Cranial mediastinal exfoliating T-cell lymphoma with secondary paraneoplastic eosinophilic inflammation. Diagnosed via radiography and cytology/flow cytometry on effusion.

&lt;p&gt;Most intermediate to high grade lymphomas would be treated initially with a CHOP-based protocol, even T-cell, often adding l-asparaginase in at the beginning with cranial mediastinal disease, to help to reduce the mass effect quickly, if it is compromising breathing. If I do not get a good response within the first cycle in a T-cell, I will probably switch to lomustine, often along with l-asparaginase. If it is low grade I sometimes consider surgery and/or radiation to get a more rapid resolution of the mediastinal mass.&lt;/p&gt;
