My wife had something similar some years ago Lichen Planus. It dissappeared as mysteriously as it appeared after much medical investigation.

I was meaning to hop back on to update - I saw the dog back for a recheck a week ago, and all the lesions seemed to be starting to regress slightly. So we may never find out what it was - I guess perhaps some sort of exuberant inflammatory response? If progress stalls again next step will be biopsy.

A long shot, but is there any possibility the dog has ingested any human medicine, especially any Vitamin D analogue?

Fan, TM, et al. (1998) JSAP vol.39 pp581-586 Calcipotriol toxicity in a dog.

The symptoms are not quite the same, but it gave me pause for thought.

Good question - they weren't noted when he was seen a week ago, and I'd certainly hope the mouth was examined (I didn't see him on that occasion). I can imagine the gingival lesions potentially being missed -ironically they are almost exactly where I would "hold" for much of an oral exam - but not so much the tongue lesions.

I don't know for sure what this is.

Calcinosis circumscripta is a possibility. Personally I can also see that there might be a viral cause or it might be due to mouthing a prickly plant, perhaps burdock.

Are these lesions really very recent or is it just that they have only just been noticed? Presumably the mouth was examined when he was seen for retching and pharyngeal swelling?

I'd suggest biopsy. You could excise one from the tongue. Tongue surgery should not be daunting – it's pretty vascular, but suture it up neatly and haemorrhage will cease. Simple interruted sutures of 5/0 Vicryl will be fine. Or you could snip one or two from the upper jaw – that might need nno sutures at all.

If it's calcinosis circumscripta than just running a scalpel through an excised nodule will pretty well give you your answer!

It's interesting that they run along the mucogingival margin like that.

I had a similar case back in my clinical days and if you search "Lingual Calcinosis Circumscripta in young GSD" in VetSurgeon's search your find a link to the gallery section and you'll find an old case of mine - not quite the same presentation but a biopsy confirmed my diagnosis. In my case there was a surgical option to remove the lesions.

Tips - Any surgery on the tongue involves lots of bleeding!
