Raised SDMA and urea don&amp;#39;t improve the prognosis. Vital to check for and rule out potential treatables like Demodex, Malassezia, and dermatophytes. Steroids would be tempting to improve quality of life, but may not be a good idea if kidney disease, active Proteus, and deep skin infection. Frequent washing, salt water, Isaderm, antibacterial wipes, and warn owner of underllying probelms. Appropriate antibiotics if no response. Clip all the hair out so you can visualise what is actually going on better. Looks inflammatory so steroids are a great idea. If there are yeast on your impression smears you can use surolan topically. If cocci I would use isaderm. If no organisms then cortavance if you have any or Elocon (mometasone) if not. Daily bathing in a nice chlorhexidine shampoo like Douxo, or the Douxo (or CLX) wipes. Check the other paws, I bet you&amp;#39;ll see mild inflammation in between the pads of those, just not as bad.&amp;nbsp;&lt;/p&gt;

&lt;p&gt;I only briefly saw it yesterday, but my impression was that yes this is painful. The dog is 3/10 lame on this&lt;/p&gt;

&lt;p&gt;Not yet.... well not until today when I saw it and I said &amp;#39;give it a shot of dex&amp;#39; whch we did. I&amp;#39;m going to call the client on Friday&lt;/p&gt;
