Z/D would be my go to, or depending on the stool consistency of diet it is on now and your biochem results including tli/b12/folate-then go to hills id-either the low fat or normal depending on your blood results. Good local hygiene-get them in and shave close but&amp;nbsp; not to 40 surgical level- the whole perineal area-have the tail close clipped&amp;nbsp; but not the tip. If owner declines clipping then bandage or soft cotton-sock the tail and trim a line matching the tail area on the perineum so you have ventilation but also better applicaition compliance by the owner and better persistence of the topical tacro. Flagy - yes this is one of the few condition I would think is appropriate for metronizaole use-and add in your cylosporin.&lt;/p&gt;
&lt;p&gt;Attention to the bedding as well-make sure none of those horrendous hessian beds owners so love here in Australia-you will never heal any skin condition with a dog lying on those. He was one of the worst cases I had seen with a bum like raw hamburger meat and one of the fistulae was the length of my finger, he had another about an inch and a half deep and various others. He also had to be sedated flat to examine each time. IIRC I had him on 7.5mg /kg Atopica, azathioprine and, to start with , metronidazole for about a month. Can&amp;#39;t remember if I had him on pred as well. I put him on Anallergenic as well and insisted they were really strict with the food trial. Cost a fortune for a 45kg dog, and took 6 months for him to come right, but he did really well in the end and I was able to wean off all meds and keep him on the hydrolysed protein diet alone. I have yet to find any surgery give anything other than temporary relief.
