Davies Veterinary Specialists (DVS), the private small animal referral centre based in Hertfordshire, has installed two new state-of-the-art ESAOTE ultrasound machines.

DVS says The MyLab Twice Vet and the MyLab Class C Vet provide fast, efficient and highly accurate imaging in a wide range of applications, including high-definition abdominal, cardiac, musculoskeletal and ophthalmic scans.

The MyLab Twice Vet, with its range of probes including the broadband 18MHz linear probe, will be used for general ultrasonographic imaging at DVS. It is capable of the most refined contrast media examinations that, for example, can help to locate and identify a liver tumour that may be rendered invisible when using conventional diagnostic procedures.

The MyLab Class C will be used predominantly for advanced cardiac work, including trans-oesophageal echocardiography for interventional procedures in theatre.

Pedro Oliveira, European Veterinary Specialist in Cardiology at DVS said: "The trans-oesophageal probe is currently a very rare piece of equipment in veterinary diagnostics. It gives us the capacity to obtain unique views of the heart, especially the heart base which can be a very difficult area to examine with regular echocardiography. It also allows real-time and direct monitoring of procedures, such as deployment of devices to occlude patent ductus arteriosus, surgery and anaesthesia in critical patients."

The first patient to benefit from the new My Lab Class C was an 11-year-old, female German shepherd dog with suspected cardiac neoplasia following several episodes of haemorrhage into the pericardium. A normal echocardiographic examination identified a small suspect area but it was in a part of the heart that was very difficult to approach surgically.

However, a trans-oesophageal examination revealed that another small mass close to the tip of the right auricle was actually the origin of the bleeding. The dog underwent surgery to remove the mass and subsequently made a full recovery. Without the My Lab Class C there would have been no viable options for this dog.

Nicola Menzies-Gow FRCVS, Professor in Equine Medicine at the Royal Veterinary College, has been appointed Chair of the RCVS Ethics Review Panel (ERP).

The RCVS ERP provides an ethics review mechanism for researchers who are based in practice and don't have access to this process through university and industry connections.

The subcommittee has representation across a range of areas of veterinary expertise, including veterinary nursing, and also includes lay researchers and scientists.

Since its formation as a trial service in 2016, the ERP has reviewed more than 530 research proposals on behalf of the College.

Nicola said: “I am honoured to be offered this position.

"Having been a member of RCVS ERP for the past three years and undertaking a similar role as the Chair of the Clinical Research Ethical Review Board at the Royal Veterinary College, I felt that I had the right expertise to take on the role

“It is vitally important that all research, regardless of where it is undertaken, undergoes ethical review to ensure that robust results are produced especially when they have the potential to influence clinical practice, and so I really welcome the work the ERP has been undertaking.

Parasitologists from ESCCAP (European Scientific Counsel Companion Animal Parasites) have been voicing their concern over the implications of the imminent changes to the UK's DEFRA-run Pet Travel Scheme (PETS).

Under the new regulations, which take effect from Jan 2012, tick control will no longer be mandatory. According to ESCCAP, a recent study has demonstrated that a population of exotic tick species has already become established in the south-east of England¹. With up to 15,000 animals travelling under PETS each month, the possibility of exotic ticks (and their associated tick-borne diseases) entering and establishing in the UK becomes increasingly likely. The European Parliament has yet to rule on mandatory tapeworm control. The decision will impact on the future distribution of, and disease threat from, Echinococcus multilocularis.

ESCCAP UK is advising all veterinary surgeons to discuss the risks with their clients and to ensure they make an informed decision about tick control when the new legislation comes into effect.

In addition, the UK branch of ESCCAP is holding a free seminar in Bristol on 28^th October to highlight the role that veterinary practices can play in helping clients understand how the changes could impact upon their pet's health.

ESCCAP says the seminar will provide all the information clinicians need to have an informed dialogue with clients. Presentations will provide an overview of the impact of the changes to PETS in relation to rabies, Echinococcus and ticks, as well as advice about how to communicate the information to pet owners and the tools and support available.

The RCVS Practice Standards Scheme (PSS) is introducing remote assessments to ensure standards are still being met during the pandemic. 

Specifically, remote assessments will make sure that practices are continuing to meet Veterinary Medicines Regulations, the Core Standards of the PSS and other key standards relating to the practice's accreditation type and level.

Mandisa Greene, RCVS President and Chair of the Practice Standards Group, said: “We have introduced these remote assessments as a means of being able to quality assure veterinary practices that are either in the Scheme or who wish to join it, while still making sure that public health is safeguarded and that the burden our face-to-face assessments may place on practices is reduced.

