&lt;p&gt;&amp;quot;Tissue plasminogen activator (tPA) is a naturally occurring fibrinolytic agent found in vascular endothelial cells and is involved in the balance between thrombolysis and thrombogenesis. It exhibits significant fibrin specificity and affinity. At the site of the thrombus, the binding of tPA and plasminogen to the fibrin surface induces a conformational change that facilitates the conversion of plasminogen to plasmin and dissolves the clot&amp;quot;&lt;/p&gt;
&lt;p&gt;So as it appears to work within the clot Tranexamic acid may work locally if it can dissolve into the clot? So I presume more effective if already in the blood and is incorporated into the clot as it forms?&lt;/p&gt;

&lt;p&gt;We had an interesting case recently with an american bully female who bled extensively throughout the season. She was spayed and was fine at the time of surgery. Started bleeding copiously 4 days after surgery from the abdominal wound. No obvious abdominal&amp;nbsp; haemorrhage on ultrasound. Had tranexamic acid inj and continued with tablets.
