There looks and feels to be bony protuberances beneath his jaw, is this osteomyelitis?
What would the treatment be

RE: What is the treatment for rabbit with abscess / poss osteomyelitis?

Agreed, this is an abscess with incisor apical involvement. I would also want to to oblique views to assess left and right incisor apices and premolars in a bit more detail, and identify how much bone needs removal. If you have CT then this is even better.

I aggressively debride, with surgical removal of all abnormal soft tissue, bone and teeth, then marsupialise the abscess and associated bony cavity to inhibit anaerobes and allow ongoing drainage and application of topical treatment (honey is great for this). Plus multimodal analgesia and systemic antibiotics.

You will also need to remove the remaining incisors in future as even if only one is extracted as part of the abscess surgery, as the others then won't undergo normal attrition and secondary problems will develop. I would opt to leave this further surgery until the abscess itself has been dealt with rather than create new open wounds in an already infected rabbit. You are likely to need to trim overgrowth in between procedures.

Be aggressive with these, leaving any infected material behind significantly reduces prognosis.

The basic rule of these abscesses is that they persist until the nidus of infection has been debided namely the affected tooth roots

Incisor removal is indicated

Abscess drainage and either marsupialisation or packing with honey soaked swabs or AIPMMA beads is warranted.

Culture (abscess capsule)

Antibiotics based on culture or (if refused) broad spectrum (I would use injectable penicillin)

Analgesia

This will take several weeks if it is going to resolve. Prognosis guarded
