Dear Wynne,

I meant that I was looking for contribution on content/fact/validated personal experience rather than opinion from the mentors, but I now realise that this is not your role. Mind you anything from some of the mentors would be good. The "referral practitioners" are a bit quiet on real stuff like the tribulations of general clinical practice, but I suppose referral practice is a bit esoteric. And there is a "practice director", who presumably can mentor other "practice directors". I'm a "general practitioner" which is catered for and also a "partner" and feel a bit left out in that respect.

On a more important note, it is gratifying that the VMD gives some clear guidance I am concerned that this is the same august body that says it would 'probably' not prosecute if a vet chose to dispense non-licensed medication on financial grounds if it were the only way to maintain animal welfare.

On the one hand they are indicating that the law is the law so decisions are clear cut, on the other they're leaving up to an individual to interpret if something's a bit illegal or quite illegal.

Dear Mr Guthrie,

Please don't call me "Sah". I'm neither Rhett Butler nor officer material. As for the mentors, well, they're obviously a sporting bunch and au fait with the mechanics of our profession, but some are a bit reticent and I thought I'd encourage them to make clear how much they know, is all.

JGW, Sah, I'm not sure why you single out mentors. The fact that they are mentors is only really relevant to this mentor scheme, not to how they contribute to the forums.

To add an anecdote and a question...

There once was another GHP (grey haired physician) who was contacted by an on-line pharmacy regarding a suspicious prescription. Apparently they had noticed that a prescription faxed to them appeared to have been altered, with the ghost outline of a label evident around where the date had been printed; indeed an earlier date faintly showed through the area of the (assumed) label. The practice confirmed that only one such prescription had been issued many months previously and the request for the ‘repeat’ prescription was rejected.

Being less contemplative and more the swift-to–respond-without-sufficient-thought type, the GHP contacted the local police. Puzzled but intrigued the pleasant female plod offered to go away, read some books, and get back to the GHP. When she did get back to said vet it was to report that a criminal offence had indeed been committed and that colleagues, as she spoke, were on their way to collect the miscreant and bring him in for questioning.

The ever-so-helpful lady PC later rang back to say that after application of thumbscrews and waterboarding the suspect had admitted the offence and had been released with a caution.

Said miscreant then phoned the practice involved and asked for a new prescription!!!! With delight the GHP refused the request and made copious notes on the relevant clinical record so that if clinical notes were ever requested the necessary information would be available to the next vet in line.

Now just how many GTPC regulations did the GHP break....?

I agree with you Bob There are individual members of Royal College Council, including some officers whom I definitely rate very highly, but "en masses " some decisions are puzzling to say the least-and how much is it the fault of Councillors, and how much the permanent staff ?

I am rather mystified that the RCVS seem to consider there are grades of illegality!!!! Do we now have to decide if something is a little bit illegal, a bit illegal or very illegal.

I am almost impressed with the VMD on this one!!!

If it is a vet running the on-line 'pharmacy' surely the VMD are responsible. If it is a proper pharmacy the controls are probably local NHS trust etc. There is a long list of agencies entitled to enter and check human pharmacies on-line or not.

Unfortunately the RCVS seems to be losing a lot of credibility with a significant proportion of the profession! There are some really good people on the committee - are they being ignored or is the bureaucracy getting the better of them?

Mentor's reply

This is something I feel very strongly about-in fact it's what 1st made me question the "wisdom " of RCVS Councillors. That was in 1987, when we were FORBIDDEN to report a suspicion that a client was involved in either organised dog-fighting, or badger baiting . I still question whether that advice was legal, as it seems to me to be aiding and abetting if a veterinary surgeon then treated the injured dogs. Until then, the understanding was that client confidentiality was paramount, unless a notifiable disease, or reasonable suspicion of a criminal offence were concerned

They've now shifted toa fence-sitting position, but my understanding of the law is that it is an offence to conceal a crime, so again I question the legality of Royal College's stance

My advice would be to obey the law, and report, and if there is a complaint, get a good barrister to point out to Royal College that they are not above the law

Love the bit in the guidance notes about contacting the RCVS FOR ADVICE. The only advice they seem to be capable of giving is how to be completely bloody useless! Oh and fence perching, they are very good at that! I hope Ms Nute is available for comment.
