&lt;p&gt;I tend just to advise castration unless there is a good reason not too, to me it is the treatment of choice and a permanent solution. Ypozane works for sure, but is expensive&lt;/p&gt;
&lt;p&gt;I would want to know if the prostate gland is enlarged first, and given the dog wouldn&amp;#39;t tolerate a rectal examination sedation may be needed.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;If smooth and symmetrically enlarged, and assuming it&amp;#39;s benign prostatic hyperplasia (most, &amp;gt;95%, likely to be), I would advise castration, so could all be done under one sedation/GA?&lt;/p&gt;
&lt;p&gt;If one, or the client, wanted a definitive diagnosis then one would have to go down the route of imaging +/- biopsy +/- prostatic washes +/- prostate specific blood test etc. Every case I can think of where a full investiagtion has been carried out, have gone round full circle and ended up being castrated or have Ypozane.&amp;nbsp;&amp;nbsp;&lt;/p&gt;

&lt;p&gt;Thanks for the interesting case. These lesions really do look like petechiae and ecchymoses so I would still be concerned about a clotting disorder and looking again at platelet numbers especially, bearing in mind you could be dealing with a thrombocytopathia, not necessarily just a thrombocytopenia.&lt;/p&gt;
&lt;p&gt;Has the dog ever travelled abroad? Is it worth testing for lungworm, anaplasma, rickettsia etc?&lt;/p&gt;
&lt;p&gt;Imaging would also be a good shout as there&amp;rsquo;s a chance neoplasia could be triggering something.&amp;nbsp;&lt;br /&gt;&lt;br /&gt;&lt;/p&gt;

Was causing penile trauma from aggressive humping etc. Owner refused to acknowledge that was the case.&lt;/p&gt;

&lt;p&gt;Prostate and bladder US scan may be helpful.&lt;/p&gt;