"All our Practice Standards Scheme Assessors will be trained in carrying out remote assessments, and practices that are due an assessment will be given the full details of how it will work in advance, just like if a physical assessment of the premises were taking place.

"In the immediate-term we will be prioritising those practices that were due assessment visits in March, April and May this year which were cancelled due to the nationwide lockdown and they are currently being contacted to make arrangements. We will then start a rolling programme of visits for the other delayed assessments over the next few months. We will aim to give all practices at least one month’s notice before a visit takes place."

Dechra has created a new app with the Royal Veterinary College which promises to help vets better diagnose and treat endocrine disorders in companion animals. 

Typically, diagnosis of endocrine disease is based on a thorough history and physical examination, followed by laboratory tests.

However clinical signs vary substantially across animals with endocrine disorders, so this new app uses AI and big data from millions of dogs in the RVC VetCompass database to improve the chances of an accurate diagnosis.

Dr Dan O’Neill, Associate Professor for Companion Animal Epidemiology at the RVC, said: "By applying the latest AI technology, the app is designed to assist primary care veterinary surgeons to better detect and diagnose Cushing’s Syndrome.

"The app provides a guide to the probability of Cushing’s syndrome as an explanation in patients showing both typical and atypical clinical signs for Cushing’s"

The app is free to download on both Apple and Android devices in the UK and EU countries by searching for 'the endocrinology app".

CVS is running a clinical improvement project in 48 of its small animal practices to improve the use of cardiac ultrasound scans in first opinion heart disease diagnosis and treatment.

The project looked first at how effective CVS small animal first opinion practices were at conducting echocardiography.

It found that a barrier to wider uptake was a lack of up-to-date equipment, along with a lack of current knowledge and training - both in using ultrasound and echocardiography equipment and in interpreting images.

CVS says that as a result, many colleagues were routinely referring suspected cardiac disease cases.

The project leaders then reviewed and recommended practice equipment, so that clinicians had the tools to perform echocardiograms in practice.

This included buying echocardiography tables - so that practitioners could scan an animal’s chest from underneath, more advanced cardiac ultrasound machines and electrocardiogram (ECG) machines. 

The company then produced a range of resources for vets and nurses, to help them to improve echocardiography scan rates and nurse cardiology clinics, including clinical guidelines, machine guides and client resources.

It then held a number of training sessions for its vets and nurses, focusing on the latest advancements in heart disease diagnosis and treatment for vets, and training nurses how to conduct nurse cardiology clinics, how to perform and interpret electrocardiography, how to take blood pressure readings and thoracic point of care ultrasound (POCUS).

Lynsay Morgan, Regional Clinical Lead at CVS who is responsible for the cardiac ultrasound clinical improvement project, said: “We want to empower our first opinion small animal vets to perform more echo evaluations and to work up their cardiology cases to a high standard.

"We also want our nurses to be involved in patients’ life-long care, and provide ongoing support to owners of pets with heart disease.

“We hope this project will improve case identification, increase skills and confidence, and see optimum diagnosis and management of cardiac disease.”

The Royal College of Veterinary Surgeons Trust's Adopt-A-Book campaign has raised almost £17k in sponsorship to pay for the restoration of books in the historical collection.  

The latest book to be restored under the scheme is a nineteenth century edition of Every man his own farrier, by F. Clater, which has been restored thanks to veterinary surgeon Fiona Dalzell.

Fiona said: "I have always loved books, and especially old books, but I got inspired by the Adopt-a-Book scheme when I came to the RCVS for a meeting of the Veterinary History  Society".

These editions, she says, are "works of art in their own right," and it has given her a "huge amount of pleasure to know that you can do so much just from a small donation." 

RCVS Trust Director Cherry Bushell explained the idea behind a campaign: "When you think of 'adoption', you may more readily recall the rescue animals treated in veterinary practices than old books. However, these old books also need your help - and are an important part of the veterinary heritage that the Trust seeks to preserve."

Restoring a book can cost between £25 and £250, and adopted books carry a book-plate naming their benefactor. They can also be dedicated 'in memoriam'.

The RCVS is seeking nominations for its 2014 Honours and Awards Round, and wants to hear about any individuals, of whatever age or experience, who have made an outstanding contribution to the profession.

The annual Honours scheme comprises two distinct and prestigious types of award: Honorary Fellowships for RCVS-registered veterinary surgeons and Honorary Associateships for non-veterinary surgeons. Both sets of awards are presented each year at RCVS Day - the College's AGM and award ceremony in London.

