&lt;p&gt;I didn&amp;#39;t drain the pleural effusion. That was the plan if frusemide not improving things.&lt;/p&gt;
&lt;p&gt;Left ventricle was difficult to image from the right side so I couldn&amp;#39;t fully assess its contractility. From the quick left sided scan [dog not happy in left lateral], the left ventricle didn&amp;#39;t seem grossly dilated. That was my rationale for ACEi rather than pimobendan.&lt;/p&gt;
&lt;p&gt;Checking on Friday and adding digoxin if rate not controlling. Not insured so I will take your advice about relative improtance of drugs.&lt;/p&gt;

&lt;p&gt;Yikes. Yes, I&amp;#39;d give clopidogrel. I&amp;#39;d also have on pimobendan in almost all cases, plus probably digoxin as I find diltiazem is rarely enough on it&amp;#39;s own for rate control especially at that rate of AF. If you need to drop a tablet, I&amp;#39;d stop the benazecare. Have you drained the pleural effusion?&lt;/p&gt;
