At 1am I had a cal of a donkey dystocia. The owner had been trying for several hours

Re: C-Section in donkey/horse

Congratulations Matthew

I think I would offer it in the same circumstances.

I know of a colleague who did a caeser on a horse, they did it under GA at the yard. One vet topping up iv, two vets operating.

The horse lived abd did fine, but the uterus bled lots and they had to do a running stich paralell to cut. Then closed as in a cow.

I think a bucket of j-lube and embryotome might have been 1st choice.

Let us know how it goes

Re: C-Section in donkey/horse

Martin

Thanks for the advice, good point. I have made full clinical notes on their record detailing the treatment discussion & risks explained. I am off this week but have asked the large animal partner to give them a courtesy call to check progress and to re-iterate the risks and possible longer term complications. I'm sure it will be fine as they have about 40 donkeys and, although looked after very well, they are trated like farm stock.

Well done, you. No doubt people will pour in to talk about late-stage complications in equidae, and uterus-closing suture patterns, but that was a very brave thing to do and was a decision that many would have backed out of.

My one recommendation, having worked in large animal practice for a number of years, would be not to predict future client mood from current situation - in other words, get yourself some arse-cover now while the going is good. It no doubt will continue to be good, but people are generally much more amenable about accepting risks and poor outcomes either before a procedure like this or straight after, when all is well.

My advice would be to get a partner (or person of similar standing) to have a chat with the client, and possibly even get a retrospective consent form signed, 'just for your records'. Make sure some indication of relative risk is included.

But well done, you - quite heroic. Have a few days off, I would.
