I had an interesting chat with another very experienced locum

He had concerns with the very easy to understand scenario that librela stops nerves growing. As nerves turn over even in older dogs this could lead to amongst other things a loss of sense of smell and he maintained that some facebook groups were highlighting real concerns

As usual we had the same consensus. Yes in an older dog, but think really hard in any younger labrador with terrible elbows. In these I will start to discuss potential issues

The question is. "Are these being reported?' This is the best way to get it on the data sheet in my opinion

Had one patient whose owner reported PUPD and opted not to have another dose, 2 months later tried agai and dog suffered suspected flomerulonephritis

Yes!
My elderly terrier was on Librela for 7 months when she became seriously ataxic and weak behind. We considered euthenasia but she seemed happy in herself so got her some support wheels instead. After 3 months she was walking unaided and she now goes on 6km walks again. Of course it may be coincidence, but…a similar issue happened with a colleague’s Schnauzer, so although rare, I am beginning to wonder if interference with NGF may cause a transient ataxia…?

There are anecdotal reports of PUPD after injection of librela. I have not seen this but a colleague was talking about this the other day

In an elderly dog there are lot of potential causes for transient neurological type signs - but coincidence is a powerful aversive.

I'd probably try once more but it would depend on the severity of the underlying DJD I guess. This really puts you in the clients shoes... but personally I doubt that Librela caused the signs described

there are online groups supporting every possible idea so.....

As far as aware we've not seen any adverse reactions, I certainly haven't. Using it quite frequently.

We have had one younger dog that the owner thought became polydipsic. No incontinence.

Have reported it as a suspected adverse reaction as I think perhaps the next issue of the data sheet will have more adverse reactions on it!

But generally very well tolerated and really useful.

Urinary incontinence a relatively common s/e. Heard of a couple that had vestibular syndrome after that didn’t recover in the usual interval of a few weeks
