&lt;p&gt;Ideally. Don&amp;#39;t know about your clientele, but I get clients who do or don&amp;#39;t stop medications without consulting me first.&amp;nbsp;&lt;/p&gt;

&lt;p&gt;Not if they have an effect. If somebody claims a given medication doesn&amp;#39;t have side effects, my first thought always is, does it have a wanted effect? But some meds will have more or more severe adverse effects than others.&amp;nbsp;In case of solensia we have a relatively good understanding of the adverse effects of the alternatives, but little about the mid to long term effects of solensia. To repeat myself, I&amp;#39;m not saying it shouldn&amp;#39;t be used. Only that, in my opinion, it shouldn&amp;#39;t be the first option, for the reasons given above.&lt;/p&gt;
&lt;p&gt;&lt;span&gt;Though I admit, I don&amp;#39;t get your point with above statement.&amp;nbsp;&lt;/span&gt;That other medications will/ might have adverse effects can hardly be an argument to ignore potential adverse effects of one specific med.&lt;/p&gt;

&lt;p&gt;Spontanously cats and injection related fibrosarcoma would come to mind... ;-)&lt;/p&gt;
&lt;p&gt;In humans it is accepted that pancreatitis can be a rare adverse event from NSAIDs. Not quickly or easily resolved and going along with a strong negative effect on quality of life, at least short term. &lt;p&gt;I&amp;#39;m going to speak as an owner and also a view that I know is shared by other vets. I want my cat to be comfortable, I&amp;#39;m not concerned about longevity, it&amp;#39;s all about quality of life.&lt;/p&gt;
&lt;p&gt;We have a 14 year old cat that was simply not enjoying life as he did before. Custard has a finite life as we all do. If I can make his life more comfortable for 6 months and he dies a little quicker then so be it. I often quote the Australian study on Metacam where cats in renal failure live longer on a 1/4 dose of metacam because they feel better, eat better. Life&amp;#39;s for living and that includes our feline friends&lt;/p&gt;

&lt;p&gt;One week post solensia in a 13 year old that has very limited range of movement all joints, spine etc. Even under ga, cannot free up but she is doing things now when all sorts of multimodal meds failed.&lt;/p&gt;
&lt;p&gt;on s/e well if we remember that some of the known s/e NSAIDs include fatal haemorrhage, kidney and heart failure, will repeat next month on QOL alone&amp;nbsp;&lt;/p&gt;
&lt;p&gt;The look is because I tried to touch her!! &lt;p&gt;&lt;span&gt;The other is possibly an unfounded worry on my account, but I can&amp;#39;t help wondering what long term effects it may have to knock out a growth factor that not only helps with nerve repair and regeneration, but also plays a role in wound healing, immune system, survival of pancreatic cells and more (at least in humans). For that reason I&amp;#39;d probably only would use it if all other options have been exhausted or the owner insists.&amp;nbsp;&lt;/span&gt;&lt;/p&gt;

&lt;p&gt;Our ginger cat has gone from strength to strength, it&amp;#39;s taken years off him. He&amp;#39;s used the litter tray once and is eating his head off, has gained weight as well. He no longer looks 14&lt;/p&gt;
