Diagnosed on clinical examination not by ECG but they seemed obvious due to the erratic tachycardia, dropped pulses and general inability to be active at all etc.

I think that I meant DCM, but the dropped pulses and tachycardia plus grey-white MMs responded well to the Vetmedin, regardless![/quote]I note also again a sentence in your OP: 'I started using pimendoban and my first case showed such dramatic improvement that it became my first-line treatment'.

I started using Pimobendan the moment it became available, before some of the cardiologists, for dogs with CHF due to mitral valve disease which had become refractory to other available medication. Ditto Dobermans in the final stages of DCM. The response in both was dramatic and increased the life expectancy of the Dobies by 18 months on average. I subsequently started it as my first line treatment in at risk dogs, I didn't need EPIC, QUEST or any other acronym trial to tell me this.

Now if that doesn't get a response form DM nothing will.

RE: Atrial fibrillation?

I stand corrected Martin.
B) You're not likely to correct AF without cardioversion which isn't likely to happen in our patients so treatment is aimed at controlling the resultant ventricular arrhythmia,

C) I think digoxin has had its day, a beta blocker or calcium channel blocker like Diltiazem may be effective but may be harmful if the dog is surviving on an escape rhythm (hence the need for an ECG). We know Pimobendan is useful especially in dogs with DCM which this dog is likely to have given its breed, but it will help control the arrhythmia as well, presumably due to its mode of action to sensitise the myocardium to calcium, similarly to a calcium channel blocker but don't use both together!

D) No doubt DM will be on to contradict all this!
