It’s the syringes that have two different scales on the same syringe that worry me more.

I discard the non-relevant syringe

Having to explain and demonstrate everything to a client is making even a 15 minute consultation impossible - it becomes like the radio disclaimers for insurance!

I had one on Monday. Post op cat spay. All clients are shown exactly how to use the loxicom syringe on discharge and what dose to give.

'Have you managed to give the loxicom ok?'

'Yes, although the dose confused me because it only said 'dose for 5kg', not the actual mls. So I looked on the leaflet and it said give 0.05mg/kg so I gave 2.5ml on the syringe.'

'But the syringe is marked in Kg, and 0.05mg/kg x 5kg is (kind of) 0.25ml anyway....?'

'Oh, yes, so it is, I'm not sure why I did that really ...'

Nothing made any sense. She'd been puzzled - so just worked out a dose for herself - but that didn't make sense either!!! Luckily no harm was done, but it reminded me that clients do the weirdest things!

Of course it can work both ways - I once typed a label for some ear drops (always in a rush - and my typing is notoriously dyslexic). I meant to say 'Apply two drops into the ears twice daily'. Unfortunately the 'e' of 'ears' got transposed to the end of the word.

I once had a client who was giving Pro-kolin paste per rectum, despite a label clearing stating orally.

I suppose one just has to assume that they are wuck fits until proven otherwise.

I once dispensed some Hibiscrub decanted into a medicine bottle for wound cleaning for a patient and couldn't find a ribbed bottle. I wrote on the label and said to the client 'Do not swallow'. She said to me some six months later she remembered my words as she swallowed it...

I agree, with medicines this is one area where you have to act like Gregory House and assume they are all idiots. I also carry a highlighter pen for drug labels for this very reason.

Akin to a practice showing people how to apply Advocate as they have seen cases where it's been put in the eyes and even rectally. I know of 3 cases of post op spays where a 6kg dog has had a 60kg dose, people simply don't think sometimes and this simple precaution can avoid this.

If you want me to put it more plainly, remember school Clive? You knew who the clever and the daft people were, presumably you were in the clever cohort. Just because they have a car and a house, doesn't change that.

We always send them out with one syringe. People do get them mixed up and don't use the scale. Sounds crackers but they seem ok with half or third full.
