&lt;p&gt;Sorry, but this is the usual cry of those fixated on a percentage value printed on the side of a vapouriser which has one, so obvious, embarrassing misunderstanding.&lt;/p&gt;
&lt;p&gt;It depends on the minute volume of the patient and &lt;span style="text-decoration:underline;"&gt;not&lt;/span&gt; what it says on the dial which was &lt;span style="text-decoration:underline;"&gt;exactly&lt;/span&gt; why the %age is not put on the vapouriser!!&lt;/p&gt;
&lt;p&gt;My analogy of my drinking overproof vodka, but only a sip an hour, and my mate drinking 5 pints of lager is apt.&amp;nbsp; He&amp;#39;ll be *issed first, it&amp;#39;s the alcohol [or anaesthetic] intake over time that is the important thing!&lt;/p&gt;
&lt;p&gt;Trouble is the &amp;quot;experts&amp;quot; who condemn the system can&amp;#39;t seem to grasp this...&lt;/p&gt;
&lt;p&gt;Oh, BTW, circuit resistance is 2cc water and O2 consumption is what the patient uses, in theory, so no need to run high flows or even a spill.&lt;/p&gt;
&lt;p&gt;I&amp;#39;ve forgotten the %age of exhaled iso, but it isn&amp;#39;t 0%, but, with clever design, the exhaled iso [minus the CO2], plus the added O2 plus the iso from the vapouriser at &amp;quot;maintenance&amp;quot; will nicely keep an animal at S3 for as long as you like.&lt;/p&gt;
&lt;p&gt;The more potent the agent the lower level to fill the jar....&lt;/p&gt;
&lt;p&gt;This is why you have them on full &amp;#39;til S3 then on 3/8, with a Stephens, and is the principle behind 3/8 instead of 2% or whatever &amp;#39;cos the percentage means nothing if the animal is not breathing, and nothing if it is hyperventilating.&lt;/p&gt;
&lt;p&gt;And the vapouriser never needs calibrating. &lt;p&gt;Induce, tube, put onto circuit whilst filling up the bag, turn down once bag full. Job done!&lt;/p&gt;
&lt;p&gt;We have had uneventful anesthesia for nearly 17 years with our ADE! one recovery problem but that turned out to have a shunt!&lt;/p&gt;

The low flow aspect obviously means that the gas within the circuit is significantly diluted by recently exhaled gas. This has a massive impact during the transition from induction to maintenance - when the patient&amp;#39;s exhaled gas has low levels of anaesthetic.&lt;/p&gt;
&lt;p&gt;The fudge of using higher flow rates and higher percentages of anaesthetic during the transition from induction to maintenance means you don&amp;#39;t really know exactly the percentage of anaesthetic gas that the patient is inhaling, and apnoea magnifies this issue.&lt;/p&gt;
&lt;p&gt;In contrast, I saw a similar low flow re-breathing system for sale at LVS last year which was specifically designed such the inhaled gas came preferentially directly from the vaporiser (and therefore at a more exactly known percentage of anaesthetic), and was said not to require a higher flow rate or higher anaesthetic percentage during the transition to maintenance. I can try to track down it&amp;#39;s name if anyone is interested.&lt;/p&gt;
