I think the photos will say more then my post. But I will try to start.
Female DSH, 6 years old. The problem with her nose has been started at February. The bump got started bigger and bigger. She has problem with her breathing

Re: Cat nose and eyes problems

Ah.
The answer is yes, the cat was euthanized. The owners didn't wanted biopsy.

I hate situations when you can't do anything. At least she is not suffering any more.

Has it been euthanased? I hope so. I don't agree with the comments about biopsying to give the client time to get used to the idea. I find this ethically unsound both in terms of fraudulently charging the owner (even if the intentions are right), and also suffering of the cat - I wouldn't want that lump biopsying for the sake of it. Get on with the euthanasia andput the poor bugger out of pain.

Re: Cat nose and eyes problems

sometimes saying that you are pretty sure that it is bad news but lets do a biopsy to check, is a good way to delay things a little while giving the owner time to get used to it. No not a cynical way of getting a few quid extra, and obviously not if funds are short of the time delay will cause suffering, but can be helpful. They seem to take the bad news better a few days down the line.

Re: Cat nose and eyes problems

Thank you, David. Your post was very useful. I know there is no other option apart PTS, but the owners are very sensitive and with heart problems, so I don't know how to say it very gently without any heart attack in my consultation room.

I will mention about biopsy, and they will decide. It is very sad that this cat is young enough.

From the xrays, I cannot see this being anything other than a sarcoma. There also appears to be lysis on the top of the head developing.

Re: Cat nose and eyes problems

Hello,

Yes, I have also very bad feeling about this case. And thank you very much for all your questions, Kate!

No, I didn't any smear, but it is very good idea. I was hoping to get some puss from there, but was wrong.

She is eating and drinking well, but the owners sad that sometimes she seems in pain. I was thinking about FeLV/FIV, but the owner sad that she was vaccinated every year. But maybe worth to check it anyway. I didn't try any medications, because I wasn't sure what I am treating.

So I think I will start from smear and then biopsy will be my next step.

Thank you very much for all your help.

It is so good to not be alone. :)

Bony destruction suggests neoplasia, fungal or less likely bacterial, would be my thoughts. Did you make a smear of what you aspirated? Even if it looked like just blood, it doesn't mean there weren't other cells that may assist your diagnosis, so if you didn't make a smear, I would repeat a fine needle aspirate and send to the lab, or take a biopsy.

How is she in herself? Have you run any bloods? Consider checking FeLV/FIV status and other causes of immunosuppression. Have you tried any medications?

Sorry for all the questions!

Re: Cat nose and eyes problems

I'd bet this is a malignancy and I'd put quite a lot of money into it.
A radiograph won't give you a diagnosis but it will get you closer and that may be all you need to advise the client, depending on the situation. Try some more radiographs and get an intra-oral one if you can.

For a diagnosis, biopsy as soon as you can. But in the pictures it looks like a malignant cancer.

Re: Cat nose and eyes problems

I'd biopsy, and would have a bad feeling until proven otherwise
