If anyone would like to discuss further I’d be happy to.

- my ability was thoroughly tested when I sat my board exams and is still tested every day just doing normal work. I don’t need extra tests.

- I can’t always tell what tissues are when they are necrotic/inflamed/neoplastic +/- crushed by forceps. I’ve guessed before and got it wrong and the potential consequences are too severe for the patient.

- pathologists are here to help you. We like to help you get a diagnosis, prognosis and treatment plan for your patient, please think of us as part of the team. We can’t always do that if we don’t have all the pertinent information relating to the case. I try not to write sarcastic comments but sometimes I’m annoyed that I haven’t been able to do my job to the best of my ability and provide the most helpful report for you, due to the lack of clinical history. Would you send a surgical referral without a history/referral letter?

RE: Scissor sharpening
It's like asking you to rectal a cow with your eyes, ears and mouth shut.
why do this? what are you trying to prove to anyone? how dumb the pathologist is? how brilliant you are for outwitting them? the only entity that suffers when you don't work with the pathologist is the patient. if you biopsied a metastatic lesion in the liver that had spread from the gut then you would give a confusing picture to the pathologist if you didn't tell them you took the sample from the liver.

RE: Scissor sharpening
I think a history must be helpful
Back to cognitive bias, am I more likely to tell a farmer a cow is PD plus when I see some fluid and he tells me she's 30 days served, versus me looking carefully when cow running with a bull and could be at any stage of pregnancy/oestrus cycle? I'd like to hope not (always confirm pregnancy with heartbeat, check for twins etc) but the expectation the cow is in calf is there given the days in calf.

I genuinely wonder if the pathologist is looking harder trying to decide what tissue they are looking at and if that is normal CF me asking if it's a MCT or not and them looking for evidence to support that

RE: Scissor sharpening
It's a nice theory but the reality, as ever, is sadly different. Once a blade stops working effectively it's no use. We get through them quickly given the throughput and we haven't time for ones that don't work.
Nice try old fruit, but somewhat a category error. It's called reducing cognitive bias, and the reason we blind scientific studies.
