The Royal College has announced that a Legislative Reform Order (LRO) to reconstitute its disciplinary committees separately from its Council has been signed by Defra Minister David Heath, and will come into force on 6 April 2013.

The LRO will amend Part I of Schedule 2 of the Veterinary Surgeons Act 1966 (VSA) and require that the RCVS Preliminary Investigation and Disciplinary Committees are made up of veterinary surgeons and lay members who are not RCVS Council members, and who are appointed independently.

This will ensure that the same group of people is not responsible for setting the rules, investigating complaints and adjudication, and will bring lay people formally into the Preliminary Investigation Committee.

The LRO will also allow the RCVS to increase the pool of people available to investigate complaints and sit on disciplinary hearings, reducing the workload on the individual Committee members whose primary appointment is to RCVS Council.

The RCVS has been working on the LRO with Department of Environment, Food and Rural Affairs (Defra) officials since late 2010, and the Order was based on consultations undertaken by the RCVS and Defra.

According to the College, the Order was commended at every stage of Parliamentary scrutiny as an effective means to address the single biggest deficit of the VSA and to improve how the RCVS regulates the profession. During debate in Grand Committee of the House of Lords on 10 January, the RCVS received considerable praise for its strenuous efforts to modernise under the constraints of the present legislation, and the LRO received unanimous support.

Following the Order coming into force, the first external members will join the Disciplinary and Preliminary Investigation Committees from July 2013. After a two-year transition period, members of the RCVS Council will become ineligible for membership of these committees. Information about how to apply to join these committees will be available shortly.

President Jacqui Molyneux said: "I am delighted the LRO has been made and I am immensely thankful for the hard work of the Defra team and my colleagues in the College. The LRO is the single biggest reform to the regulation of veterinary surgeons since the 1966 Act, and it will bring the RCVS in line with regulatory best practice and improve the perception of the independence of the RCVS disciplinary processes."

CVS is launching a New Graduate Farm Vet Exchange Programme to give young vets with up to two years’ experience the chance to spend time at another farm practice to develop their skills.

Launching this spring, CVS says it's the only programme of its type in the veterinary sector.

The programme will be open to all of CVS's young vets and will operate within all 12 of its farm practices in the England, Scotland and Northern Ireland.

Each CVS farm graduate vet will be able to go on an exchange in their second year of the programme and every placement will last up to two weeks.

The subject matter of each exchange will be at the direction of the graduate rather than following a specific programme, so participating graduates have the freedom to explore their own areas of interest.  

Anna Bruguera Sala, Programme Co-ordinator and vet at Alnorthumbria Farm Vets, said: “The major benefits of our new graduate exchange programme is that it is open to all young vets of different levels of experience and provides bespoke learning opportunities tailored to each individual.

"It will also create connections, so experienced colleagues can support an exchange student with their new skillset when they are back within their own practice.”

According to research conducted by Petplan, 65% of dog owners say they find visits to the vet difficult because of their dog's anxiety. 

25% of owners said they have seen their dog running away on hearing the word 'vet' and 30% have actively avoided a visit to the vet because of the animal's stress. 

To try and get round the problem, 30% of owners use language to avoid alerting their animal: either spelling out the word 'V-E-T', or using a different word, like the 'dogtor'.

47% of owners disguise the trip and take their dog for a walk first, whilst 46% give their dog extra treats that day.  

Zoetis Business Consulting has announced the launch of Profitsolver, a software tool designed to boost practice profits.

The software has already been rolled out in the US, where Zoetis says that it has delivered an average of £103,000 extra profit to veterinary practices within a year.

Profitsolver uses the practice's actual data - not benchmarks or accepted norms - to enable the practice to manage costs, set prices and gain return on investments. By using an activity-based costing model and using a variety of data such as practice costs, which staff perform which part of the procedure, how long the procedure takes and what equipment and materials are used, Profitsolver calculates the correct pricing to both cover costs and deliver a predetermined profit margin.

Zoetis says that Profitsolver helps practices to understand how the overall practice profit is being generated. For instance, for price sensitive procedures such as vaccination, the practice can match their prices with the competition, but maintain their profit margin by increasing the price for other clinical services.

Nick Steele, national consulting manager at Zoetis said: "Veterinary practices have no standard method for pricing their services and they are often unable to determine which services make money and which don't. This hampers the practice's ability to accurately value the business and, as a result, profit levels tend to be low for many practices. Profitsolver can turn this situation around and already has a very successful track record in the US."

