the view that whole virus vaccines have the potential to induce a stronger and more broadly based response to circulating influenza strains than those contained in sub-unit vaccines because of the presence of the full set of virus proteins in the vaccine*.

The study, conducted by the Animal Health Trust, involved two groups of EIV seronegative horses.  The first group, consisting of seven vaccinates, were given two dose of Duvaxyn IE-T Plus, 28 days apart, and challenged with A/equi-2/Sydney/07 (H3N8) virus strain 14 days after the second vaccination.  The second group of unvaccinated horses was challenged at the same time.  Duvaxyn IE-T Plus caused a statistically significant reduction in both clinical signs of the disease and in viral shedding.

Helen Barnes, EMEA Equine Business Manager for Fort Dodge, said: "The results of this challenge study provide reassurance for owners that horses vaccinated with Duvaxyn will be protected against circulating strains of equine influenza, including the Sydney strain which proved so damaging.

"As the benefits offered by whole virus technology are increasingly recognised, the popularity of vaccines based on this technology is re-emerging, particularly to combat a potential pandemic.

"The entire Duvaxyn range of equine vaccines, including the recently licensed Duvaxyn WNV for West Nile Virus, are based on the whole virus approach as we believe it offers a broad and  consistent level of protection."

The Royal College of Veterinary Surgeons and British Small Animal Veterinary Association have issued a joint statement concerning the postgraduate qualifications CertAVP, PGCertSAM and PGCertSAS.

The statement follows the introduction earlier this year of the BSAVA's Postgraduate Certificates in Medicine and Surgery.

Dr Frances Barr, BSAVA's Director of Education (pictured right) said: "There has been a natural interest in how the BSAVA PGCertSAM and PGCertSAS compare with the RCVS Certificate in Advanced Veterinary Practice (CertAVP). Detailed discussions over the last few months between BSAVA and the RCVS have resulted in agreement that the two qualifications are academically equivalent, as outlined in a formal position statement. Individuals can therefore choose to study for the qualification which suits them best."

For thirty years, veterinary postgraduate qualifications at certificate level have been available within the UK; until recently, these were organised through and awarded by the RCVS. In recent years the Certificate in Advanced Veterinary Practice has been introduced, with assessment of accredited modules by a range of universities, and the final award made by the RCVS. With the development of the BSAVA Postgraduate Certificate programmes, questions have naturally arisen regarding the equivalence of these two programmes, and whether a stated requirement for 'a certificate' for a particular purpose refers to the general level of postgraduate qualification or to one particular qualification. It is natural that prospective candidates and employers should ask such questions when considering options for postgraduate study.

The joint position statement by the RCVS and the BSAVA:
Representatives from the RCVS CertAVP subcommittee and BSAVA have therefore discussed the RCVS and BSAVA certificate programmes in detail. The statement here is intended to set out agreed principles which might be needed to allow resolution of any future questions about equivalence or accreditation.

• The RCVS Certificate in Advanced Veterinary Practice (CertAVP) and the BSAVA Postgraduate Certificates (PGCertSAM and PGCertSAS) are academically equivalent.
  Each qualification is positioned at Level 7 (Masters) within the QAA Framework for Higher Education Qualifications in England, Wales and Northern Ireland, and carries 60 academic credits at this level. The qualifications are therefore, by definition, academically equivalent.
• The RCVS CertAVP and the BSAVA PGCert are similar, but not identical, in content (depending on the modules studied). They differ in structure.
  The two qualifications are therefore likely to appeal to different groups of individuals, depending on their interests and preferred style of learning. Both qualifications are intended to develop practitioners who are able to deliver a consistently high standard of care to their patients.
• Transfer of academic credits between the two programmes (RCVS CertAVP and BSAVA PGCert) would, in theory, be possible.
  The two programmes have a different modular structure, and some difference in content (depending on the modules studied). Formal requests by candidates to transfer credit from partial completion of one programme towards the other would be considered on an individual basis, according to the guidelines of the RCVS or BSAVA (with guidance from Nottingham Trent University), as applicable.
  The concept of academic credits from either qualification being accepted towards a full Masters degree in the future is welcomed by RCVS and BSAVA.
• Accreditation of the RCVS CertAVP and the BSAVA PGCert for any particular purpose would be considered against defined objectives.
  The principles pertaining to academic equivalence and credit structure would apply. No universally applicable principle would be in place, but accreditation arrangements should usually consider outputs, or statements of competency, rather than inputs, such as specific qualifications. Therefore where the particular purpose requires evidence of postgraduate development, this is the wording that should be used rather than to detail specific qualifications. It is acknowledged that such an approach is already being taken with respect to the Practice Standards scheme, and the remaining RCVS Diplomas.

