She's a 3yo (ish) crossbreed dog that the owners have adopted and brought back with them from Bahrain, she's been back for about 3 months now. I first saw her 4 days

Re: Potential leishmania case
is she has active dental infection and eye infection with swelling of soft tissue it could just be an infection. either bacterial or aspergillus or even a foreign body up nostril etc if it is one sided. ? and another one to consider is lungworm given the epistaxis and other bizarre symptoms.

i would maybe be more inclined to investigate the more normal angles. try some antibiotics and monitor for any improvement whilst you are running other tests. what were the blood abnormalities?? any enlarged lymph nodes? pyrexic?
Thank you, i did start her on antibiotics when I 1st saw her (not sure if I said that before, prob forgot!) to see if that would help but she sadly declined very rapidly. But thankyou all for your help anyway.

Re: Potential leishmania case
The way the head symptoms are unilateral does at least suggest that, as Maya says, you should not forget less exotic diseases. I'm sure you are really working hard on this one, but nasal radiographs and endoscopy are definitely indicated as long as you reckon she's fit for a careful GA. For instance, it's rather suggestive of something intranasal progressing to intraorbital abscess.
Thanks for all your responses, She was in fact due for nasal radiographs on the Wednesday last week but we were having issues with our blood tubes (the gel portion of the serum gel kept ending up on top so the serum was unobtainable) so postponed those until we got the biochem/haem back from the external lab, which we didn't get back until thursday evening, she then came in on Friday very depressed with the swollen eye and sadly came in on Saturday when my boss was in and was at that point blind in both eyes and was euthanased.
poor teeth with a lot of periodontal inflammation (much more than I would have expected given the state of the teeth)
I'm not quite sure what you mean here although it could indeed be significant. Could you be more exact - maybe post a photograph?

Sorry, don't have a picture, her teeth had almost no tartar on them but her gums were very inflammed.

Re: Potential leishmania case

The way the head symptoms are unilateral does at least suggest that, as Maya says, you should not forget less exotic diseases.
conjunctival swabs for leishmaniasis PCR are supposed to be very sensitive for active infeciton. if you have an antibody level doesnt mean active infection, just exposure unless it is a very very high level then it could be active. you need a rising titre approx 2wks apart to confim active infection.

dont know if any of that has been useful, but i would keep an open mind, just because the animal is foreign doesnt mean it can't pick up an infection or get an inflammatory process. would be interested to see what the end diagnosis is!!

good luck.
