Working up your own cases and those of your colleagues (assuming they want this!) will be much less hassle than dealing with another vet in another clinic back and forward. With 20 vets I would think you have enough on your plate! Once we go above 3 consulting at once I find that things become a bit chaotic!

RE: if you could design your ideal job..
because if there are enough vets and in this I mean over 3, then having someone look after the cases and crucially mentoring those that take the bloods etc is great for everyone and helps to reduce referrals as well so keeping monies in the practice
Yes and no. Don't want to cream off all the fun cases and just leave the other vets with the boring cases and vaccinations. You have a very fine line to tread in my opinion.

Is this a new rota just for you or for the whole team?

RE: if you could design your ideal job..
have shied away from medical referrals in the past as I am usually really busy and hectic and just couldn't see how it would work so maybe this opportunity would give space for that?
I went to a David Church lecture at London Vet Show, always good value. He went through a case where the blood work was subtle but led to the diagnosis. In this case the sodium / potassium ratio of 27 shouldn't be relied on for Addisons and as he said 'young dogs, you don't want to miss that'

Now I'm the first to admit that you simply can't know it all, and that was a complicated case. In a 20 vet practice you are going to throw up lots of these cases where all vets just want is a bit of guidance or a second opinion and this would be wonderful to have within a practice, everyone wins

Having worked in a lot of places the one thing that does overwhelm people and also where a real difference can be made is managing complicated medical cases. In this I mean looking at the lab results and deciding on the next best step. In a busy practice these are often done at a bit of a pace and by vets who may not know the best thing to do. So I would almost disagree with your statement of

I genuinely see vets struggle with medical cases, not helped by many part time vets so continuity is an issue. (and locums )
