 We have had 3 anaesthetic incidents in the last week in the dental room and we wish to make sure that we are not missing anything.

&lt;p&gt;Apologies for leaving out the details, I was trying to keep the message as brief as possible.&amp;nbsp;&lt;/p&gt;
&lt;p&gt;But to answer your questions;&lt;/p&gt;
&lt;p&gt;&lt;/p&gt;
&lt;p&gt;1. the cats were given medetomidine 80ug/kg , Ketamine 2.5 mg/kg and &amp;nbsp;methadone 0,025mg/kg ie&amp;nbsp;&lt;/p&gt;
&lt;p&gt;Dogs medetomide 0,02mg/Kg, methadone 0,2mg /Kg&lt;/p&gt;
&lt;p&gt;2. all cats and &amp;nbsp;received regional dental blocks with bupvicaine&lt;/p&gt;
&lt;p&gt;3. When they appeared light , the &amp;nbsp;Isoflurane was increased , and dental regional blocks were reapplied in the area we were working on at the time.&lt;/p&gt;
&lt;p&gt;4. last recorded&amp;nbsp;Body temp for each patient were, 36,9C, 37,9C and 38,5 C&lt;/p&gt;
&lt;p&gt;5. The bradycardia was recorded but not acted upon until the call was made and anaesthesia was reversed. Isoflurane turned off and antipamezole given and &amp;nbsp;iv.. epinephrine&amp;nbsp;was administer iv to the cat that died.&lt;/p&gt;
&lt;p&gt;6. last recorded blood pressures were. 70/40, 70/ 30 and 140/100 (however the last one &amp;nbsp;was 15 minutes before the incident)&lt;/p&gt;
&lt;p&gt;I hope I have written it in a manner that is clear?&lt;/p&gt;

&lt;p&gt;Sorry to hear about those incidents and in particular the patient you lost.&lt;/p&gt;
&lt;p&gt;I think it would be interesting to know which doses were given to those patients and via which route to have a better picture of what could have happened. Did you perform any local blocks on those patients?&lt;/p&gt;
&lt;p&gt;How was the lighter plane of anaesthesia managed in those cases? Were top-ups administered?&lt;/p&gt;
&lt;p&gt;Were those patients hypothermic at the time of the event? You say that the bradycardia occurred over a period of 20 minutes - how did the patients respond to your interventions to treat bradycardia in that time?&lt;/p&gt;
&lt;p&gt;Finally, did you have any blood pressure measurements around the time of the incidents?&lt;/p&gt;
&lt;p&gt;&lt;/p&gt;
&lt;p&gt;I apologise for asking more questions than giving answers, but I think that all those factors can be contributing to complications and as part of your review, it would be important to consider those elements.&lt;/p&gt;