The College says it is keen for the Fellowship nominations net to be cast as widely as possible in order to offer recognition of veterinary achievements not only in more traditional fields like academia and clinical practice, but also in science, education, industry and politics.

In addition, Honorary Fellowship nominees no longer need to have been an RCVS member for 30 years, meaning that younger veterinary surgeons who have gone significantly above and beyond the call of duty will also be eligible for consideration.

Honorary Associateships are intended to celebrate the achievements of those who have contributed to the health and welfare of animals by working in fields related to the veterinary profession, and could be from a similarly wide range of backgrounds, for example, veterinary nurses, scientists, lecturers, farriers, charity workers, farmers, conservationists or those in industry and commerce.

Nick Stace, RCVS Chief Executive, said: "At my first RCVS Day this summer, I was hugely impressed with the calibre of candidates who received our awards, and delighted we were able to formally mark their achievements. A new study, published in the latest issue of the Journal of Small Animal Practice (JSAP), has shown it may be possible to predict the likelihood of re-obstruction and stricture formation occurring following perineal urethrostomy in male cats¹.

In the study titled “Evaluation of urethral orifice cross-section dimensions following perineal urethrostomy in male cats”, 24 male cats with lower urinary tract disease underwent the procedure. The urethral orifice cross-section was estimated by the largest urinary catheter that could be easily inserted through the stoma at three time points: preoperatively, intra-operatively and 12 days postoperatively. Cases of obstruction recurrence and stricture were documented within the six month follow up period.

Uri Segal, corresponding author for the paper, said: “The intraoperative stoma ranged from 1 to 10 (median 10) Fr and the 12-day postoperative stoma ranged from 4 to 10 (median 8) Fr. There was significant reduction of stoma size 12 days postoperatively, compared to the measurements taken intraoperatively.

"Postsurgical stricture formation and re-obstruction was documented in five of 24 (21%) of cats. Obstruction recurrence is probable if the intra-operative stoma size is 8 Fr or less. Furthermore, if the stoma size is 6 Fr or less, re-obstruction is almost certain (all of these cases in the study were re-obstructions). Conversely, an intra-operative stoma of more than 8Fr had a low risk of recurrence.”

Nicola Di Girolamo, editor of JSAP said: “These findings may function as a guiding rule for surgeons performing perineal urethrostomy, by providing an intraoperative measure of how likely re-obstruction and stricture formation is to occur. It should be remembered, however, that the functional urethral stoma cross-section may vary during the urination cycle and the measurements in this study are likely to only represent a fraction of the functional urethral cross-section size.”

Intervet/Schering-Plough Animal Health has released diagnostic data which the company says indicates that calf scour problems caused by cryptosporidia are on the increase.

A summary of 2010 results from the company's ScourCheck scheme, which identifies infectious scour pathogens in calf faeces, show the protozoan parasite Cryptosporidium parvum being present in 37% of samples.

Intervet/Schering-Plough ruminant veterinary adviser Drew McGurren said: "Ever since we started testing calf faecal samples in the 1990s, rotavirus and cryptosporidia have always been identified as the two main pathogens implicated in infectious scour problems. But the latest data - and feedback from vets in practice - suggest cryptosporidia incidence is on an upward trend.

"Ten years ago our top line reporting used to refer to rotavirus being implicated in around a third of all infectious calf scour problems and cryptosporidia in roughly a quarter. But in 2009, we detected cryptosporidia in 30% of samples. And this year - from the 250 plus faecal samples we have looked at so far - the incidence seems to be even higher."

Drew stressed that scours caused by cryptosporidia are not always confined to housed calves: "Suckler calves between one and two weeks of age can be very susceptible to infection at grass, particularly if conditions underfoot are muddy around ring feeders. Early May has seen quite a lot of rain, and the parasite thrives in damp conditions."

The major source of cryptosporidia is thought to be either adult cows (which act as carriers without showing signs of disease) or infected scouring calves passing the parasite in their faeces. Drew said: "The infectious dose of the organism is very low and if ring feeders are not moved regularly the disease threat that builds up can be very similar to the housed situation. Faecal contamination of feed and water troughs can also be reduced by raising and covering them.

"Cryptosporidiosis in calves is often seen in combination with other diseases, particularly rotavirus. So vaccinating cows against rotavirus with Rotavec-Corona one to three months pre-calving - as well as a sound colostrum feeding regime and maintaining high hygiene standards - can often tip the balance in your favour."

