And yes, you can top up with buprenorphine, just start low dose. Fentanyl patches are more about background analgesia really - and most effective when applied and working before the pain stimulus is applied. &lt;p&gt;We routinely hospitalise TPLO patients though we have debated whehter this is needed as few receive any special nursing or additional meds. We use pre op methadone and then start NSAIDs at anaesthetic recovery. Fewer than 10% will get additional opiates and many are lightly weight bearing within 12 hours of surgery.&lt;/p&gt;&lt;div style="clear:both;"&gt;&lt;/div&gt;</description></item></channel></rss>