Zoetis piloted the UK version of Profitsolver with John Goulding at St Georges Veterinary Group. He said: "By and large the veterinary profession still relies on the mark-up of medicines where profits are concerned. Even practices that have made a concerted effort to move away from this type of pricing structure are still making their money from the sale of medicines rather than from vets' fees. For years I have wanted to know exactly how to price our services and this new Profitsolver software does just that.

"Most costs have an element of staff time, an element of fixed assets or equipment and an element of disposable items. Profitsolver takes all of these into account, adds the predetermined profit margin and produces an accurate pricing structure for each procedure undertaken within the practice.

"It's important to take the time at the start to put accurate data in from the practice's latest set of accounts and the Zoetis business consultant will guide practices through this to make the initial set up as easy as possible. For instance, for major pieces of equipment such as X-ray machines, you have to work out how many times a year you are likely to use them.

"By using Profitsolver, we worked out that if we do exactly the same amount of work as last year we can increase our bottom line by at least £200,000 for our 18 vet practice. Since last November, when we installed Profitsolver, we have completely changed our pricing structure moving away from focusing on medicines and the size of the animal to rethink how we charge our clinical care.

"An unexpected benefit is that Profitsolver has made me much more confident about the prices we charge to clients as it is all broken down, so we can see exactly how the invoice is made up. I would be at a loss without it now."

In an article published in the autumn edition of the Equine Veterinary Journal, Professor René Van Weeren, Chair of Equine Musculoskeletal Biology at Utrecht University, suggests that equine lameness needs to be redefined to reflect new digital assessment techniques that use preset parameters to detect gait asymmetries.¹

The traditional method for detecting equine lameness is subjective, using expert visual evaluation of gait to identify the presence or absence and degree of asymmetries. The lame horse is described as having a disorder, defect or loss of function and this clinical diagnosis has associated welfare implications if the horse is still asked to perform.

The article explains how advanced computer technology is starting to change all this. Optic motion capture or the use of inertial measurement units (IMUs) is now enabling the detailed study and quantification of the horse’s gait; objective assessment against preset thresholds. The paper points out that while these digital systems can overcome the temporal and spatial limitations of the human eye they simply rely on one or a number of set gait parameters to decide about lameness and this can present obstacles.

Thresholds are forcibly based on a limited reference population, which doesn’t adequately reflect the millions of horses in the world. The individual environmental and mental conditions for each horse and its day-to-day gait variations are also not accounted for. On this basis to use the term ‘lame’ because a horse demonstrates a subtle gait alteration, causing it to fall below the threshold, could be inappropriate, particularly as it may not affect the horse’s welfare in any way.

Professor Celia Marr, Editor of the Equine Veterinary Journal said: "An important first step is for researchers and clinicians to start to discriminate clearly between 'asymmetry' and 'lameness' and not to use these as interchangeable terms when interpreting gait analysis data. Asymmetry is often, but not always, a hallmark of lameness, but is not a clinical term, whereas lameness is."

The RCVS Veterinary Nurse Disciplinary Committee has ruled that Somerset-based William Kane is fit to practice after he was convicted of three counts of causing serious injury by dangerous driving.

Mr Kane was sentenced to two years’ imprisonment, suspended for 18 months in September 2024.

He was also disqualified from driving for two years until he had passed an extended driving test, and was ordered to undertake 200 hours of unpaid work within 12 months, and to pay a victim surcharge of £187.

It was alleged that the conviction rendered Mr Kane unfit to practise as a veterinary nurse.

From the outset, Mr Kane admitted the facts contained in the charge, but did not accept that this rendered him unfit to practise as a veterinary nurse.

After the criminal proceedings, Mr Kane wrote to express his deepest regret for his actions and the pain and guilt they had caused, stating that there was not a single day that had passed that he had not thought about the family that was affected by his actions, and the lifelong consequences that they now face.

He went on to express that he understood that as a member of a trusted profession, his conduct must reflect not only his clinical competence, but also his integrity and accountability.

He also noted that he had sought to recognise and address the consequences of his actions and that he is determined to make a positive contribution to society and to the profession.

The Committee found the charge proved on the basis of Mr Kane’s admission and the Certificate of Conviction.