Intervet Schering-Plough says that if cryptosporidia is diagnosed as the problem, parasite replication and excretion can be reduced with Halocur. This is the only product licensed to treat and prevent cryptosporidiosis. Administered orally to calves after feeding (daily for seven days), it can reduce the severity of diarrhoea and prevent the infection spreading to other calves. Treated calves have also been shown to require fewer antibiotic and anti-inflammatory treatments, as well as less rehydration therapy.

Vetoquinol has released the second set of results from a survey of over 1000 dog owners which the company says has revealed some surprising findings on owner attitudes to pain and pain relieving medications.¹

In particular, Vetoquinol says it thinks veterinary surgeons will be surprised by the depth of feeling expressed by pet owners about the issue, and that there are areas where there is potential for the profession to relate to the concerns of clients and adapt communication accordingly.

The key findings highlight that dog owners are likely to be shocked by a diagnosis of long term painful illness in their pet and are very likely to search for further information on the Internet about the pet's condition. Most dog owners think that NSAIDs are likely to cause side effects, with the perceived risk being much greater than the actual level of risk. The results also indicate that even low levels of risk of side effects may be unacceptable to dog owners.

• In common with many other surveys, most dog owners (68%) felt their dog was a member of the family and another 22% said their dog was their best friend.
• 73% of people selected the word 'devastated' to most accurately describe their feelings if their dog was diagnosed with a potentially painful long-term illness.

Mark Leddy, Marketing Manager at Vétoquinol said that the strength of the bond between pet and owner should always be front of mind: "A diagnosis of osteoarthritis, for instance, is such a common one, often being made several times a day, so it is easy to become matter of fact about it; yet for the client that news may actually be quite traumatic and the impact hugely underestimated."

• Asked to rate pain, dog owners perceived the most painful condition to be a broken leg.
• On a 0 to 10 scale, with 10 being most painful, 57% rated pain due to chronic arthritis in old age as 8 or above and 36% rated post-neutering pain as an 8 or above.
• Fewer people rated pain due to arthritis in a young dog as 8 or above (51%). Vetoquinol says this suggests that owners might not be good at identifying pain levels in individual animals.

Where clients get their information from is important:

• 98% said they would use the Internet to get information about their dogs' illnesses and medication, whilst 67% said they would do this every time they had a concern. Vetoquinol notes that this means there is greater scope for misinformation.

The majority of respondents (95%) felt that treatment of osteoarthritis should be started as soon as signs appear and then long term, either consistently or as needed. However, despite recognising the need for treatment, many expressed concerns about side effects when giving their dog long-term medication, with 77% considering it important or very important to choose a medication with a limited range of side effects. 

• 48% of those polled felt that NSAIDs were good treatments with a risk of side effects
• 15% rated them more positively as good, effective treatments
• 2% claimed they would object to or resist their vet's recommendation to treat with NSAIDs.
• Only 1% felt that pain controlling medications would never cause side effects, with most respondents claiming side effects were most likely to be seen sometimes (67%) or often (19%).

Vetoquinol says the reality is that one in a thousand pets given an NSAID is likely to experience side effects, which vets may feel is not something owners should worry about unduly. The reality for pet owners is that they perceive the risk to be much greater and if informed of the actual level of risk may still experience a significant level of fear that their pet, with whom they have a special relationship, could be that one in a thousand.

Mark Leddy says that, based on these results, there is great scope for the veterinary team to discuss the level and type of risk entailed in administering medication: "Such an approach will help pet owners make more informed decisions about treatment, allay their concerns and provide them with the support they need at what is probably a very difficult time for them."

Paul Aldrige MRCVS has won £2000 from Hill's Pet Nutrition after removing a cluster of uroliths from the bladder of an English bulldog.

He won the prize after sending the uroliths to the Minnesota Urolith Center to participate in a competition sponsored by Hill's UK in support of the launch of new Prescription DietTM c/dTM Reduced Calorie for cats.

Hill's says that although the winner was a canine patient this time, it was good news for the Bulldog that the sample was sent in, as the uroliths in question were found to contain urate.

Purine uroliths, including urate, accounted for 6.4% of uroliths submitted to the Minnesota Urolith Center between 1981 - 2007¹. Staff at Center said that urate uroliths can vary in size, color, and texture: "This particular sample represents a good example of the need to submit uroliths for quantitative analysis. Physical appearance alone is insufficient to characterise mineral composition."

English bulldogs are one of the 66 breeds identified by the Minnesota Urolith Centre known to be affected by urate urolithiasis. Hill's says that foods such as its Prescription DietTM u/dTM can be used to help reduce the risk of uroliths recurrence in the future.
