&lt;p&gt;I think this depends a lot on the clinical symptoms of the patient. I will sometimes use metronomic in patients with minimal clinical signs but if markedly symptomatic you tend to need something else first (stent, conventional chemo etc).&lt;/p&gt;
&lt;p&gt;More often I use metronomic if there has been a good response to conventional chemotherapy to see if we can maintain that benefit - I have typically used chlorambucil for that purpose.&lt;/p&gt;
&lt;p&gt;Sure this is a situation where everybody does something a bit different!&lt;/p&gt;
