Many things can be left to next day when there is time to think things through, get othe people's thoughts etc. my daughter has just spent two days in hospital with abdominal pain . Waited 48 hours to see consultant and was sent home on painkiller to await a diagnostic laparoscopy in June. The consultant told her that most people wait months to see her so that ws ok. Apparently as it's not an acute abdominal emergency they will manage it on painkiller, tramadol, naproxen and paracetamol till she can have the tests. I would be horrified to treat my patients like that. Mother is now dipping her hand in her pockets to pay for private op on Friday .
I assess my out of hours patients and often leave none urgent things till the morning though we do our own ooh , there are no ooh providers near here and we re a mixed practice

Re: BAVECC 24/7 survey

'19. In your
experience, how does out-of-hours customer service compare between an
on-call system and a dedicated out-of-hours provider.'

It depends.

I believe that the perceived service received to be 'on the eyes of the client' better with the on-call system, simply because the client is accustomed to this particular vet and practice, even if the only thing they do is to give a painkiller and keep the patient unmonitored in a cage for the night.

If you add to the question: 'in your experience', I would say that the OOH is far superior when:

- the staff is sufficiently competent and trained, and their knowledge of the practice and its tools is sufficient (I've been asked to work several times as a locum in a OOH practice where I have never been before, this with a QVN who was also a locum, and had never been in that practice either. Perfectly working x-ray machine, no idea of how to use it).

- Access to the clinical history of the patient is paramount in certain cases. ( - My dog is having these little white tablets, two a day, it's for something to do with his kidneys - it's not sufficient)

Where you ask if a dedicated OOH service gives better service than an on call serv ice, the choices are better, similar or worse.

But in my opinion a dedicated OOH (can) do a professionally better job (more experieced/specialised/skilled, better equipment, longer supervision and intensive care options. However an on call service of the own vet often gives a better personal care service (know the clients, your own vet, just the one case, not a mayhem of cases, often the owner can stay with the pet or assist instead of just having to hand it over). So it is not similar. For a PTS the own vet on call is miles nicer, for a GDV or respiratory distress or RTA the dedicated OOH might be live saving.

It should be up to the owner to choose what they prefer and/or can pay for. That is also one reason why I think that this obligation of 24/7 is not of this time anymore. Owners themselves can choose where to go. There is enough business interest to provide the service. It is not that there is nowhere to go, the problem at the moment is that there is no continuity of clinical histories. It would be great if clinical histories could be uploaded in a cloud and made accessible to clients at any time. I am sure that will come and solve the issue at some point. Or clients could carry a flash disk with the clinical data of their pet uploaded.

By a Dedicated Out of hours provider, do you mean any, deputising service that closes during the day or is there another definition?

[/quote]

Hi Malcolm,

Was thinking more dedicated out-of-hours staff. The provider could be the same business but use dedicated out-of-hours staff or trade under a different name out-of-hours. By this definition the service itself would be shut during the day as the staff will be off duty. 'In' hours ecc will come later.

Hope that helps and makes sense.
