I also expected the practice to grow to allow a second member of staff. It still could perhaps but vagaries of bank funding has always made that too high risk a step to take.&lt;/p&gt;

&lt;p&gt;Certainly things have changed since the 1970&amp;#39;s but many are not happy with them. Since retirement I have been working in the NHS (in a voluntary capacity) and I can confirm that many patients I speak to are unhappy with the service they get fron their GP&amp;#39;s.&lt;/p&gt;
&lt;p&gt;Of course I don&amp;#39;t think it is acceptable to work for 48 hrs without sleep. The practice I referred to in my post had what might be described as a &amp;#39;night shift&amp;#39; the staff so employed could sleep during the day.&lt;/p&gt;
&lt;p&gt;I think the change in &amp;#39;gender balance&amp;#39; does have a bearing on this - and also the perceived security risk to women working on their own at night. True also that 50 years ago most veterinary surgeon&amp;#39;s wives were full time &amp;#39;home makers&amp;#39; rather than career orientated. To return to the medical profession many GP&amp;#39;s are now women and have family committments which result in them working part time adding to the overall shortage of doctors.&lt;/p&gt;
&lt;p&gt;I recently attended an NHS Clinical Commissioning Group meeting where a local GP told me he found it virtually impossible to recruit doctors for his practice willing to work full time let alone &amp;#39;unsocial hours&amp;#39;.&lt;/p&gt;

You&amp;#39;d be surprised at what the population gets up to in the evenings down here! We lost our one and only bakery a couple of years ago so if you don&amp;#39;t make your own bread you have to buy imported white or brown from the UK. I know where Francisco is coming from though. &lt;p&gt;The use of OOH services has really allowed many corporates to exist. It controls the cost of running the practice without severely affecting profit. I doubt many practices make a profit from OOH although they might if they charged as one OOH group does. This was the final straw for me. An &amp;pound;800 bill for a 1&amp;frac12;&amp;quot; wound!&lt;/p&gt;

&lt;p&gt;Perhaps the reason why some independent practices are struggling is that many no longer provide a 24/7 cover for their clients. One local practice refers &amp;#39;emergencies&amp;#39; to an &amp;nbsp;out of hours&amp;#39; veterinary clinic some 20 miles away. If an owner lives on the wrong side of the practice this can mean a journey of up to 30 miles. Add to that that the owner may not own a car this can create real and expensive problems. &lt;p&gt;[sigh] after pages and pages we get to the point which is that good vets are good, &amp;nbsp;and not because of what they label themselves but just &amp;#39;cos they are. &amp;nbsp;They also have the &amp;quot;art&amp;quot; of practice which, as usual, has been either ignored or sneered at.&lt;/p&gt;
&lt;p&gt;BTW, the best bread at the best restaurant I go to came from Waitrose so multiples can compete but only if they have the best products, ie vet. surgeons.&lt;/p&gt;