The Committee found a number of factors relevant to the case:

• there was no explanation for the accident
• Mr Kane was not intoxicated, nor was he distracted by using his mobile phone
• he was not carrying out any type of manoeuvre, for example overtaking where he shouldn’t
• his car was not in a dangerous condition
• no concerns had been raised about the manner of his driving, other than the moment immediately before the collision
• the judge repeatedly described the accident as a ‘momentary lapse of attention’, whereby Mr Kane’s car drifted into the opposing lane of traffic for a couple of seconds and there was then a catastrophic collision
• whilst the consequences were severe, the culpability, said the judge, was low and just crossed the threshold between careless and dangerous driving;
• the judge found there to be no aggravating features
• the judge, who had the benefit of watching dash-cam footage from another car that captured the collision, described it as a ‘mistake’.

The question for the Committee was whether Mr Kane’s conviction of causing serious injury by dangerous driving, when put into proper context, rendered him unfit to practise as a veterinary nurse.

This was not a conviction in any way linked to his practise and there was no suggestion that he represented any sort of a risk to animals in his care.

The Committee was not, therefore, concerned with any issues arising out of the need to protect animals, but rather with whether a finding of unfitness to practise was needed to uphold standards of conduct and behaviour in order to maintain public confidence in the profession.

Kathryn Peaty, Chair of the Committee and speaking on its behalf, said: “The Committee took into account the public interest which includes maintaining public confidence in the profession and the regulator and upholding proper standards in the profession.

"The public interest also includes allowing an otherwise competent veterinary nurse to continue in practice, where appropriate.

“Agreeing with the judge that this was indeed an exceptional case, the Committee did not consider that Mr Kane’s conduct was liable to have a seriously detrimental effect on the reputation of the profession and concluded that the public, in full knowledge of the circumstances of this particular case, would not expect a finding that the conviction renders him unfit to practise as a veterinary nurse.

"Rather, the public would recognise that whilst the consequences were appalling and tragic for the family involved, in terms of Mr Kane’s culpability this was a momentary piece of dangerous driving, categorised by the judge as a ‘mistake’ rather than anything more blameworthy.

“The Committee therefore concluded that Mr Kane’s conviction does not render him unfit to practise as a veterinary nurse.

"This is not to in any way detract from the catastrophic consequences for the family, but rather is to reflect the context, exceptional circumstances and level of culpability in this case.”

The RCVS has released the results of a competition held at BSAVA Congress earlier this month, in which veterinary surgeons, veterinary nurses and other members of the practice team were asked to identify three animals from sound only.

The correct answers were a purring cat, a Chihuahua dog (the breed had to be specified) and a guinea pig.

Of 370 entrants, only 104 (28%) were correct. Some of the more outlandish answers included, for the cat noise: elephant, lion, whale and dolphin; and, for the guinea pig noise:  ferret, meerkat, dove, chicken and frog. However, the majority of those getting it wrong were stumped by the requirement to specify the breed of dog, with Jack Russell Terrier or some 'yappy little thing ' proving a favourite.

The winner of the competition was Liverpool-based veterinary surgeon, Anna Rowntree, who won an iPod Nano. She said: "I thought the competition was good fun and a real novelty. It certainly drew me to the stand and was not what I would have expected from the RCVS - it proves they can be approachable! I was very excited to have won and I still can't believe I have."

The noises competition linked to the College's theme for the event, 'Falling on deaf ears?', which examined how the organisation takes account of responses to its consultations, debunking the myth that those who take the trouble to reply are not listened to. The topic was addressed by President Professor Sandy Trees in a presentation on the Saturday of Congress. Visitors to the stand were also given RCVS-branded earphones to reinforce the listening message.

The British Equine Veterinary Association has launched a pilot project to tackle equine obesity.

The scheme uses a traffic light colour system of vaccination reminder stickers which veterinary surgeons can place on the front of passports at each vaccination appointment. If the 6-month pilot is a success, the initiative will be rolled out across the UK in the summer.

BEVA says that despite the best efforts of numerous equine welfare charities to address the obesity issue, a significant proportion of owners are either not recognising obesity in their horses, or not being motivated to take action.

For this reason, the Association decided to try and confront the problem in a different way, using knowledge gained from the Behavioural Insights Team (BIT) – a government think tank specialising in behavioural economics and psychology.

