Could also be an adverb eg "it occurs biweekly". I'd take it to mean twice weekly.

I don't think I've ever heard / read this term before, but i would have assumed it to mean twice a week most likely, given that fortnightly would be used if it was, well, fortnightly.

I would probably say / write:

"every 2 weeks", or "on alternate weeks" or "every second week" or "give a week, then miss a week" or something like that assuming is related to medicine dosing (though I'm struggling to think of ever giving this instruction...? For itrafungol, I make a point of ensuring the dosing is well understood, but probably would summarize it as "week on, week off, week on, week off, week on, finished" or something like that?

Not that I know anything whatever about gardening, but I think a plant that lasts one year is annual, one that takes two years for its life cycle is not biannual but biennial.

Twice weekly. Twice a month is bi monthly. Every two weeks is fortnightly.

Not complicated or is it? In the publishing world it is not so clear. Americanism?

Cambridge dictionary suggests it can be either.

A quick google produces:

adjective
done, produced, or occurring every two weeks or twice a week.

so not very helpful really..... I would avoid using the term as it is ambiguous.
