My favourites:

TEETH - Tried Everything Else, Try Homeopathy

and, from a former practice, before black bags were the norm for cadaver disposal:

GBJ - Grey Bag Job (aka Terminally ill - sick, I know)

There is a good collection on a BBC web page here - http://news.bbc.co.uk/1/hi/health/3159813.stm. I particularly liked UBI (Unexplained Beer Injury).

But of course as it says in our practice manual section on acronyms (made in an attempt to translate increasinly cryptic clinical notes), "Remember all owners (and their lawyers) have the right to see their own clinical records - bear that in mind when making clinical notes of any sort".
