Don't forget the scrubs need to be worn with clogs!

I think I was the one that started this 'shopping in scrubs' thing some time ago. When I lived in Sussex I was amused to see one of the vets from a nearby practice shopping in the local Co-op supermarket, clogs and all!

The only time I went shopping in a scrub top was when my shirt got liberally coated with anal gland secretions! I thought it better to get funny looks than have people looking suspiciously at each other to find out who was smelly!

Pretentious? Moi? As Miss Piggy used to say!

One thing I never thought Bath Referrals would be accused of is showmanship! Especially if anyone had actually met Alasdair and I. But with Alasdair's scrubs and the stethoscope round my neck... well I'm thinking I might need an agent now. If you want real style though, you should have seen Arlo in scrubs. Arlo - I dare you to post a photo of that here!

Yup - arrived but I have spent the last few days organising a visa fora friend of my wife to visit us. Over 100 questions and a tree load of paper to allow a three week visit.

It got buried among the dozens of emails about this!

I was watching in fullscreen (which, it turns out, removes the title bar, so my automatic CPD recorder can't tell I'm watching anything vetty - one for me to keep an eye on in the future methinks) but only 480p. Turning up the resolution does look nicer, but the lack of shades of red is still there (my suspicion is that the area which is one specific shade of red occupies the same area on the screen, just has more pixels in it). My general rule of thumb is that if the nerve/vessel is thin enough that I need to turn the resolution up to 720p to even see it then it can't be all that important anyway.

I think the issue occurs throughout, but was most obvious during the initial dissection where it was a little hard to see exactly what was being uncovered (and obviously at this point I was kinda hoping to see a facial nerve pop up complete with pom poms and a marching band). The issue with youtube did occur to me - you'd probably need to host it yourself which might cause bandwidth issues, although it is possible that the deterioration occurs at your end rather than the youtube end. I think it's a fair bet to say that the more compression algorithms it runs through the more data you lose (as each works by throwing away its own particular choice of "useless" information). At the least, make sure the compression ratio at your end is as low as possible (to see what a difference this makes, load up a gory picture in paint.net (or whichever image manipulation software floats your boat) save it as a JPEG and play around with the compression settings to see what it does to the preview).

As a slight aside (and no, this isn't genuine misplaced optimism for video #2) I watched a cool lecture fairly recently (I think it was on TED.org) about fluorescent dyes that could be given IV perioperatively and which bonded to proteins only found in certain tissues (such as nervous tissue) which mean they then glow under UV light making them almost impossible to miss. It would also give Alisdair a nice tan :-)

Ego only slightly bruised. And the mask was a mistake. But the prologue was filmed in theatre and local rules are that the surgeon wears a mask in theatre. I don't wear my scrubs shopping by the way. We will try and pay attention to the details like this for Return of Al the Knife

Oh, and another thing, are you watching in HD? I just noticed that when I clicked the youtube 'quality' button on the bottom right of the video, the default was 360p or 420p. Ratcheting it up to 720p HD made a very significant difference to the clarity.

Interesting - thanks for that feedback, Jon. I suspect this is not something we are going to be able to address with a different codec, because the video is first compressed here (which I might be able to tweak), but then uploaded to youtube which compresses it again (which I have no control over). I wonder whether it might help if the camera was closer to the action (macro) for certain shots. That can be tricky - any closer on this one, and I would have been shoving Alasdair out of the way! But perhaps possible if I just did it for key moments, once every 5-10 minutes.

Can you tell me where on the timeline the problem is most acute, or is it the whole thing?

Does it help at all if you expand the video to fill the whole of your computer monitor?

I found that too, Jon, but I thought that it was the fault of my monitor that suffers from that problem on several websites. Perhaps I have been blaming it unfairly.

It was not my intention to denigrate the surgery nor the filming (neither of which I am competent to criticise) but, following earlier comments on this site about stethoscopes, shopping while dressed in scrubs and dressing in scrubs for interviews, this appeared to be another instance. I accept that this was inadvertent but first impressions can be important.

I have fairly frequent contact with several medical consultant surgeons and after viewing the interesting video I was about to forward it to an ENT friend when I began to wonder whether the prologue would compound the impression, common in medical circles, that vets just play at being surgeons and do not need to take asepsis seriously. However, I shall still forward the video (with appropriate acknowledgments, Arlo, of course).

I am sorry if my comments have upset anyone, especially the surgeon concerned, and I accept the cudgels raised in his support.

I thought the video was brilliant, and anyone who knows Alasdair knows he doesn't do 'showmanship' (and anyone who DID do showmanship certainly wouldn't wear a mask in that manner ), he's a great bloke and an excellent surgeon. After work tonight I stayed on intending to catch up a bit but my eye was caught by this video as it landed in my inbox and I thought I'd have a look for a couple of minutes... 45 minutes later I'd watched the whole thing and still had a desk covered in unopened post!

Still, at least I was able to log the 45mins as CPD.

Langford's loss is Bath Referrals' gain.

Also I thought his idea about zero cons fee for service personnel as mentioned in the press this week was brilliant too.

I filmed it with Alasdair, and really, there was no showmanship intended. Alasdair came into the theatre ready to go, and I asked him to say a few words about what he was about to do. If anything should be criticised, it is my directorial skills! Point taken, and a lesson for next time.
