Realise this is probably a bit late but here goes...

Sometimes hard to get all the things you want in one place - neuro is in such demand around here it can be very hard sometimes to get non-urgent cases seen in the last 12 months, but it is getting a bit better now. Often have to check with two or 3 places before someone can see our case.

We use North West Veterinary Specialists a lot (in Frodsham - not to be confused with North West Veterinary Referrals who used to be called NW MRI). Karen Crawford is on the internal medicine team there and she did her residency at Bristol who have a strong feline medicine department. No opthalmologist though.

Liverpool Uni SATH are good but it can be hard to get anything in to see them as they are usually swamped.

Could also consider Paragon Referrals in Wakefield.

Where in the North West - it stretches from Cheshire to Cumbria?

Near(ish) to me, with all those disciplines, I'd suggest trying Rutland House Referrals, Dogwood, Northwest Veterinary Specialists and Woodcroft.

For individual disciplines, I would have suggested some others.
