&lt;p&gt;I just wonder if in your personal information/demographics section you should ask questions relating to marriage/partner, children, pets, outside interests/hobbies/sports etc all of which have a huge bearing on personal happiness and can change the whole perspective of the the ability to cope with pressures of work. &lt;p&gt;I tend to agree that some question related to how totally work oriented are you. Some of us become too involved with work and families pay the price!&lt;/p&gt;
&lt;p&gt;Might be a good idea to do a survey of close relatives at some stage! One of the practices I worked in the partners were very much reluctant to allow time off/money for CPD, didn&amp;#39;t encourage certificates and the like and most of us found that frustrating and slightly demoralising.  It certainly impacted on how happy some staff were on a work-front.