The 'middle tier' postulated in the current RCVS Consultation on Specialisation, and the proposed 'acknowledged veterinarian' status under consideration in Europe, are not currently sufficiently defined to allow a position to be agreed.

The Saddle Research Trust has announced the launch of a new paper to help vets understand the significance of saddle fit on the health and welfare of horse and rider.

Saddle fitting, recognising an ill-fitting saddle and the consequences of an ill-fitting saddle to horse and rider was written by equine vet Dr Sue Dyson together with Master Saddlers and saddle fitters Mark Fisher and Sue Carson.

The paper has been published in the Equine Veterinary Education Journal, the official education journal of the British Equine Veterinary Association and is free to view until April 2016.

The paper encourages vets to work as part of a multi-disciplinary team with saddle fitters, trainers and physiotherapists, to help educate horse owners about the multiple problems that can be caused by ill-fitting saddles.¹

Key advice includes:

• The saddle should be assessed off and on the horse, without and with a rider.
• The fit of the saddle for both the horse and rider must be evaluated.
• A well-fitted saddle should distribute weight evenly via the panels to the horse's thoracic region, with complete clearance of the spinous processes by the gullet.
• The saddle should remain fairly still during ridden exercise at all paces.
• The saddle must also fit the rider to enable them to sit in balance.
• Signs of an ill-fitting saddle include equine thoracolumbar pain, focal swellings under the saddle, ruffling of the hair, dry spots under the saddle immediately after exercise surrounded by sweat, and abnormal hair wear.
• If a saddle does not fit the rider, the rider may not be able to ride in balance with the horse, and this may induce equine thoracolumbar pain.
• A saddle of inappropriate size and shape for the rider may induce rider back pain, ‘hip’ pain, sores under the ‘seat bones’ and perineal injuries.

The paper also includes a glossary of saddlery terms, an explanation of saddle fit accessories, a range of informative photographs and a section on who is qualified to provide professional advice on saddle fit.

The paper concludes: “Correct saddle fit for horse and rider is an important equine welfare issue and the veterinary profession needs to recognise this and be at the forefront of helping to educate the horse owning public. Ideally veterinarians should work in conjunction with professional saddle fitters, trainers and physiotherapists to improve both horse and rider comfort.”

The Royal Veterinary College has received a donation of £1m from the Jean Sainsbury Animal Welfare Trust towards a £1.5m refurbishment of the Beaumont Animals' Hospital in Camden, London.

The Hospital will be renamed as the Beaumont Sainsbury Animals' Hospital in Jean Sainsbury's honour, as a reflection of the matched values shared by Jean and the College. The Hospital has remained open during the refurbishment, which is due to be completed in Spring 2011.

Graham Milligan, Clinical Services Director at the Royal Veterinary College said: "This Landmark donation enables the College to create a 21st century veterinary primary care practice that is unrivalled in Central London. We are delighted to be able to celebrate Jean's life by renaming our Hospital in her honour, as she was dedicated to improving animal welfare and promoting responsible care of animals and sound primary veterinary care."

Jean Sainsbury, who died in 2007, was a frequent visitor to the hospital and provided funds to assist clients in receipt of state benefit with essential care and treatment of their pets. Jean especially valued the work of the Beaumont Animals' Hospital because it provides the highest quality first opinion services for pet owners in Camden, and that this service is based on the ethos that all treatments and procedures should be necessary and appropriate to the animal's needs.

Colin Russell MBE, Chairman of the Jean Sainsbury Animal Welfare Trust said: "The Trustees feel it is a fitting tribute to Jean Sainsbury, who was dedicated to animal welfare, to have her name added to the Beaumont Animals' Hospital which she supported so enthusiastically during her lifetime."

For many years Jean Sainsbury was a supporter of the Royal Veterinary College and its hospital for small animals in Camden.  Grants to the College helped to fund the first phase of the Queen Mother Hospital for Animals at the Hawkshead campus in Hertfordshire and grants were used at the Camden Town site to refurbish operating theatres and waiting and reception areas in the animals' hospital. Since 1994 the Trust has subsidised veterinary costs for those on low incomes. Jean Sainsbury's support was formally recognised in 1994 by the award of an Honorary Fellowship of the College.

