Michael Woodhouse Users control who they can receive messages from in their profile (choices are nobody / colleagues / everyone). I did ask the developers to set the system so that the default is everyone, and people can then change that setting as they wish, but I need to check whether that is still happening (in the software upgrade, the system might have reset to a default for new members of 'only colleagues').

The limitation with the alternative route is that you still the direct supervision (when seeing cases) by an existing diplomat so would likely need some kind of link with a referral centre/university. i believe most people achieve it by doing blocks of clinics at a centre alongside practice.

Hi Michael, and thanks for advice. ....Alternative rout of residency would be absolutely fantastic and I would definitively go for it without of second thoughts. I just didn't even try to think about it because it is almost impossible to get alternative residency in Europe (I was trying for years).

My concern would be if you have studied to PhD level then you would likely find the CertAVP offerings a little 'noddy'.

I would suggest you looked down the European diploma route. They all have to offer an 'alternative' pathway that is supposedly achievable in practice. You don't (or shouldn't) have to work in a fancy referral centre - so long as you have a sufficient caseload in your particular field..

We have appointments of around 20 minutes, students are taking histories but I must be present all the time, we are mainly referral clinic (first opinion only during OOH and weekends), and I'm seeing between 10 and 20 patients per day (over weekend probably 20-40 per day)......I would say that quarter of that are new patients (so around 1000/year).......not so bad if we consider facts that we have one nurse for 3 clinicians and no ACA-s, and that population of Croatia is about 4 500 000

I've worked in the University of Bristol first opinion clinic. this requires overseeing the students. Students greet the clients, take the histories and talk to the client. the clinician has to oversee this, talk to the client and check the diagnosis. In addition you have to debrief the student. Very similar I expect to Croatia.

10 minute appointments, 5 hours a day, so 30 a day

30 x 5 days = 150 a week

45 weeks for the clinician = 6750 cases a year.

Recent first opinion clinic

0830 start

10 minute appointments

4 hours in the morning, finish 1230

Start again 1430 finish 1820

Total appointments maximum 48/day

48 x 5 = 240/week

With 1000 appointments that's months work in a year

In this case over 10.000 cases a year.

Consider that the average consult charge is in the range of £30-£35, so to pay your salary of £40.000 you will need to see 1142 cases year (assuming £35) in addition your employer will need to pay NIC and overheads. Most employers will be looking at a vet taking in at least 3 times their salary to break even, so you'll need to see 3.428 cases/year to break even, assuming all can be charged at first consultation rate which is pretty unlikely.

Thanks for your comment Marie but why do you think that the caseload in practice is so different to what I am used to ? I'm normally doing about 1000 consults per year including medicine, dermatology and ECC (meaning wound management, unblocking cats, providing transfusion, dealing with poisoned patients, HBC, )...I'm definitely not good in surgery (just because I'm spoiled since I have surgeon on call for last 10 years :-))) but in my (limited) experience I don't think that caseload in standard practice in UK is much wider then in other parts of Europe.

[/quote]. I'm not sure what you mean by consults Iva but in General Practice in the UK a caseload of 3000 - 4000 consults pa (not including routine surgery) might still be common. Someone out there will correct me if this is wrong - maybe I've gone " Dino "

Thank you so much for being willing to help me with advices....Gaining as much UK experience as I can before definitive decision to move was exactly what I had in mind.Of course I'm delighted with Rory's offer and I've accepted it immediately :-) THANKS RORY!!!......I didn't consider gaining european specialist status (at least not jet) so I would say that AVP practice would be the prefect route for me (if you have any in mind please let me know) especially due to the fact that my ideal placement would be in smaller town. BTW. I have to correct my statement about the location-I'm absolutely not excluding North or any other location( I'm totally impressed with UK anyway) but it would be essential to be able get to any direct flight to Croatia in less then two hours. ....Yes I now I'm little bit demanding

I guess to some extent it depends upon what level of referral work you want to do. If you want be at the 'cutting edge' then specialist status is the only way but if this is not so important another route to consider is that there are a number of AVP practices around, usually situated in towns where there are no specialist centres in a reasonable radius. Some of them are impressively equipped with overnight vets/VNs, doppler ultrasound, videoendoscopy, CR radiography, even CT. With a number of imagers, cardiologists and anaesthetists now offering mobile services you would potentially be able to work to quite a high standard at such a practice.

These centres typically accept low-level referrals from smaller practices who do not, for example, have overnight hospitalisation. They are typically cheaper and so also may accept referrals of clients who may only have a basic level of insurance and cannot afford a specialist centre. If you find a job in one of these, you may find that there is enough medicine caseload such that you don't need to do a lot of surgery beyond the odd neutering or stitch up.

Of course you would need AVP status and as you say the RCVS may grant you this on the basis of your already impressive CV.

I would take up Rory's kind offer to go and see practice at a specialist centre, and maybe also see practice at a few AVP hospitals for comparison.

As a small animal practice on the South Coast who have recently advertised a new position opening up because of an increasing case load, I would say that a willingness to participate in "out of hours" and weekend rota is a bonus. We do our own "out of hours" and this has not been popular with the prospective candidates we have had applying. (It is tiring but I find it gives an extra opportunity to deal with some of the more challenging cases).

if your communication skills are as good as your clinical skills I think you would find a position quite easily. However you have to also factor in the cost of living.

Hi Andy, thank you for reply......I'm definitely hoping to continue my referral interests but unfortunately I don't think that residency is an option at least for the first year since if I decide to move I will move with my family and I guess that we will need some time to adapt and settle down. I was thinking more in direction of advance certificate (though according to my talk with RCVS they might consider me for advice practitioner status due to my clinical an academic references). I would certainly do some surgery CPD since I'm totally aware how much I need it.

I still didn't make any definitive decision (though I presume that I would be able to decide very quickly if the right opportunity would arise:-)).

The aim of my post was to find out what are preferences of UK veterinary vacancies market since you obviously are in need for vets and most of vacancies posts look similar (enthusiastic, experienced, great communication skills)...So I'm very grateful for all opinions, information, and advices.

i suspect that where you look might depend on what you are hoping to do longer term.

if you are hoping to continue your referral interests then looking to apply for a residency (for a European diploma) might be a consideration and I suspect your cv will stand you in a good position for these (with some experience in the uk first).

if you are aiming for primary care practice then maybe looking at some surgery cpd and then a very supportive practice might be preferable.

Of course it will take time to get confident and competent though I would say that I'm pretty familiar with lump removals and most of exotics...ferret with insulinoma at 3 am is absolutely my favourite ;-)

The main point of my post was exactly that...what does it mean experienced an competent and do you really believe that someone can be absolutely competent in all disciplines?..I must say that I didn't meet many people equally competent and (maybe more important) enthusiastic in surgery and medicine in the same time.

Because you would be expected to do surgery- not necessarily anything complex but neutering, lump removals and dentals are considered everyday procedures. If you aren't doing surgery now it will take time to get confident and competent.

Additionally you are likely to be seeing different species- rabbits, guinea pigs, chickens, budgies and tortoises are seen in most first opinion practices in the UK but this may be something you are already familiar with.