Lucy Grieve, President Elect of BEVA and part of the Association’s obesity campaign working group, said: "Determined to look at new ways to positively engage with horse owners, we harnessed the BIT’s experience of what methods work most effectively and came up with a simple, practical scheme revolving around vaccination visits, which could be affected by vets themselves."

The idea is to use the routine annual or six-monthly vaccination visit as a time to assess a horse’s body condition. After the vaccination is given and the body condition assessed, a colour-coded vaccination reminder is stuck to the front of the passport, both to remind the owner when the next vaccination is due and to start a conversation about body condition. 

A green sticker indicates a ‘healthy’ body condition. Amber means the horse is carrying too much fat tissue and needs moderate changes to diet, exercise, management, rugging and clipping regimes. Red implies that the horse is carrying excessive amounts of fat tissue which are placing the horse in morbid danger.

Amber or red stickers provide the opportunity to talk about the potential impact on the horse’s health and what needs to be done.

If there isn't enough time to talk about it during the appointment, the sticker includes a colour-specific QR code which the owner can use to access additional information via their smartphone in their own time.

One of three short, colour-specific videos will explain the reason their horse has been designated the colour of sticker on their passport, leading to a link to more specific advice on what the owner needs to do next. Having swotted up on the background information beforehand, the owner can then discuss the various management options with their vet at a convenient time. 

Lucy said: "The first challenge is helping owners recognise when their horse is overweight. Once this has been established then we can make a plan to correct the problem as a team.

"The owner needs to be on board and committed in order to carry out the tough task of reducing the weight of their horse. We hope that owners will be ‘nudged’ by the sticker intervention to consider the information they have been offered and start to tackle the problem before it causes life-threatening disease."

Nine equine veterinary practices were invited to participate in the pilot scheme, including Loch Leven Equine Practice in Kinross. Managing director Liz Somerville said: "We have been focusing on equine obesity for the last couple of years including running a #FitnotFat campaign last year to try to highlight the growing obesity problem in our horses. Unfortunately, it sometimes feels that we are banging our heads against a brick wall so when BEVA came up with a new approach to try and get the message through to our owners it was too good an opportunity to miss."

BEVA says that at the end of the pilot, it will assess how well the project has worked for the vets involved, what proportion of owners used the QR codes, visited the advice pages and sought guidance from their vets.

Zoetis has produced new guidance to help veterinary surgeons develop successful strategies for controlling worms in foals and youngstock.

According to the company, information on best worming strategies for foals and yearlings remains largely anecdotal, making it difficult to develop a clear plan for this age group. Zoetis veterinary surgeon Wendy Talbot has therefore reviewed the recent literature to develop an eight-point plan. 

Explaining the guidance, Wendy writes:

Foals and yearlings are usually more susceptible to worms than are adult horses because they have had little chance to develop any immunity. They are more vulnerable to related diseases and tend to have higher egg shedding, which increases the risk of infection. The main parasitic culprits in the UK for foals less than six months of age are large roundworms (ascarids). In older foals and weanlings, small and large strongyles, tapeworms (and pinworms) are the main considerations.¹ Yearlings may also have a second wave of large roundworm infection at 8-10 months of age.² Any control strategy will need to take into account the individual circumstances such as stocking density, pasture management and previous disease history.

Guidelines for parasite control in foals suggest a first treatment with fenbendazole at 2-3 months of age, targeting large roundworm, followed by another treatment at 5-6 months. Alternatively, pyrantel may be considered for these doses; however, there may be resistance in some areas and foals with large adult ascarid burdens may be at higher risk of intestinal rupture when this drug is used.⁴ At weaning (approximately six months of age), it is advisable to perform a faecal worm egg count (FWEC) to determine if treatment for strongyles is also required; the macrocyclic lactone anthelmintics (ivermectin or moxidectin) are expected to have the highest efficacy. At 9 and 12 months of age treatment for stronglyes and a larvicidal dose for encysted stages of small strongyles (moxidectin or a 5 day course of fenbendazole) are indicated.  A tapeworm treatment should be included with one of these doses.^2,3

