Going to go back to basics next week, re-take history, re-do full CE and think about whatever the up to date problem list is at that point! Wouldn&amp;#39;t worry too much re protein levels if not symptomatic, we&amp;#39;ve had a few odd rssults in the past which didn&amp;#39;t match. Can get referred liver pain to most abdominal places. Would crack out the roids personally, otherwise normally will resolve.&lt;/p&gt;

&lt;p&gt;- hypoalbuminaemia&lt;/p&gt;
&lt;p&gt;- anaemia&lt;/p&gt;
&lt;p&gt;So I&amp;#39;d look at urine - incl UPC - try and r/o PLN as cause for low albumin. Could have eg pyelonephritis on top of something else (sometimes I&amp;#39;ve found cats with pyelo are non specifically sore around the hindquarters).&lt;/p&gt;
&lt;p&gt;Try and classify anaemia in terms of reg or non reg - what&amp;#39;s FeLV /FIV status?&lt;/p&gt;
&lt;p&gt;BAST in case marked liver disease but so marked it no longer raises the liver enzymes (although end stage liver looks odd on ultrasound and small) which could fit with low glucose and low albumin. I guess a PSS is possible too given age.&lt;/p&gt;
&lt;p&gt;Then I guess consider GI disease - a pancreatitis or cholangitis on top of eg pre exsisiting IBD may explain things.&lt;/p&gt;
&lt;p&gt;Is he a hunter? Neospora or toxo can do some weird things. I&amp;#39;ve had young cats with toxo with almost a myositis with it.&lt;/p&gt;
&lt;p&gt;Lymph nodes ok? Always have lymphoma somewhere on my list.&lt;/p&gt;
&lt;p&gt;Could it be 2 things- underlying illness not previously diagnosed and then something on top eg neutrophilic cholangitis?&lt;/p&gt;
&lt;p&gt;Good luck.
