I would say a pretty big percentage. As far as they don`t die from other reasons I would say 40-50% of dogs over 8 years will need treatment for pyo. A similar, slightly lower number will get mammary tumours. We are allowed to these procedures with a "medical" reason...So if there are owners that wish to have theirs dogs castrated/sterilized I`m not gonna be difficult about it. It`s a silly old law made by pencil pushers who never operated a dog in their life.

To Gareths` question; I think you`d have some difficulties getting a job not speaking norwegian.Altough most people speak english, they prefer speaking norwegian when having to explain an anamnesis. I know of an irish guy who started in a surgery without speaking norwegian, but as he is living with a norwegian girl, and working in a surgery I think he learned it within 6-8 months.

I don't know the answer to that. There do seem to be a few, but that's purely a subjective opinion. Certainly they are quite pro-active in my practice, and will for example spay if bitches have repeat false pregnancies and so on, but it must mean more risky operations in older bitches. As I said, I don't really see the point in it.

Re: What hours do people work?

Approx what % of bitches end up with pyos?

Population control doesn't seem to be a problem Wynne. They are allowed to use chemical control methods, rather than spaying (though not all use them) and personally I just don't really get the point. In terms of responsibility, I do think they tend to have a strong identification with the idea that rights go hand in hand with responsibilities. 'Right to own a dog' goes with 'responsibility for its welfare.'

Re: What hours do people work?

We have infusion pumps for all cases, so fluid overload is a very small risk. There is no evidence that going hypotensive (I can't fine the paper you mention so don't know by what degree) peripherally affects outcome in anaesthetised 'normal' (i.e. young, fit) patients. The use of medetomidine will give you peripheral hypotension, but actually remains one of the few anaesthetics that maintains central blood pressure (which is what matters) - unless the paper measured central pressures then it's difficult to draw conclusions about it.

There are risks with IVFT also, especially in short surgeries where they are often forgotten during prep, passing instruments, paperwork etc. How many are you willing to risk over-infusing for the sake of satisfying a surrogate outcome?

Re: What hours do people work?

Was it just the very very very sick cats that were given fluid therapy?
Because the very fact it is a service not a commodity and the end price is variable.
Any time I want a service provided I look online, and one of the things I want to know is how much it costs.
It depends on the service. Maybe things are different in Australia but here I wouldn't expect to find a doctor's, solicitor's, dentist's fees or even an osteopath's fees on a website. If I wanted to buy something that's different.

However I do make a point of emphasising the quality of our service over the opposition who don't even intubate, let alone monitor of put on fluids. But I'm sorry to say that I tend to agree that fluids are an unnecessary expense for routine neutering just not with their reasoning.
I agree you don't normally expect to see the prices online, but I don't think that's a reason for not putting them up. I think the fact that the opposition is so much cheaper than you gives you a great chance to put your prices online and point out exactly what is included with your service. Our opposition don't even include analgesia in the price they give people over the phone, so for us it was a great way to ensure people are educated about why our headline price is more than anyone elses locally.

It seems strange to me that for professional services people are happy to have work done then wait until they get to reception before they nervously wait to find out just how much they have spent. No one would by a product without knowing prices, so why are services different?

We have people phoning every day to find out prices. They're not a big secret after all so why not make it really easy for people to find.

I also found that we have a lot of visits to our website outside of our opening hours, so by providing clients with answers to all their questions at a time they are looking for them puts us ahead of the opposition. After all, if they want an answer and we don't provide it for them, they will keep on looking and potentially find it at mr Cheap and Cheerful down the road.

We were also tentative about putting animals onto IV fluids until we started measuring blood pressure routinely, and a paper was published in the Australian Veterinary Journal demonstrating just how common marked hypotension was during routine surgery (aroud 46% of cases). Turns out hypotension is pretty common. It at least gives us the ability to correct hypotension quickly, and we have certainly seen an improvement in the recovery from anaesthetics with most dogs wakling out the door as though they haven't had a GA. We also figured that by the time we've put an IV catheter in, it's not that much more to hook up some fluids so we don't charge much extra for the fluids, but the perceived value to the client is quite large.

My life got much easier once I moved from the Uk back to Norway :). The works hours quoted here are similar to what I was working in England...and left little time for other activities. Now I work 9-16.00 monday to friday. No on-calls.No weekend work. No overtime(the expetional times it happens it is heavily compensated financially). And I am making about twice as much as I was making in my first job in the Uk.

I don`t think I could ever go back to 08.30-19.00 after this.