For yearlings, two methods have recently been proposed for worm control: the first involves using faecal worm egg counts (FWECs) at more frequent intervals than for adults to guide dosing for strongyles in the grazing season.¹ The second suggests three baseline treatments targeting strongyles in spring, summer and late autumn, with FWECs in between to identify and treat any still shedding high numbers of eggs, up to a maximum of six treatments per year.² Macrocyclic lactones have the highest expected efficacy against small strongyles and should form the basis of treatments; however, pyrantel can be considered for some treatments where it has been shown to have continued efficacy. Both strategies include a larvcidal treatment for encysted small redworm combined with testing or treating for tapeworm in late autumn. The inclusion of one or two doses of a macrocyclic lactone each year (usually already used for small strongyle control) should be sufficient to prevent large strongyle related disease.³

The wide differences in the circumstances of foals and yearlings, together with geographical variations in resistance patterns and a lack of consensus on best practice in this group means it is not possible to define a single approach to a worming plan. An important point to note is that FWECs may give surprising results because higher counts are generally seen in youngstock.

Eight ways to keep clients' youngstock worms controlled:

1. Clean pasture is key to keeping foals and youngstock healthy. Regular removal of droppings is crucial to a successful worm control plan.
2. All foals are considered to be susceptible and at risk of acquiring ascarid infection. Some degree of anthelmintic intervention should be considered and in most circumstances administration at 2-3 months of age and again at 5-6 months of age is advisable.⁴
3. Older foals are primarily at risk of strongyle infection; macrocyclic lactone anthelmintics have the highest expected efficacy.
4. Worm egg counts are likely to be higher in yearlings compared to adults and interpretation of them requires good knowledge of all the circumstances including management, clinical history, previous worm control and test results. Cut-off values for treatment have been suggested to be between 100-500 epg.
5. Moxidectin for the treatment of encysted small redworm is advised for all youngsters (> 6 months of age) in late autumn.¹
6. Faecal worm egg counts are advisable in late winter/start of spring to identify high shedders. These should be treated as necessary with moxidectin particularly after a mild winter.¹
7. New arrivals should be dosed with moxidectin and then quarantined for three days.¹
8. A yearly faecal egg count reduction test is advised to check efficacy against strongyles and ascarids for all class of drugs used on the premises.¹

The International Society of Feline Medicine (ISFM) is encouraging more practices to join its Cat Friendly Clinic programme in 2017.

Set up in 2012, the programme was developed to raise the standards of care and welfare of cats in the veterinary clinic, and to provide veterinary surgeons with practical solutions to problems that they face when dealing with their feline patients. The programme is also designed to boost business by strengthening the bond between the clinic and its feline clients.

Four years later, with 732 practices accredited worldwide (369 in the UK) and a further 1,000 US practices accredited under the American Association of Feline Practitioners scheme licensed from ISFM, the programme has become a well-recognised global standard in feline veterinary care. Last October, it was the winner of the 2016 Veterinary Record Innovation Award.

ISFM says it has now signed six new commercial partners for the scheme: Boehringer Ingelheim, Ceva Animal Health, Elanco, Idexx Laboratories, Merial and Royal Canin, and will be working with these partners to expand the programme further.

Lockdown has seen a rise in the numbers of dogs and cats in the UK, including imported pets. Horiba says some of these are coming in via the official route, which includes preventative measures to reduce health risks. However, many are moved outside of UK regulations and some will become ill in the days or even months after they arrive. Ian will explain how timely, accurate testing will help detect the threat to these animals.

The Animal Welfare (Import of Dogs, Cats and Ferrets) Bill introduced as a private members Bill earlier this year by Winchester MP, Danny Chambers MRCVS, has passed into law.

The new law allows the Government to: 

• raise the minimum age at which dogs and cats can be imported into the UK (as pets) from 15 weeks to 6 months.
• ban the import of heavily pregnant dogs and cats — specifically animals more than 42 days pregnant.
• prohibit the import of dogs and cats that have undergone mutilations (such as ear-cropping, tail-docking, declawing of cats).
• limit how many pet animals may be brought into the country under non-commercial rules to no more than 5 animals per vehicle, or 3 per foot/air passenger.

• require that the owner travels with the pets, or within 5 days of the pets arriving.

Danny said: “As a vet, I've treated many dogs with cruelly cropped ears or docked tails, leaving them physically scarred and emotionally traumatised.

"Knowing this Bill will help us put a stop to this abuse and have a bigger impact on animal welfare than I could possibly have achieved in a lifetime treating individual animals, demonstrates how important this legislation will prove to be.

"Thanks to the support of organisations like the BVA and other animal welfare nonprofits, we got the Animal Welfare Bill through the Commons, through the Lords, and today passed into law."
