I&amp;#39;m not doing vaccination appointments since we have vaccination clinic onsite. Btw. is it 3000-4000 pa caseload for practice or for one surgeon? I did ask the developers to set the system so that the default is everyone, and people can then change that setting as they wish, but I need to check whether that is still happening (in the software upgrade, the system might have reset to a default for new members of &amp;#39;only colleagues&amp;#39;).&lt;/div&gt;

&lt;p&gt;I would suggest you looked down the European diploma route. They all have to offer an &amp;#39;alternative&amp;#39; pathway that is supposedly achievable in practice. Students greet the clients, take the histories and talk to the client. the clinician has to oversee this, talk to the client and check the diagnosis. In addition you have to debrief the student. Very similar I expect to Croatia.&lt;/p&gt;
&lt;p&gt;10 minute appointments, 5 hours a day, so 30 a day&lt;/p&gt;
&lt;p&gt;30 x 5 days = 150 a week&lt;/p&gt;
&lt;p&gt;45 weeks for the clinician &amp;nbsp;= 6750 cases a year.&lt;/p&gt;
&lt;p&gt;Recent first opinion clinic&lt;/p&gt;
&lt;p&gt;0830 start&lt;/p&gt;
&lt;p&gt;10 minute appointments&lt;/p&gt;
&lt;p&gt;4 hours in the morning, finish 1230&lt;/p&gt;
&lt;p&gt;Start again 1430 finish 1820&lt;/p&gt;
&lt;p&gt;Total appointments maximum 48/day&lt;/p&gt;
&lt;p&gt;48 x 5 = 240/week&lt;/p&gt;
&lt;p&gt;With 1000 appointments that&amp;#39;s months work in a year&lt;/p&gt;
&lt;p&gt;In this case over 10.000 cases a year.&lt;/p&gt;
&lt;p&gt;Consider that the average consult charge is in the range of &amp;pound;30-&amp;pound;35, so to pay your salary of &amp;pound;40.000 you will need to see 1142 cases &amp;nbsp;year (assuming &amp;pound;35) in addition your employer will need to pay NIC and overheads. Most employers will be looking at &amp;nbsp;a vet taking in at least 3 times their salary to break even, so you&amp;#39;ll need to see 3.428 cases/year to break even, assuming all can be charged at first consultation rate which is pretty unlikely.&lt;/p&gt;

I&amp;#39;m normally doing about 1000 consults per year including &amp;nbsp;medicine, dermatology and ECC (meaning wound management, unblocking cats, providing transfusion, dealing with poisoned patients, HBC, )...I&amp;#39;m definitely not good in surgery (just because I&amp;#39;m spoiled since I have surgeon on call for last 10 years :-))) but in my (limited) experience I don&amp;#39;t think that caseload in standard practice in UK is much wider then in other parts of Europe.&lt;/p&gt;&lt;div style="clear:both;"&gt;&lt;/div&gt;[/quote].

If you want be at the &amp;#39;cutting edge&amp;#39; then specialist status is the only way but if this is not so important another route to consider is that there are a number of AVP practices around, usually situated in towns where there are no specialist centres in a reasonable radius. Some of them are impressively equipped with overnight vets/VNs, doppler ultrasound, videoendoscopy, CR radiography, even CT. With a number of imagers, cardiologists and anaesthetists now offering mobile services you would potentially be able to work to quite a high standard at such a practice.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;These centres typically accept low-level referrals from smaller practices who do not, for example, have overnight hospitalisation. They are typically cheaper and so also may accept referrals of clients who may only have a basic level of insurance and cannot afford a specialist centre. If you find a job in one of these, you may find that there is enough medicine caseload such that you don&amp;#39;t need to do a lot of surgery beyond the odd neutering or stitch up.&lt;/p&gt;
&lt;p&gt;Of course you would need AVP status and as you say the RCVS may grant you this on the basis of your already impressive CV.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;I would take up Rory&amp;#39;s kind offer to go and see practice at a specialist centre, and maybe also see practice at a few AVP hospitals for comparison.&lt;/p&gt;

(It is tiring but I find it gives an extra opportunity to deal with some of the more challenging cases).&lt;/p&gt;

&lt;p&gt;i suspect that where you look might depend on what you are hoping to do longer term.&lt;/p&gt;
&lt;p&gt;if you are hoping to continue your referral interests then looking to apply for a residency (for a European diploma) might be a consideration and I suspect your cv will stand you in a good position for these (with some experience in the uk first).&amp;nbsp;&lt;/p&gt;
&lt;p&gt;if you are aiming for primary care practice then maybe looking at some surgery cpd and then a very supportive practice might be preferable.&amp;nbsp;&lt;/p&gt;

&lt;p&gt;Additionally you are likely to be seeing different species- rabbits, guinea pigs, chickens, budgies and tortoises are seen in most first opinion practices in the UK but this may be something you are already familiar with.&lt;/p&gt;

&lt;p&gt;Thanks for your comment Marie but why do you think that the caseload in practice is so different &amp;nbsp;to what I am used to ? I&amp;#39;m normally doing about 1000 consults per year including &amp;nbsp;medicine, dermatology and ECC (meaning wound management, unblocking cats, providing transfusion, dealing with poisoned patients, HBC, )...I&amp;#39;m definitely not good in surgery (just because I&amp;#39;m spoiled since I have surgeon on call for last 10 years :-))) but in my (limited) experience I don&amp;#39;t think that caseload in standard practice in UK is much wider then in other parts of Europe.&lt;/p&gt;&lt;div style="clear:both;"&gt;&lt;/div&gt;</description></item></channel></rss>