The extensive £1.5 million refurbishment of the hospital includes improvements to the reception and waiting areas for clients, refitting of the five consultation rooms; major upgrades to the dispensary and treatment room, a complete refit of the radiography suite including updating to the latest digital standards, a refit of surgical facilities including the creation of a new dedicated dental suite, the expansion of ward facilities for exotics, an overhaul of the ward space for dogs including expansion of the isolation facilities (to minimise likelihood of spread of infectious diseases) as well as complete refurbishment of seminar room facilities for students, new office accommodation and major improvements to living quarters for the veterinary nursing team.

A study being conducted by the University of Liverpool and CVS has concluded that vets have wide-ranging perceptions of what constitutes a workplace injury, with some harms being seen as just ‘everyday norms’. 

Starting in 2022 ‘The context, consequence and prevention of veterinary workplace injuries: a qualitative and quantitative study in the UK’ study aims to explore the context, consequences and behaviours surrounding veterinary workplace injuries - to improve the safety of the profession.

The first phase of the research was a cross-sectional survey of 740 broadly representative veterinary professionals.

Initial analysis, published in Occupational Medicine¹, explored how injuries and their context are perceived by veterinary profession. 

Themes identified during this phase of analysis were:

• Injuries have become normalised, accepted and seen as everyday norms in veterinary practice - resulting in many not being reported;
• Differences in how injuries were defined, with small animal vets expecting injuries to involve blood, and equine/farm vets expecting them to reduce their ability to work or require the need for medical treatment;
• Equine and farm animal vets having a high threshold before acknowledging that an incident is a work-related injury. As such dangerous events, such as kicks, are seen as ‘everyday’ events and are not deemed an injury, and so will not be reported.
• Many suggest all injuries should be reported, however minor injuries were often overlooked, for example needlestick injuries;
• A large proportion of colleagues (especially vets) see reporting injuries as too onerous;
• Injuries caused by staff themselves (e.g. trips) were less likely to be reported than injuries that could be blamed on an external factor (e.g. dog bite). 

CVS says the veterinary industry should take note of the discrepancy of perception of what constitutes work-related injury in its practices and reporting protocols.

Practices may need an individualised approach to minimise their workplace injuries: some may need to work on revisiting their ideas around acceptable risk, whilst others may be good at minimising work-related harm, but have a complex or blame-associated reporting system which needs to be simplified.

Dr. John Tulloch, Lecturer and European Specialist in Veterinary Public Health, at the University of Liverpool who is leading the research, said: “Injuries that occur within veterinary practice can tragically at times be life-changing and are often avoidable.

"The objectives of this study are to understand how veterinary professionals define injuries and to understand what injuries they do or do not deem reportable.

"The results of this stage of the research have been both startling and complex.

"They highlight a profession in drastic need of cultural change and personal attitudes, with respect to injury prevention and mitigation.”

Dr. Imogen Schofield, Director of Clinical Research at CVS, said: “Using these findings, we will be able to develop and strengthen injury prevention measures through safety policy, education, and training.

"We anticipate that this project will result in safer workplaces within the wider veterinary community and, indirectly, in improved animal treatment through a fitter, healthier and safer profession.”

CVS says the research will lead to the development of open access educational tools aimed at promoting injury awareness and prevention.

The tools will focus on understanding why reporting injuries is important, how to develop a culture of accountability without blame, and when to seek medical attention and take time off work.

They will also investigate how to encourage clients to be safety ambassadors, and how to empower colleagues in challenging situations regarding restraint, aggressive patients and equine safety.

In addition, they will look at improving cat handling via International Society of Feline Medicine training and Cat Friendly Clinic accreditation, facilitating a rise in usage of hard hats in equine, and raising awareness of the risks associated with sharps plus sharp bin provision.

RCVS Council has voted to make it a requirement that remote prescribing (without physically examining the animal) can now only be carried out by veterinary surgeons who can 'provide a 24/7 follow-up service involving physical examination, plus or minus further investigation, if required’. 

The follow up service ‘can be provided personally by the veterinary surgeon or practice, or by written agreement with a veterinary services provider which is local to the client (as with the current situation for [out-of-hours] care provision)’.

The new rule comes into force on the 1st November, to allow practices to make any necessary arrangements.

The RCVS Council also decided that the temporary derogation from the usual requirement to conduct a physical examination before an animal is regarded as ‘under care’ be reviewed as a standing item at each subsequent Standards Committee meeting, until the normal guidance provisions are fully restored.

Fort Dodge has unveiled an initiative to help practices improve peri-operative care and the management of hospitalised cases with the launch of its Early Warning Scoring System (EWSS) kit.

The kit contains cat and dog anaesthesia and observation charts for the effective monitoring of cats and dogs during operations, post-operatively and for hospitalised medical cases.  

