 I am trying to find forms on line with a list of questions to ask during history taking. I think I saw something similar before, but I am not able to find it at the moment (maybe I am not using the right words for my search...)

&lt;p&gt;I asked because I saw a case on sunday and, retrospectively, I should have asked about the dog&amp;#39;s diet. I rarely asked about diets because nowadays most owners use commercial diets, but you still occasionally find the ones that have their own ideas about what is best for their pets, and they don&amp;#39;t always tell you! &lt;p&gt;Have you read the VDS booklet on consult skills? We&amp;#39;re not supposed to ask many questions, just let them talk until they are done! I tried it, doesn&amp;#39;t work!&lt;/p&gt;

I dread to think what the standard consult fee would have to be, if all the questions on Mike&amp;#39;s forms had to be asked to everyone&lt;/p&gt;

&lt;p&gt;I think for GP, what is the problem, when did it start, how was it when it started and how has it progressed since are good openers. Do a physical exam and then ask more questions depending on the complaint and clinical signs.&lt;/p&gt;
&lt;p&gt;Don&amp;#39;t be led off on too much of a tangent by the history though. Why do you want them? Depending on the initial complaint depends where things go.&lt;/p&gt;
