 We are thinking about revamping our hospitalisation sheets and I was looking for ideas as to how others lay them out.

&lt;p&gt;We have the space on the kennel sheet and anaesthetic monitoring form that takes the standard labels printed from Teleos. The &amp;#39;animal details&amp;#39; label contains all the info we want.&lt;/p&gt;

&lt;p&gt;I took the template from the AnimalCare one I think it was and then trimmed the fat off it to fit our needs a bit (no need for it to be 24hr as we go home at 10pm&amp;nbsp;&lt;img src="/emoticons/v2/Happy_smiley.png" alt="Happy" /&gt; ). Still wasn&amp;#39;t sure what I would actually write in some of the boxes though which!&lt;/p&gt;
&lt;p&gt;Then I tried to upload it to Teleos so that could be printed daily with all animal/owner details on already rather than having to fill in by hand or print random sticky labels etc. This was a sticking point as it was required to be put in an rtf file format (who uses that these days!?) in order to be populated by the info on the teleos database. Then, for no apparent reason, it shrank it to be A5 size in the middle of an A4 page (I think I determined that it might have been possible to make it A4, not sure, but that would have required making all the receipts and other paperwork generated in the clinic A4 also...). Actually it might be OK still at that if you like plenty of free space for jotting notes down rather than wall-to-wall boxes to be completed - I can send you a copy if you like.&lt;/p&gt;

In short... it can&amp;#39;t.