EWSS observation charts are widely used in the NHS but this is believed to be the first time that such a system has been developed specifically for use in veterinary practices.

They have been designed by Mike Davies BVetMed CertVR CertSAO FRCVS, Technical Services Manager at Fort Dodge, and are intended to help veterinary nurses and veterinary surgeons identify patients whose condition is deteriorating before they reach a critical state.

Mike Davies speaks around the country on preventive medicine and geriatrics and his experience of working closely with practices is that many do not have good protocols in place for nurse monitoring of patients. The EWSS grades parameters such as heart rate, respiratory rate, blood pressure and CNS response and the charts incorporate areas in which the practice can record pain using a visual analogue scale or a numeric scoring system.

The company has also launched Dorbene vet, a new product providing fast-acting sedation and analgesia for dogs and cats. Its active ingredient is the trusted and well proven alpha-2 agonist medetomidine hydrochloride, which affects the nervous system by inhibiting noradrenaline mediated neurotransmission. A dosing guide for Dorbene vet is also included in the EWSS.

The launch of Dorbene vet complements Fort Dodge's existing companion animal products, analgesic Torbugesic and dissociative anaesthetic Ketaset. Used alone it offers dose-related sedation within 10-15 minutes, which is rapidly reversible using the alpha-2 antagonist atipamezole for predictable recovery. It is presented in 10 ml and 50 ml multi dose vials in amber coloured glass which protects the product from light.

UK veterinary surgeons strongly prefer recommending branded treatments, according to a survey conducted by specialist advertising agency Masius.

The survey, conducted amongst members of www.vetsurgeon.org, received 725 responses. It gave a conclusive insight to veterinary surgeons' opinions on branding, with 87% of respondents saying that they prefer to recommend a branded POM-V over a generic equivalent.

While branded treatments are clearly preferred over generics, vets recognise that higher prices can be an issue for clients. Price is justified mainly through a higher level of trust in the quality of the product (through extensive trialling). They also cite the added support in the event of client usage issues and the value that clients attach to brands. The cascade is a further factor, requiring the use of licensed drugs.

When it comes to supplements, vets are still in favour of brands, but less so - as the cascade is not a factor. On the issue of price differential, most vets felt that a premium of 10-25% is justifiable for a brand over a generic.

Jonathan Posner, Business Director at Masius said: "As well as the quantitative data, we also got some interesting qualitative comments as well. Looking at these, one can see a range of opinions - from the vets who value the easier client acceptance of brands, to those who feel that their professional care of the patient is paramount and that a generic should be used where it delivers the same benefit as a brand. Client ability to pay was also mentioned as a factor - a generic being more likely to lead to compliance for a client on a tight budget."

The fact that branded products come with better information and marketing support was agreed by the large majority (93%) of respondents.

Purina has launched the very snappily named Pro Plan Veterinary Diets NF Renal Function Early Care and Pro Plan Veterinary Diets NF Renal Function Advanced Care, designed to meet the nutritional needs of cats in International Renal Interest Society (IRIS) stage one and two (early phase) and IRIS stage three and four (late phase) respectively.

Purina says the foods contain adjusted and graded levels of protein and phosphorus to address the delicate balance of providing needed protein in older cats against uraemic toxin production in late phase renal decline.

The current advice is that phosphate should be restricted when the renal patient has confirmed CKD, and IRIS provides therapeutic serum phosphate targets for each stage.

Dr Ellie Groves BA(Hons) VetMB MRCVS, Purina Technical Manager for the UK and Ireland, said: “As obligate carnivores, cats have a high requirement for protein, however, around 20% of senior cats have decreased ability to digest protein, and so are absorbing less protein.

"We know that this loss of lean body mass is significantly correlated with an increased risk of mortality.

"But we need to be careful – in renal disease, there is the potential for protein breakdown products to increase in the circulation, acting as toxins.

"So, we need to fine-tune the amount of protein, depending on the stage of chronic renal insufficiency (CRI).

"A dual approach, with Early and Advanced products, allows tailoring of protein levels.” 

“Alongside protein, phosphorus content is also tailored to the stage of kidney decline.

"Restricting phosphorus can slow the progression of chronic kidney disease by reducing morphological damage to the kidneys and is thought to be the most important influencer of survival in cats with CKD.

"The levels of phosphorus in the products, are tailored, alongside the protein, to reflect the needs of the cat, with increased restriction when cats are in advanced decline and the kidneys are struggling most.”

NF Renal Function Early Care is available in a dry format (1.5kg) and a pouch (tender pieces with chicken 85g).
