&lt;p&gt;I think the clients do get a better OOH service from an OOH provider. The vet and nurse are on site so ready to deal with an emergency and inpatients get round the clock care that any heroic sole practitioner would struggle to provide. They are not waking up their own vet (though I used to find it really hard to sleep properly on call) and having to wait for him/her to come back to the surgery.&lt;/p&gt;
&lt;p&gt;Yes the fees are high but then they so they should be for a 24/7 veterinary emergency service. There are some down sides - there is often a lack of continuity of cases and sometimes transporting of patients to and from the user practice can be problematic.&lt;/p&gt;
&lt;p&gt;And I did work for an OOH service (not Vets Now) for 2 years and we never turned down an appointment. &lt;p&gt;I would never go as far as saying that &lt;span style="text-decoration:underline;"&gt;every&lt;/span&gt; OOH call should be offered an appointment on &lt;span style="text-decoration:underline;"&gt;every&lt;/span&gt; occasion. &lt;/p&gt;
&lt;p&gt;I would decide on a case by case basis. in an OOH clinic where one is working anyway, I would almost always offer to see private clients other than those which are clearly&amp;nbsp;utter nonsense such as nail clips 4am&amp;nbsp;and requests for flea treatment at midnight.&amp;nbsp;&amp;nbsp;PDSA clients when working for vets-now get a basic level of emergency care, most/many of these are told to come the next day; cats that have been sneezing for 3 weeks etc, and one I&amp;nbsp; once had was a GSD that had had diarrhoea for 3 years.&lt;/p&gt;
&lt;p&gt;If doing on call in addition to working all day (rare, last one was Jan 2010) I would only see what I considered to be an emergency, or if owners really insist. I see on call work as for emergencies only and not an extension of normal daytime service. &amp;nbsp;&lt;/p&gt;

I suspect it was embarrassment about fees,because when (on 2 occasions ) I did a shift for them when they were desperate, I was put under considerable pressure from nurses/lay staff to undercharge (which, of course I resisted )&lt;/p&gt;

&lt;p&gt;I agree with this, I am not saying I&amp;nbsp; personally would/wouldn&amp;#39;t act ethically. In fact I have never had an incentive based job. I am just saying I am sure that some vets (not all) with such incentives could be encouraged to perform non necessary procedures. I also feel that as vet we should all be recommending the best treatment/options/diagnostics etc without an incentive to do so. Our incentive should be improving animal health and welfare&lt;/p&gt;

&lt;p&gt;Why is it any different to a partner or sole owner?&amp;nbsp; Are you suggesting that a partner cannot be ethical bacuse his/her remuneration if directly influenced by turnover.&amp;nbsp; Infact a partner is more susceptable,&amp;nbsp; given that the costs of a practoce are largely fixed, any additional revenue made, &amp;pound;30 here &amp;pound;50 there&amp;nbsp;is 100% profit.&amp;nbsp; At least an assistant would only receive a %age of this.&lt;/p&gt;
&lt;p&gt;No if you cannot behave ethically, you cannot behave ethically and you need to look at yourself, it is no good blaming the incentives.&lt;/p&gt;

&lt;p&gt;You wouldn&amp;#39;t last long as my assistant, as I feel every OOH caller should be OFFERED an appointment-if they decline then any ill consequences are their fault&lt;/p&gt;
&lt;p&gt;I left Vets Now because of this very issue.They gave telephone advice to an owner of a vomiting dog,which subsequenyly died&lt;/p&gt;
&lt;p&gt;Any profits from the transaction, if the dog had been seen would have gone to Vets Now,not me, so I didn&amp;#39;t suffer financially directly&lt;/p&gt;
&lt;p&gt;I suffered financially because the owners who wanted to bring the dog in,but were too polite to insist moved to another practice which did it&amp;#39;s own OOH&lt;/p&gt;
&lt;p&gt;I also moved, so Vets Now have lost my retainer and any profits from my work&lt;/p&gt;
&lt;p&gt;Worst of all the dog is dead-and can never be brought back&lt;/p&gt;
&lt;p&gt;In this area a coughing dog is certainly an emergency-maybe you&amp;#39;re better at diagnosing Angiostrongylus over the phone than I am?-and I consider myself rather an expert!!!!!!!!!!!!!!!!!!!&lt;/p&gt;

&lt;p&gt;Incentives are a good idea but raise ethical questions? As a vet who has taken an oath perhaps we should alwasy be recommending th best for the individual animals. I alwasy recommending routine worming flea tx, dental care etc even though Im not n an incentive scheme. I would hate to think I would start recommending dental scales to animals with very mild plaque build up just to earn more myself if I didnt think the job really needed doing.&lt;/p&gt;

&lt;p&gt;I see the point however as a vet I do not feel its my job to see every coughing dog in the night. Many of my clients are quite happy to wait until the morning and just want reassurance/advice from a vet to say it is ok to wait until th morning.&lt;/p&gt;
&lt;p&gt;There are definately times I would see a coughing dog in the night (suspected heart failure, other serious signs or very distressed owner). Howveer there are some times where I feel it wouldn&amp;#39;t be ethical to advise an owner with a coughing dog to come to the practice in the night. In fact I am sure many clients I have spoken to OOH would be annoyed if I always said their animal needed seeing. &lt;/p&gt;
&lt;p&gt;Getting commission for OOH would be a nice reward however I am still unsure how many vets would see this as ethical practice? Would I be more inclined to see things OOH that I genuinely feel could wait? &lt;p&gt;In a situation where a more experienced colleague can reach&amp;nbsp;the same&amp;nbsp;diagnosis and clinical outcome by carrying out fewer investiagtions than a younger less experienced one might, the former although arguably&amp;nbsp;a better clinician will generate less revenue for the practice.&amp;nbsp; How can the better clinician be rewarded by finance led medicine? &lt;/p&gt;
&lt;p&gt;In the practice I am working at the moment, I do not advise pre op bloods for young healthy animals in for neutering where clinical exam is normal, yet a colleague does and therefore he generates more revenue. If we were on commision led pay (we&amp;#39;re not)&amp;nbsp;he would earn far more than I for something which in my mind is ethically questionable.&lt;/p&gt;
