Options appear to include:

1) Primary suture apposition

2) Lateral incisions and undermining of hard palate and then primary suture apposition

3) Wire (in some form) from 108 to 208 tightened enough to close gap

4) other materials from 108 to 208 holding together to close gap

5) encircling around maxilla with 40lb nylon leader line

I have done options (2) and (3) before, most recently doing option (2) on Sunday alongside an esophagostomy tube, and has done well (although did take until today to eat voluntarily, possibly mostly due to anosmia?). However I'm conscious that is more oral trauma than option (1), is more effort than option (1), and if option (1) is similarly reliable then perhaps I should try that next time instead. I used 5-0 monocryl plus simple continuous.

Many thanks!

Best haemostatic agents to have on hand for surgery?

New haemostatic gel claims to transform veterinary surgery - VetSurgeon News - VetSurgeon.org - VetSurgeon.org

Has anyone used this Vetigel themselves to give feedback on? It strikes me that if I have a severe bleed not responding could be useful to have, but would like to have some positive feedback from other vets first.

What other products are there that you can recommend in a haemorrhaging open surgery?

I was reading this report on PDA surgeries and was impressed with the super-low mortality rate in spite of relatively normal rupture rate:

Surgical ligation of patent ductus arteriosus in dogs: Incidence and risk factors for rupture - PubMed (nih.gov)

Surgical ligation of patent ductus arteriosus in dogs: Incidence and risk factors for rupture - Grimes - 2022 - Veterinary Surgery - Wiley Online Library

They quote use of "a topical hemostatic agent" amongst strategies used to stem bleeding after rupture, and this got me wondering what the best topical hemostatic agents might be to have on hand?

"Digital pressure, hemostatic clamps, hemostatic agents, and suture or vascular clips can all be used to control hemorrhage in the event of rupture. This low mortality rate is encouraging; despite massive hemorrhage, hemorrhage control and survival is possible with proper preparation, including blood typing, access to blood products, access to hemostatic materials, and knowledge of their application."

Thanks in advance!

-------------------------------------------------------------

(On a side note, can anyone enlighten me on what "hemostatic clamps" and "vascular clips" might mean within this PDA paper?

Does a hemostatic clamp mean something normal like mosquito forceps, or does it mean something like Satinsky forceps, or something altogether different?

It seems to have made itself a smooth little pit in the mucosa, not currently inflamed or obviously painful.

Are there still options to correct the position of this tooth now that it is fully erupted? I presume that things like ball therapy or even more advanced orthodontics are only useful before this point, but I'm happy to be corrected.

If correction of position isn't possible, should we be offering referral for crown shortening, to prevent issues where the tip of the crown contacts the mucosa? Or to prevent pain?
I haven't seen one for a while so hoping to seek advice about the best treatment plan for a 9 week old westie who has lingually displaced mandibular canines as per photo.
Bit off his food earlier this week so naturally I checked his teeth and found a mass lesion just below 409 and moderate gingivitis 407/408. mass lesion firm to touch. Remainder of his teeth/mouth looks absolutely fine. Fairly regularly check his teeth and he is on t/d (which he was still eating but not his wet food as much) so I think these changes are recent. See photos below. So xrayed and not sure whats going on, but the bony reaction on the mandible is where the mass lesion is and I am concerned it could be early neoplasia (although I really really REALLY hope not )

I think 407 and 408 will need extracting, although neither are mobile, but I also need to biopsy the bony area (unless it resolves as have put him on clavaseptin for the moment in case its osteomyelitis. he definitely seems brighter and is back to his normal appetite now.)

Would appreciate your thoughts on this and also best way to biopsy this? Both mandibular images were on same settings, so definite difference between the affected and non affected side
