Rule out fleas and stress, otherwise possibly allergic and usually if allergic would consider allergy to cats as most likely.

Clearly an unusual presentation!First test would be for Felv/FIV and then ...biopsy .

Both sides ? lots of coalescing lesions roughly the same size like biopsies almost, thrombophlebitis , bearing in mind age of cat could be hepatocutaneous syndrome , n-stage calcinosis cutis , NME ,recovered ATE just internal iliacs. mycosis fungoides, scaulding although its a funny place for it.
