Apologies for the long delay in replying here.

Thanks for the colour Doppler and 2D images. On the second video, in the 2D loop; there are points where the lumen appears to be patent. The colour Doppler sample box in the first video loop is a little too proximal to the abnormality to appreciate if there is any loss of flow.

It is one of those cases where I would love to take a look see myself but I suspect that with some manipulation and rotation of the probe, you may well be able to lose this completely. My first suspicion is this is a slightly oblique view through the ventricular myocardium. But I would suggest a recheck and monitor. An obstructive lesion may form anywhere but its appearance seems atypical for this in this location where the ones I have seen are more homogenous, less hyperechoic and more rounded

Out of interest, what was the cause of the whistle murmur you identified? Was there any evidence of mitral valve disease?

If you do repeat echo, please update us with what you find!

Kind regards,

The mass seems to disappear/appear with the heart beats so I’m not sure if it’s in the lumen or outside of it.
