Initially presented to previous vets in Feb/March with 3 wk history of PUDPD, weight loss and soft stools. Owner says he spent 600 pounds on tests,

Re: 11yo Beagle with pitting oedema and PU/PD

Has she been outside UK.? Makes me think of Ehrlichiosis.

Re: 11yo Beagle with pitting oedema and PU/PD

Hi Emma - thought something was niggleing about beagle pain syndrome:

juvenile polyarteritis syndrome in beagles - does not sound systemic enough though :(

http://www.vetstream.com/canis/Content/Disease/dis60169

http://www.sciencedirect.com/science/article/pii/0090122992902138

but the renal disease sounds a bit like Alabama rot? But there does not seem to be necrosis of the skin or alopecia!

Still confusion!

Re: 11yo Beagle with pitting oedema and PU/PD

[quote user="ilanit"]Let us know if you find anything.
There's no fever. The Pro-BNP was low and after discussion with the owner he has decided a trial course of steroids for a week then PTS if no improvement. He is reluctant to do bile acid stim + ANA due to cost and also the fact that in any case the chances of a complete cure are low. Plus she's urinating now almost constantly and becoming reluctant to move.

Will see how she goes on the preds.

Re: 11yo Beagle with pitting oedema and PU/PD

challenging case!

Also thinking possible vasculitis. It's Beagle - could be SLE. Any skin involvement, fever? Check ANA?

Let us know if you find anything. Am quite curious.

Good luck

With persistent PU/PD the dog will be suffering from washout syndrome where your lab results may be the levels of a over hydrated dog ( i.e sl diluted) Low Creat due to wash-out and low residual muscle mass, sl ALKP, low urea - end stage hepatitis/ shunt?

What is appetite like and what does it eat? What is its protein intake like? On any other medication especially oestrogen? (take it is spayed?)

Blood smear to check rbc morphology, see if any regeneration?

Oedema - my DDs would also include lymphoma, vasculitis until proved otherwise. Doesn't sound circulatory as low BP would have other signs such as syncope, coughing etc

Re: 11yo Beagle with pitting oedema and PU/PD

HI

With that ALKP active primary liver disease unlikely unless end stage - I presume it was not on steroids?

Pitting oedema usually suggests venous/lymphatic obstruction or vascular issues in the absence of hypoproteinaemia. I presume lnn are all OK?

Cutaneous vasculitis would seem unlikely as there are few skin/haircoat changes.

RHS cardiac failure warrants consideration. Canine juvenile polyarteritis syndrome (in Beagles) also not represented.

Perhaps there could be a systemic vascular disease?

Re: 11yo Beagle with pitting oedema and PU/PD

Slight elevation in ALKP and low urea. Slightly underweight dog and lowish HCT. I think I would be looking for some liver function tests. With normal tp/alb cannot directly relate it to the oedema unless simply due to a hepatopathy.
