I have a 11yo, MN, Yorkshire Terrier with transitional cell carcinoma, he is actually doing very well and has already survived a year since his symptoms first started. This is probably because his TCC is not really into his bladder neck yet but

RE: Can't clear E.Coli UTI in dog with transitional cell carcinoma
please find a link to the ISCIAD guidelines that includes Subclincal bacteria. www.iscaid.org/.../Urinary-guidelines.pdf
Hi Ian,

Thanks for interesting post.

Do you know is there a reason that oxytet was omitted from these guidelines? Someone mentioned it as odd in this forum before and I've always wondered since.

Cheers!

RE: Can't clear E.Coli UTI in dog with transitional cell carcinoma

HI Kara

interesting post

There is increasing recognition of asymptomatic bacturia in which we don't treat . These are patients in which you have a positive culture but have no clinical signs.

This is similar to humans as they now may not treat a bacteriuria in a diabetic or even a renal transplant case. If they feel not symptomatic in the interest of minimise overuse of ABi and selection of a resistant bug.

The challenge with your case is that the TCC may be the cause of the any stranguria etc . so clincal signs could be due to UTI or TCC.

But if the dog was asymptomatic if it was me i would monitor .

It unexpected that with the prolong courses and the persisting bug the sensitivity profile hasnt change.

please find a link to the ISCIAD guidelines that includes Subclincal bacteria. www.iscaid.org/.../Urinary-guidelines.pdf

In this text it described bacteriuria only with no pyuria but i do know that in some human guidelines they also have patients with bacteriuria and pyuria and are asymptomatic etc that they don't treat

All the best

Anecdotal, but with these persistent E. coli UTIs I've found convenia works quite well, as does enrofloxacin.

RE: Can't clear E.Coli UTI in dog with transitional cell carcinoma
I then tried TID dosing with amoxyclav and that seemed to work but then a few days after abs finish he is positive again.
I'm not a lot of help, but watched a webinar recently about urinary tract infections and culturing. The take home message seemed to be that this specialist cultures far far less than they ever used to simply because of this issue, where a course of antibiotics failed to clear a culture result. She inferred that you need to consider the case, and if it was in fact causing an issue. The case she quoted was 'you get a positive urinary culture but no clinical signs and has had antibiotics, what are you going to do?'

I'm sorry I'm not more use than that, but the message that I took was treat the dog not the urinary culture

    Hopefully helps (a bit?)
