 I&amp;#39;ve heard some anecdotal talk about the use of using diluted maropitant intranasally in cats with chronic idiopathic upper respiratory issues. When we worked her up the PCR was negative for Chlamydia, mycoplasma, Calici band herpes. Obviously she may not have been shedding at the time of swabbing.&lt;/p&gt;
&lt;p&gt;The CT scan showed BL chronic otitis media and rhinitis. There was occulsion of the nasopharynx by fluid and inflamed mucosa and soft tissue mass effect on the RHS...could have been fluid/pus/polyp/other mass.&lt;/p&gt;
&lt;p&gt;The owner is super keen. &lt;p&gt;I assume that Chlamoydophila is recrudescent and is not always detected. I think Mycoplasma can also be implicated, can it not? I attempt to&amp;nbsp;share with&amp;nbsp;the owners the idea of preemptive controlling of the flare-ups using&amp;nbsp;a multimodal approach, and make this teamwork where owners are the most important carers.&amp;nbsp;My concoction for the&amp;nbsp;chronic URT infection in cats includes&amp;nbsp;nebulisation +/-bisolvon BID-SID or steam inhalation with care in the shower room for 20-25 min; bisolvon in food and in water, encouragement to focus on stringent hygiene at home to reduce reinfection, reduce excessive stress including monitoring intra-colony dynamics, separation from other cats and using other anti-stress strategies. I also use Meloxicam. Doxycycline 10mg/kg SID for 4-6 weeks is my AB&amp;nbsp;of choice. I dispense 100mg tablets for the course to prepare a suspension and keep the suspended doxy in the fridge. I have come across an oral liquid form of doxy but this does not seem to be commonly available, it&amp;rsquo;s cheap and can be ordered.&amp;nbsp;As a&amp;nbsp;second AB, when this is an option, I offer Cefovecin q12-14d for the course of 3 injections, which I find helpful strategy for the follow up checks. I wonder if any of these is of help. Cannot&amp;nbsp;share the personal experience on Maropitant intranasally&amp;nbsp;but really intrigued by it. &lt;p&gt;There is probably a way of writing/issuing a prescription to try and restrict the client&amp;#39;s (possibly) injudicious use of antibiotics. I would be wary of a repeatable antibiotic prescription. I have a case at work: chronic sinusitis in a middle aged cat, lives in a colony at a dog rescue centre (!) .&amp;nbsp; Last year, she had a CT&lt;/p&gt;
&lt;p&gt;scan and anasal swab which showed bordetella. She responded well to a 4 week course of Marbofloxacin and 2 weeks of meloxicam.&lt;/p&gt;
&lt;p&gt;&lt;/p&gt;
&lt;p&gt;However ( as they do) she has flare ups of blocked nostrils and snottiness. She can go from normal and no stertor at all,&amp;nbsp; to quickly needing a few days of IVFT because poorly.&lt;/p&gt;
&lt;p&gt;I have tried using meloxicam at first signs but it didn&amp;#39;t help. Nor did amoxyclav. She has been getting 2 weeks of marbofloxacin occasionally.&lt;/p&gt;
&lt;p&gt;Owner pressuring me into putting marobofloxacin on repeat prescription (suspect he would like it permanently). I&amp;#39;m not comfortable with this but I don&amp;#39;t seem to have a better option.&lt;/p&gt;
