Can I check the dose of Fragmin? The BSAVA formulary puts it at a loading dose of 100-200IU/kg IV then 200-300IU/kg q 6-8h although I thought it was 100IU bid too

Yes. We would use this for embolic disease, (as opposed to a thrombus) whether it is cats with ATE or dogs with pulmonary thromboembolism.

There's nothing you can do about a thrombus in the LA. Its like Russian roulette! However I do recall an anecdotal story of a big LA thrombus in cat, that had disappeared on a follow up scan months later. So natural thrombolysis I assume.

So for cats with thrombi, or smoke in a dilated LA or recurrent embolic issues the current treatment is:

Aspirin 75mg 1/4 to 1/2 twice a week PLUS clopidogrel (Plavix) 75mg 1/4 sid

Perhaps consider aspirin or clopidogrel (Plavix) but I don't know how effective these are if a thrombus has already formed. There is a study ongoing at the moment comparing aspirin with clopidogrel in prventing recurrence of aortic thromboembolism in the aptly named FATCAT study:

I've used clopidogrel in the past in a couple of cases but can't recall offhand if they seemed to do any better.

I have heard talk of using low molecular weight heparin in cats but don't remember if this was to deal with a thrombus sat in the left atrium, or to treat ischaemic disease once the thrombus had embolised...
