&lt;p&gt;Not so much of a big deal for me, as you can pay yourself a small salary tax and NI free (currently 8k pa) plus the first 5k of dividends are tax free. &lt;p&gt;Yes, I am agency-free, and tend to have long relationships with practices. If it&amp;#39;s a regularly quiet place where I get an extended break (but still available for unexpecteds), I will adjust accordingly. You are correct - it costs me to have a holiday, various professional body fees, the concern of &amp;#39;letting down&amp;#39; a practice thro illness etc.&amp;nbsp;&lt;/p&gt;
