&lt;p&gt;Look at last night. We had a dog in with a swollen throat, probable stick injury. So what are we to do?&lt;/p&gt;
&lt;p&gt;The practice closes at 18:30. This case may need hospitalization post operation.&lt;/p&gt;
&lt;p&gt;Therefore you have to refer, in effect you have little choice.&lt;/p&gt;
&lt;p&gt;Once at a referral centre they will naturally do what they wish to do and here&amp;#39;s a critical point, the buck stops at the referral centre. In effect first opinion vets have it a little easier in that if it&amp;#39;s out their scope then a referral centre needs to sort it, that probably explains the CT scans etc&lt;/p&gt;
&lt;p&gt;To any new graduate, seek out a practice that does their own OOH, the case load is more varied and more interesting, your career will benefit. It&amp;#39;s not all about the hours in your formative years&lt;/p&gt;

Does it mean a slightly diluted version of what the referral centres practice? I it what the DC &amp;#39;experts&amp;#39; believe is necessary or is it what the majority of vets try to achieve within the welfare, budget and owner choice?&lt;/p&gt;
&lt;p&gt;My gold standard is the later! Referral is great and I appreciate the ability to send cases to people far cleverer than I am. I learn lots from every referral as well!&lt;/p&gt;
&lt;p&gt;Must get on to the 1CPD app and reflect! &lt;p&gt;There is no gold standard, it&amp;#39;s a name invented by vets wanting to make everyone follow their own agenda.&lt;/p&gt;
&lt;p&gt;It&amp;#39;s all about evidence based medicine and listening to the client. I&amp;#39;m having a run of clients with 14 year old dogs who frankly don&amp;#39;t want the CT scan and associated medical/surgical &amp;#39;gold standard&amp;#39; interventions. Many have been burnt by previous pets getting this. What they want is for vets to listen and make their pet comfortable, it was ever thus&lt;/p&gt;

&lt;p&gt;It really is no point in confirming a diagnosis of &amp;nbsp;hyperthyroidism for eg if the owner not going to treat&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Suppose another example would be a stable dog on OA meds. A SG and dips would tell you more than a renal panel&amp;nbsp;&lt;/p&gt;&lt;div style="clear:both;"&gt;&lt;/div&gt;</description></item></channel></rss>
